基于患者全息视图的静脉血标本检验前质量管理路径的构建与应用

Construction and application of a quality management pathway for venous blood specimens before the test based on the holographic view system of patients

摘要: 目的:构建基于患者全息视图的静脉血标本检验前质量管理路径并评价其应用效果,以改进静脉血标本管理质量。方法:护理部联合医务处、检验科、后勤管理中心、临床科室、信息中心多部门,基于医院患者全息视图信息,形成检验标本轨迹展示-问题信息警示-临床处理反馈-管理信息推送-持续质量改进的检验前静脉血标本业务管理流程,构建检验前静脉血标本质量管理路径,整体架构由临床业务模块、信息处理模块和三级专项质量管理模块构成。管理路径应用3个月后评价其效果。结果:与前一年同期相比,基于患者全息视图的静脉血标本检验前质量管理路径应用后,静脉采血不合格标本发生率由0.31‰下降至0.19‰,标本留取质量不合格发生率由0.17‰下降至0.07‰,静脉血标本检验前周转时间缩短,标本延时送检率下降,差异均具有统计学意义(P<0.05)。结论:基于患者全息视图的静脉血标本检验前质量管理路径实现了对标本全流程的追踪管理,为临床医、护、技提供了便捷的查询渠道,减少了沟通时间成本,有效提升了检验前标本质量,降低了静脉采血不合格标本发生率。

Abstract: Objective: To improve the collection, storage and delivery quality of venous blood specimens, shorten the time for delivering specimens, and reduce the incidence of unqualified specimens. Methods: The nursing department collaborated with medical affairs department, laboratory department, logistics management centre, clinical departments, and information department. The core focused on specimen collection and storage quality, barcode quality, collection and storage method quality, specimen delivery quality, data analysis, and continuous improvement. Based on the hospital's holographic view system of patients, this study established a workflow for venous blood specimen management before the test, consisting of specimen trajectory display, problem information warning, feedback for clinical processing, management information push, and continuous quality improvement. A quality management pathway for venous blood specimens before the test was constructed, which consisted of three modules. The effectiveness of the pathway was evaluated after a 3-month application period. Results: In comparison to the same period last year, after the implementation of the quality management pathway for venous blood specimens before the test, the rate of unqualified venous blood specimens decreased from 0.31‰ to 0.19‰. The rate of unqualified specimens due to improper collection and storage decreased from 0.17‰ to 0.07‰. The turnaround time was shortened, and the delay rate of specimen transportation decreased, and the differences were statistically significant (P<0.05). Conclusion: The quality management pathway for venous blood specimens before the test based on the holographic view system of patients achieved tracing and management of the entire specimen process, providing a convenient query channel for clinical medical staff, reducing communication time costs, effectively improving the quality of venous blood specimens before the test, and reducing the rate of unqualified specimens.
