护理质量现场督查问题反馈模式的构建及应用

Construction and application of feedback model for on-site inspection of nursing quality

摘要: 目的:构建护理质量现场督查问题反馈模式并验证其效果,以期为临床护理质量督查提供指导。方法:以SAID反馈沟通模型为理论基础,运用文献分析、半结构访谈法及问卷调查法明确督查者和被督查者对护理质量现场督查的态度、感受及需求,采用专题小组讨论法形成护理质量现场督查问题反馈条目池;采用德尔菲法在全国范围内选取21名专家进行两轮函询,形成护理质量现场督查问题反馈模式并在两家医院应用,比较模式应用前后督查者和被督查者对护理质量现场督查过程的评价。结果:形成的护理质量现场督查问题反馈模式包括明确问题、陈述情境、说明影响、指导改进4个环节和16项具体要求。两轮专家函询问卷有效回收率分别为91.7%、95.5%;专家权威程度系数分别为0.90、0.92;肯德尔和谐系数分别为0.17、0.24(P<0.05);模式应用后督查者和被督查者对现场督查过程的评价得分高于模式应用前(P<0.01)。结论:护理质量现场督查问题反馈模式形成过程科学、规范,可作为护理管理者进行护理质量现场督查问题反馈的参考依据。

Abstract: Objective: To construct the feedback model of on-site nursing quality supervision and verify its effect, in order to provide guidance for clinical nursing quality supervision. Methods: Based on the SAID feedback communication model, literature analysis, semi-structured interview and questionnaire survey were used to clarify the attitudes, feelings and needs of supervisors and supervised on on-site supervision of nursing quality, and thematic group discussion was used to form the item pool of feedback mode on on-site supervision of nursing quality. Twenty-one experts were selected nationwide and two rounds of correspondence consultation were conducted by Delphi method. The feedback model was formed and applied in the two hospitals, and the evaluation of supervisors and supervised on on-site supervision of nursing quality before and after the application of this model was compared. Results: The feedback model of on-site supervision of nursing quality was formed, including 4 links and 16 requirements, and the 4 links were: identifying the problem, stating the situation, illustrating the impact, and direct improvement. The effective questionnaire recovery rates were 91.7% and 95.5%, respectively. The degree of expert authority was 0.90 and 0.92, respectively. Kendall harmony coefficients of experts were 0.17 and 0.24 (P<0.05), respectively. After the application of the model, the score of on-site nursing quality supervision of the supervisors and the supervised were higher than those before (P<0.01). Conclusion: The formation process of the feedback model of on-site nursing quality supervision is scientific and standardized, and it can be used as a reference for nursing managers to feedback problems in on-site supervision of nursing quality.
