Abstract: This paper reviewed the continuity of care outcome evaluation indicators for stroke from process evaluation indicators and outcome evaluation indicators. Process evaluation indicators included information continuity, management continuity and relationship continuity. Outcome evaluation indicators included clinical outcome of patient, utilization rates of medical resources and patient satisfaction. In addition, this paper put forward weakness in current research and direction in future research, so as to provide a reference for future establishment of uniform and standardized outcome evaluation indicators for stroke.
