常见临床护理操作技术难度系数的评定

Evaluation of the technical difficulty coefficient of common nursing operations

摘要: 目的:测定天津市常见临床护理操作技术难度系数。方法:便利选取2 082名护理管理人员及高年资护士,采用问卷调查法对84项临床常见护理技术操作进行难度评分,采用加权线性和法计算各项操作项目难度系数,最后通过专家会议法进行修订。结果:调查人员的熟悉程度、判断系数和权威程度分别为0.66、0.88和0.77,评分者间信度为0.455,临床护理操作技术难度排前三位的分别是心脏除颤、中心静脉导管维护、洗胃机洗胃,后三位的分别是测体温、测体质量和测身高。结论:临床护理操作技术难度差别较大,通过科学测定护理技术难度,为科室总体技术难度评定、工作量统计等提供基础,进一步为科室合理配置人力资源、制定绩效考核方案等提供依据。

Abstract: Objective: This study aimed to determine the technical difficulty coefficient of common nursing operations in Tianjin. Methods: Totally 2 082 nursing administrators and senior nurses were selected through convenient sampling, and were offered a questionnaire produced on the basis of relevant literature reviews to evaluate the difficulty score of 84 common nursing operations, which were decided first according to the technical difficulty coefficient of each operation item through weighted linear sum method and then revised through Delphi method. Results: The familiarity, judgment coefficient and authority of the respondents were 0.66, 0.88 and 0.77, respectively, and the inter-rater reliability was 0.455. The three most difficult nursing operations weredefibrillation, care and maintenance of central veins and gastric lavage through machines, while the three least difficult ones were body temperature measurement, body weight measurement and height measurement. Conclusion: The difficulty coefficient of clinical nursing operations varies greatly. Scientific measurement of technical difficulty of nursing operations provides a basis for the overall technical difficulty assessment and workload analysis of clinical departments, and offers further theoretical support for the rational allocation of human resources and the establishment of performance appraisal programs.
