糖尿病足溃疡负压伤口治疗的证据总结

Evidence summary for the negative pressure wound therapy of diabetic foot ulcers

摘要: 目的:总结糖尿病足溃疡患者负压伤口治疗的相关证据,旨在形成规范化的治疗方案,为医务人员提供借鉴和参考。方法:系统检索国内外相关数据库、指南网和专业协会网站中有关糖尿病足溃疡患者负压伤口治疗的临床决策、指南、专家共识、证据总结、系统评价、随机对照试验,并对符合纳入标准的文献进行质量评价,遵循证据汇总原则提取与整合证据。结果:共纳入12篇文献,包括1篇临床决策、4篇指南、3篇专家共识、1篇证据总结、2篇系统评价、1篇随机对照试验。形成应用目的、应用时机、终止时机、负压参数设定、负压引流材料及更换、皮肤保护、效果评价7个维度共20条证据。结论:医护人员应根据糖尿病足溃疡患者负压伤口治疗的临床证据,并结合患者的个体情况制定个性化治疗方案,进而改善糖尿病足溃疡患者的临床结局。

Abstract: Objective: To summarize the relevant evidence on negative pressure wound therapy in patients with diabetic foot ulcers, aiming to establish standardized treatment plans and provide guidance and reference for healthcare professionals in clinical practice. Methods: All relevant content related to negative pressure wound therapy in patients with diabetic foot ulcers in relevant databases, guideline networks and websites of professional associations were systematically retrieved, including clinical decision-making, guidelines, expert consensus, evidence summaries, systematic reviews, and randomized controlled trials. The quality of the literature that met the inclusion criteria was evaluated, and the evidence was extracted and integrated. Results: A total of 12 articles were included, including one clinical decision-making, four guidelines, three expert consensus, one evidence summary, two systematic reviews, and one randomized controlled trial. A total of 20 pieces of evidence were formed in seven dimensions, including application purpose, application time, termination time, negative pressure parameters setting, negative pressure drainage material and replacement, skin protection and effect evaluation. Conclusion: Healthcare professionals should formulate personalized treatment plan according to the clinical evidence of negative pressure wound treatment in patients with diabetic foot ulcers, so as to improve their clinical outcomes.
