肿瘤患者输液港自我管理行为现况及影响因素分析

Status and influencing factors of self-management behavior in cancer patients with implantable venous access ports

摘要: 目的:调查肿瘤患者输液港自我管理行为现况并分析影响因素,为提高患者自我管理能力提供参考。方法:采用便利抽样法,于2022年10月至2023年10月选取8个省、自治区的19家医院812例留置输液港肿瘤患者,应用癌症患者输液港自我管理行为评估量表进行问卷调查,利用多元线性回归分析影响因素。结果:肿瘤患者输液港自我管理行为得分为(122.90±19.16)分,得分最低的维度是“维护信息获取”(得分率为71.9%)。回归分析结果显示,影响因素包括主要疾病诊断、文化程度、婚姻状况、家庭居住地、留置天数、患者是否参与置港部位的选择、担心输液港可能会损坏、输液港影响社交活动、对输液港益处的感知和满意程度(P<0.05)。结论:肿瘤患者输液港自我管理行为良好,但维护信息获取能力需要加强。医护人员应鼓励患者参与置港决策,并提升患者对输液港益处的认知和满意程度,从而提高其自我管理行为水平。

Abstract: Objective: To investigate the current status and explore the influencing factors of self-management behavior in cancer patients with implantable venous access ports, providing reference for improving the self-management behavior. Methods: From October 2022 to October 2023, a convenience sample of 812 cancer patients with implantable venous access ports from 19 hospitals in 8 provinces or autonomous regions was investigated by the tumor patients with implantable venous access ports self-management behavior assessment scale. Multiple linear regression analysis was used to identify influencing factors. Results: The score of self-management behavior for implantable venous access ports was 122.90±19.16, with the lowest scoring dimension being "access to maintenance information" (scoring rate was 71.9%). The regression results revealed that the factors influencing self-management behavior in cancer patients with implantable venous access ports included primary disease diagnosis, education level, marital status, family residence, duration of placement, involvement in the selection of port placement site, concerns about damage to the port, impact of the port on social activities, perception of the benefits to port, and satisfaction with the port (P<0.05). Conclusion: Overall, cancer patients has a relatively high level of self-management behavior for implantable venous access ports, but there is still room for improvement in terms of access to maintenance information. Healthcare providers should encourage patient participate in implanting decision-making, enhance patients' awareness of the benefits of ports and improve their satisfaction, so as to promote their self-management behavior.
