Abstract: This article reviewed the definitions and models of transitional care, and explored the trends of transitional care practice in China and abroad. Three measures to improve the quality of transitional care research and practice were recommended, including: to provide scientific basis for national decision, to produce high-quality research evidences, and to identify the global direction of transitional care development and the priorities of research in transitional care.
