抗阻呼吸训练对颈段脊髓损伤患者呼吸功能和膈肌移动度的影响

Effects of an anti-resistance respiratory training program on respiratory function and diaphragmatic mobility in patients with cervical spinal cord injury

摘要: 目的:分析抗阻呼吸训练对颈段脊髓损伤患者呼吸功能和膈肌移动度的影响。方法:选择2019年—2021年在海南省某三级甲等医院收治的80例颈段脊髓损伤患者作为研究对象,将患者随机分为对照组和观察组每组有40例,对照组采用常规康复训练,观察组在对照组基础上采用抗阻呼吸训练,对比两组患者的呼吸功能指标、膈肌移动度、Borg评分、生活质量、临床疗效及并发症发生情况。结果:干预后,观察组患者的总干预有效率为95.00%,明显高于对照组的75.00%(P<0.05)。观察组患者的最大通气量、用力肺活量、第一秒用力呼气量、膈肌移动度明显优于对照组(P均<0.05),Borg评分及呼吸困难、疾病影响、活动能力评分明显低于对照组(P均<0.05)。结论:抗阻呼吸训练可有效提高颈段脊髓损伤患者的呼吸功能、膈肌移动度,保持患者呼吸通畅,并提高生活质量,安全性较高,值得在临床推广使用。

Abstract: Objective: To examine the effects of an anti-obstructive breathing training progam on respiratory function and diaphragm mobility in patients with cervical spinal cord injury. Methods: A total of 80 patients with cervical spinal cord injury admitted to a tertiary grade A hospital in Hainan province from 2019 to 2021 were recruited. Patients were assigned into the experimental group (n=40) or the control group (n=40) randomly. Patients in the control group received routine rehabilitation training, while patients in the observation group received the resistance breathing training program. The respiratory function index, diaphragm mobility, Borg score, and quality of life were compared between the two groups. Besides, the clinical efficacy and complications of the two groups were also observed. Results: The total clinical effective rate of the experimental group (95.00%) was significantly higher than that of the control group (75.00%) (P<0.05). In the experimental group, the MVV, FVC, FEV1 and diaphragm mobility were significantly higher than those in the control group. Moreover, the Borg score, dyspnea, disease impact and mobility score were significantly lower than those in control group (all P<0.05). Conclusion: The anti-obstructive breathing training program can effectively improve the respiratory function and diaphragmatic mobility of patients with cervical spinal cord injury, maintain patients' respiratory patency, and improve patients' quality of life. The program is safe and worthy of clinical promotion.
