Internet+ Nursing Services as an emerging thing has been tested in six municipalities or provinces in China with relevant support documents having been issued in various places. But as far as the current development goes, reevaluation of services orienting to social needs, redefining the capacity of nurses in offline services, the standardization of offline services and offering support to perfect a data platform of the care system have become the key point to the survival and development of "Internet+ Nursing Services". Accelerating the development of nursing standardized language systems and achieving a seamless integration with electronic medical records as well as formulating the standards of nursing service industry that suit the new service environment and conditions is of top priority for the innovative development of "Internet+ Nursing Services".

Effect of mobile internet management in the continuous nursing of children with Congenital Heart Disease surgery

Objective: To explore the effect of mobile internet management in the continuous nursing of children with Congenital Heart Disease surgery. Methods: A total of 160 children with Congenital Heart Disease surgery from December 2017 to April 2018 were randomly divided into experimental group and control group, with 80 cases in each group. The control group were treated according to routine methods, while the experimental group were applied the mobile internet management to the continuous care except the routine nursing follow-up mechanism. Children in both groups were followed up for 12 months after discharge, and the weight of children, re-hospitalization rate, parental care skills, self-efficacy of the two groups were compared. Results: There was no significant difference in body weight when discharge between the two groups (P>0.05). The weight of the experimental group was significantly higher than that of the control group at 6 months and 12 months after discharge (P<0.05). The re-admission rate of children in the experimental group and the control group was 8.75% and 23.75%, respectively, showing no statistical defference (P<0.05) between the two groups. The parental care skills of the children in the experimental group at discharge, 6 months and 12 months after discharge were significantly higher than those in the control group (P<0.05). There was no significant difference in parental self-efficacy between the two groups at the time of discharge, 6 months and 12 months after discharge (P>0.05). Conclusion: The mobile internet management in the continuous care of children with Congenital Heart Disease surgery can promote the weight gain of children, improve the parental care ability of the children, and has a high value for improving the short-term prognosis of children.

To understand the current situation of medical waste management in the context of "Internet+ Nursing Service" and figure out the problems existing under present circumstances, which mainly included: the lack of relevant medical waste management system for home care services, the incomplete relevant provision of medical waste sorting and treatment facilities for home care services, and nurses appointed through the internet lacking knowledge on the management of medical wastes, etc. As Chinese "Internet+ Nursing Service" has just started, it is requisite to establish and improve the relevant medical waste management system, rationally configure the facilities of medical waste sorting treatment, strengthen nurses' understanding and publicity of regulations on medical waste management as well as the classification and disposal of medical waste so as to ensure the proper disposal of medical waste and avoid the harm to people and the ecological environment.

Application of follow-up management on cloud platform for home-based Peritoneal Dialysis

Objective: To improve follow-up management of patients receiving home-based Peritoneal Dialysis (PD) through employing a follow-up management system established on cloud platform. Methods: The follow-up platform employing cloud technology for home-based PD was officially launched in September 2018, which was used for patient admission, grading management, medical follow-up, follow-up evaluation, reminding of follow-up, intervention of complications, case closure and data analysis. Results: From September 1 to December 31 of 2018, 300 PD patients were recruited in our center and received a total of 1798 times of follow-up. The average times of follow-up increased from 5.19 to 5.99 after application of the follow-up management platform, and the participation rate of patients increased by 11.5%. The completion rate of PD diary increased from 46.7% to 62.3% (χ2=13.243, P<0.001). The time to figure out and take care of complications after they occurred by full-time PD nurses decreased from 24.5 (13.3, 36.0)h to 4.5 (2, 12) h (u=481.000, P<0.001). The overall satisfaction score of the follow-up platform service evaluated by PD patients was 4.37±0.69. Conclusion: The application of home-based PD cloud follow-up management platform not only improves the satisfaction and compliance of diary records of patients, but also increases the opportunity to spot and treat complications in time and work efficiency of full-time PD nurses.

Objective: To analyze the effects of Online To Offline (O2O) model in the health management of young and middle-aged patients with hypertension in community. Methods: The 262 young and middle-aged patients with primary hypertension to accept medical examination or treatment in a community health service center in Urumqi from February to May, 2017 were included and randomly divided into two groups. The patients in intervention group were with O2O model health management for 6 months, and those in control group were with community contract service. Results: The scores of hypertension knowledge and compliance of the intervention group were higher than the scores of the control group (P<0.05). The blood pressure of patients in both groups were lower after the intervention, and the antihypertensive effect was more obvious in the intervention group (P<0.05). Conclusion: The O2O model is benefit to improve the health management knowledge, the treatment compliance and the blood pressure control of young and middle-aged patients in community.

Objective: To investigate the continuing care needs and influencing factors in patients with Nasopharyngeal Carcinoma. Methods: A total of 205 Nasopharyngeal Carcinoma patients with radiotherapy and chemotherapy were investigated using the general condition scale, the self-designed nasopharyngeal cancer continuing care needs scale, and the Cancer Fatigue Scale. Results: The total score of continuing care needs was 66.38±7.17. The scores of continuing care needs from high to low were health education, daily care, complication care and psychological care. The influencing factors were physical fatigue, the number of symptoms, discharge time, and satisfaction of care. Conclusion: Nursing staff should strengthen the health education for patients with Nasopharyngeal Carcinoma, and provide personalized guidance to meet patients needs according to their continuing care needs and influencing factors.

Objective: To translate and revise the Second Victim Experience and Support Tool (SVEST), and analyze its reliability and validity. Methods: The SVEST was cross-culturally adapted to form a Chinese version of SVEST, which was used to investigate 382 medical staff of their psychological properties. Results: Exploratory factor analysis extracted 6 common factors, and the cumulative variance contribution rate was 69.985%. The Cronbach's α coefficient and the test-retest reliability of the total scale was 0.824 and 0.888, respectively. The Cronbach's α coefficient of each dimension was between 0.666 and 0.910. Conclusion: The Chinese version of the SVEST scale has good reliability and validity, and it can be used as a testing tool for the study of the second victims of medical institutions in China.

Objective: To determine the prioritizing topics and outcomes of nursing practice guideline for Emergency Percutaneous Coronary Intervention, so as to lay foundation for guideline recommendations. Methods: The priozitizing topics and outcomes was initially formed on previous literature research and target population interview, then three rounds of the Delphi method was adopted to determined the final prioritizing topics and outcomes. Results: The 7 prioritizing topics, including assessment and identification, condition monitoring, oxygen inhalation, safe transport, rehabilitation, skin preparation, and iodine allergy test, and 20 outcomes were finally determined in our practice guideline. Enthusiasm index of all the three rounds of Delphi was 100% and the authority indexes were above 0.90. Conclusions: The prioritizing topics and outcomes included in Nursing Practiing Guideline for Emergency Percutaneous Coronary Intervention were formed by previous literature research, target population interview and Delphi method, which gives directions to guideline development.

Objective: To translate the Illness Perception Questionnaire Revised for Dental (IPQ-RD) into Chinese and to test its reliability and validity. Methods: After contacting and obtaining the authorization of the author of the original questionnaire, we translated, translated back, adapt the culture of IPQ-RD, determined the dimensions and items of the Chinese version of the questionnaire, and formed the Chinese version of the IPQ-RD. Results: The content validity index of the single item of Chinese versionof IPQ-RD reached 0.80 or above, and the total item content validity index was 0.948.Factor analysis extracted 8 common factorswith the cumulative contribution rate of 80.378%. Each item in the corresponding common factors have a large factor load value (>0.4). The total Cronbach's alpha coefficient was 0.914 and the refraction reliability coefficient was 0.746. Conclusion: The Chinese version of IPQ-RD has good reliability and validity and could be used to evaluate the degree of disease perception among the caregivers of preschool children with caries.

Depression and risk factors of women in their second-third trimester of pregnancy in Shaanxi province

Objective: To explore the depression and risk factors of pregnant women in their second-third trimester in Shaanxi province. Methods: Totally 877 pregnant women were selected from 6 hospitals in Shaanxi province using convenience sampling method. The social demographic questionnaire, the Edinburgh Postnatal Depression Scale (EPDS), the Pregnancy Stress Scale (PPS) and Perceived Social Support Scale (PSSS) were applied. Results: The 26.9% had antenatal depressive symptoms and average score of EPDS was (9.32±5.24). Hierarchical multiple regression analysis showed the risk factors were housing satisfaction (OR=1.429, P<0.01), relationship with partner (OR=1.543, P<0.01), relationship with mother-in-law (OR=1.572, P<0.01), social support (OR=0.929, P<0.001) and pregnancy stress (OR=1.040, P<0.001). Conclusion: The incidence of depression of pregnant women in the second and third trimester is high in Shaanxi province, which is related to social demographic factors, pregnancy stress and social support status. Nurses should focus on the pregnant women with depressive symptoms. Comprehensive care including mental health guidance during pregnancy should be provided to help them to successfully pass the pregnancy.

Burdens of main caregivers of children with Congenital Heart Disease surgery and its influencing factors

Objective: To investigate the current status of the main caregivers of children with Congenital Heart Disease (CHD) undergoing surgery and analyze the influencing factors. Methods: A total of 168 main of CHD children who underwent surgery in a hospital were selected by convenient sampling method. The general information of the children and their main caregivers was collected. The main caregiver's burden was assessed according to the Zarit Caregiver Burden Interview (ZCBI). Multiple linear regression analysis was used to analyze the influence factors of the burden of main caregivers. Results: The main caregivers of 168 children with CHD had a ZCBI score of 23-67 (43.15±9.32), which was a moderate burden, including 76 cases (45.23%) with mild burden, 82 cases (48.81%) with moderate burden. and 10 cases (5.96%) with severe burden. Multiple linear regression analysis showed that the age of the child, postoperative recovery, the main caregiver marital status, education level, daily care time, depression, and family monthly income could affect the burden of caregiver. Conclusion: The burdens of main caregivers of children with CHD undergoing surgery is affected by multiple factors. Nursing staff can help reduce their burden by giving caregivers physiological, psychological guidance and social support.

Feasibility study on shortening preoperative fasting time in patients undergoing arthroplasty

Objective: To evaluate the safety and feasibility of shortening preoperative fasting time in patients undergoing arthroplasty. Methods: A total of 107 patients who underwent arthroplasty from a hospital in Beijing were divided into control group (n=55) and experimental group (n=52) according to the time sequence. The control group was given guidance according to the traditional preoperative fasting method. The experimental group was fasted after 12:00 pm. on the 1st day before surgery, and 330 mL of Fructose drinks were orally administered 2 hours before the operation, and then abstained from drinking and eating. Objective measures (blood pressure, blood glucose, heart rate, exhaust time, urination time, etc) and subjective comfort (preoperative anxiety, thirst, hunger, nausea and vomiting) were recorded and compared between the two groups. And observe the occurrence of adverse reactions in patients. Results: There were no significant differences in postoperative blood pressure, heart rate, blood glucose, hunger, uresis duration, and nausea and vomiting between the two groups (P>0.05). The patients in the experimental group were better than the control group in terms of preoperative anxiety, postoperative thirst and exhaust time, and the difference was statistically significant (P<0.05). There were no serious adverse reactions occurred in either groups. Conclusion: It is safe and feasible to shorten the fasting time before arthroplasty, and it is helpful to reduce the symptoms of preoperative anxiety and thirst and shorten the postoperative exhaust time.

Effects of Ecological Systems Theory based family management intervention among parents of leukemia children

Objective: To explore the application effect of Ecological Systems Theory based family management intervention among parents of leukemia children. Methods: Totally 99 parents of leukemia children in a tertiary children's hospital were recruited using convenience sampling method. The experimental group received Ecological Systems Theory based family management intervention for 8 weeks, and the control group took routine care. Parents' disease management ability, family intimacy and adaptability, positive and negative emotions, and social support status were assessed before intervention, after 4 weeks intervenion, and after 8 weeks intervenion respectively. Results: In the experimental group, the disease management ability, positive emotion, objective support, subjective support and social support total score were better than those of the control group at the 4th and 8th intervention point (P<0.05). The support utilization and negative emotion scores also improved by time (P<0.01). There were no significant differences in family intimacy and adaptability scores between the two groups (P>0.05). Conclusion: Ecological Systems Theory based family management intervention could effectively improve the disease management ability of parents of leukemia children, positively regulate parental emotions, improve the social support of families with leukemia, and help families cope with diseases.

Development and reliability and validity evaluation of community nursing care needs questionnaire for patients with chronic diseases

Objective: To form the community nursing needs questionnaire for chronic disease patients, to measure its reliability and validity, and to provide scientific assessment tools for understanding and meeting the community nursing care needs of chronic disease patients. Methods: On the basis of literature analysis, theoretical research and expert consultation, we formed the questionnaire, including 5 dimensions which were health education, nursing technical support, disease management, health promotion and psycho-social support. The validity and reliability of the questionnaire were evaluated with 256 patients with chronic diseases. Results: The Cronbach's α of the total scale and each dimension were 0.93 and 0.70~0.89, respectively, which indicated good internal consistency. The CVI of the total scale and the dimensions were 0.98 and 0.95~1.00, respectively. Five factors were extracted by exploratory factors analysis and 63.13% of total variance were explained. Conclusion: The community care needs questionnaire for chronic diseases patients has a good reliability, and its dimension setting is suitable for evaluating community care needs of patients with chronic diseases.

Objective: To analyze the effects of service-learning on the practical teaching in geriatric nursing. Methods: Qualitative study and quantitative study were combined to compare the effects of service-learning method. Totally 103 nursing students were randomly divided into two groups. The traditional practice teaching was adopted by the control group, while the experimental group were with service-learning on the basis of traditional teaching. The attitudes toward the aged, the ability of critical thinking and the final test score of the two groups were compared. The nursing students and the staff of institutions, evaluation about the teaching program were collected by interview. Results : There were significant difference in the attitudes to the aged before and after teaching in the test group, and the attitudes to the aged and test score after teaching between the two groups (P<0.05). The total scores of critical thinking ability, the seeking truth ability, systematization ability and self-confidence in the experimental group were improved after the teaching (P<0.05). The students and staff of institutions evaluated the service-learning teaching method in a positive way. Conclusion: The service-learning is benefit to improve the students' attitudes toward the aged and the critical thinking ability, and it is beneficial for the development of nursing care for the aged and nursing education.

Establishment and practice of project management system for improving nursing quality

Objective: To introduce the project management method into nursing quality management to establish the best path for quality improvement and promoting the overall improvement of nursing quality. Methods: The management of nursing quality was improved with searching for quality issues as the starting point and the whole process was monitored continuously. Besides, questionnaires were used to learn about the difficulties in nursing quality improvement as well as the recommendation rate of a management item to the quality management project. Results: A system with three dimensions including long-term mechanism, process management and evaluation criteria was initially established in the improved nursing quality management project. The selection of topics in project improvement process were most difficult tasks. The net promoter score for project management increased year by year with statistically significant differences being observed between the two consecutive years within five years (P<0.05), and the differences of the help provided by the design team for quality improvement between the two consecutive years were also statistically different (P<0.05). Conclusion: Project management with improved quality of care is a new comprehensive application model for multiple management with promising prospects in the future.

Objective: To understand the progress of Chinese hospital hand hygiene from 1986 to 2016. Methods: 200 hospitals in 14 provinces and the People’s Liberation Army were stratified sampled and retrospectively investigated. The progress and trends of hand hygiene work in China was analyzed. Results: 99.50% (199) hospitals have established hand hygiene management systems, used alcohol-based hand rub, carried out hand hygiene training, and conducted hand hygiene supervision throughout the hospital. 97.00% (194) hospitals began to use paper towels, 83.50% (167) hand hygiene work was included in hospital performance evaluation, 84.50% (169) had hand hygiene knowledge awareness surveys, 90.50% (181) carried out hand hygiene compliance surveys. The most hospitals began hand hygiene work in 2009 and 2012. Conclusion: The measures for the promotion of hand hygiene in China have been implemented nationwide, and the policy orientation has obviously affected the advancement of various tasks.

Effect of Mindfulness-Based Stress Reduction training on anxiety, depression and immune function in HIV-infected/AIDS patients

Objective: To investigate the effects of Mindfulness-Based Stress Reduction (MBSR) training on anxiety, depression and immune function in HIV-infected/AIDS patients. Methods: Ninety-six HIV-infected/AIDS patients admitted to hospital for infectious medicine were randomly divided into control group (48 cases) and intervention group (48 cases). The control group received routine care, and the intervention group received 8 weeks of MBSR training on the basis of the control group. Results: There was no significant difference in the Self-rating Anxiety Scale (SAS), Self-rating Depression Scale (SDS), and CD4+T-lymphocyte counts between the two groups at baseline (P>0.05). 8 weeks after intervention and 6 months after intervention, the SAS and SDS scores of the intervention group were lower than those of the control group (P<0.05). The CD4+T lymphocyte count of the intervention group was higher than that of the control group (P<0.05). Conclusion: MBSR training can reduce anxiety and depression in HIV-infected/AIDS patients and improve their immune function.

This paper provides a comprehensive review of the literature on transport classification, transport process, team composition, transport vehicle and equipment, and adverse events for the development of ECMO inter-hospital transportation in China.

Objective: To analyze the effect of standardized procedures of pain care on patients after craniocerebral surgery. Methods: The pain status after craniocerebral surgery was assessed, and the standardized procedures of pain care was used, and the pain management effects was compared between standardized procedures of pain care group and the group without it. Results: The score of pain and the incidence of moderate to severe pain in the group with standardized process of pain care were significantly lower than that in the group without standardized process of pain care (P<0.01). The score of satisfaction with standardized process of pain care was significantiy higher than that in the group without it (P<0.05). Conclusion: The standardized procedures of pain care is suitable for patients after craniocerebral surgery and it is worthy of clinical application.

Analysis of factors infuencing Intra-Abdominal Pressure during Enteral Nutrition in patients with glioma

Objective: To analyze the influencing factors of Intra-Abdominal Pressure (IAP) in patients with glioma after operation, and to provide reference for effective Enteral Nutrition support in patients with glioma surgery. Methods: From October 2017 to April 2018, 164 patients with postoperative enteral nutrition for glioma treatment in a neurological intensive care unit in a tertiary grade A hospital in Beijing were investigated. The IAP was measured before and after enteral nutrition to analyze the factors affecting IAP. Results: There was a statistically difference in IAP between patients with glioma before and after enteral nutrition. The Body Mass Index (BMI), Positive End-Expiratory Pressure (PEEP), APACHE II and IAP were positively correlated (r=0.631, 0.455, 0.342, P<0.05), and the Glasgow Coma Scale (GCS) score and IAP were negatively correlated (r=-0.231, P<0.001). BMI, PEEP, APACHE II and GCS score were the influencing factors of IAP, which could explain 41.4% variation. Conclusion: In the process of Enteral Nutrition, patients with glioma after operation should be paid attention to the changes of IAP. This study provides a basis for clinical nursing monitoring indicators, in order to ensure the safety of patients' nutritional support.

Objective: To analyze the effect of the information construction of the gynecological malignant tumor health management center. Methods: Gynecological malignant tumor patients who were admitted to our hospital were assigned into experimental group (60 cases) and control group (60 cases). The control group adopted the current gynecological malignant tumor health management model, and the experimental group used gynecological malignant tumor health management center for health management. The incidence of postoperative complications, the incidence of Peripherally Inserted Central Catheter (PICC) catheterization complications, the degree of cancer-related fatigue, the incidence of malnutrition, patient compliance, and satisfaction with health management services were compared between the two groups. Results: The incidence of postoperative complications, PICC catheterization complication, cancer-related fatigue, and malnutrition in the experimental group were lower than in the control group (P<0.05). Patient compliance and satisfaction with health management services were higher than those of the control group (P<0.05). Conclusion: Health management of patients with gynecological malignant tumor health management center can reduce the incidence of postoperative complications, PICC catheterization complications, cancer-related fatigue, the incidence of malnutrition and improve the patient compliance, and satisfaction with health management services.
