化疗相关性口腔黏膜炎风险预测模型的系统评价

Risk prediction models for Chemotherapy-Induced Oral Mucositis: a systematic review

摘要: 目的:系统评价化疗相关性口腔黏膜炎风险预测模型,为临床选择合适的预测模型或开发新模型提供参考依据。方法:系统检索数据库中发表的化疗相关性口腔黏膜炎风险预测模型的相关文献,分析纳入研究的偏倚风险和适用性。结果:共纳入8篇文献,包含12个模型,其中11个模型报告了受试者工作特征曲线下面积,为0.61~0.97。5个模型报告了校准度,仅1个模型进行了外部验证。年龄、化疗方案、化疗次数、体质量指数、放射剂量、口腔清洁度和吸烟史为模型中重复报告的预测因子。纳入研究的适用性均较好,但偏倚风险高。结论:化疗相关性口腔黏膜炎风险预测模型整体的区分度和适用性良好,但偏倚风险高,各模型的预测因子差异较大。今后还需要根据不同肿瘤类型开发更具有针对性、偏倚风险低的预测模型,并进行内部和外部验证。

Abstract: Objective: We systematically evaluate the risk prediction models for Chemotherapy-Induced Oral Mucositis in order to provide reference for clinical staff to select appropriate prediction model or develop a new one. Methods: The studies about risk prediction models for Chemotherapy-Induced Oral Mucositis were searched. Two researchers independently screened the literature, extracted the data, and evaluated the bias risk as well as applicability of the included study. Results: A total of 8 studies including 12 models were identified. Among them, 11 models reported the area under the ROC curve which was 0.61-0.97, and 5 models reported the calibration. Only one model was externally validated. Predictors reported repeatedly of the models were age, chemo-therapy regimens, times of chemotherapy, body mass index, radiation dose, oral hygiene and smoking history. All included studies showed good applicability, but had high risk of bias. Conclusion: The risk prediction models for Chemotherapy-Induced Oral Mucositis have good discrimination and applicability, but the risk of bias is high. And the predictive factors of each model are different. In the future, it is necessary to adjust the existing models or develop more targeted, low bias risk prediction models according to different tumor types under the guidance of methodological statements, and conduct internal and external validation.
