Abstract: Objective: To explore the effect of transitional care on self-care behaviors and health behaviors in patients with heart failure. Methods: Totally100 heart failure patients were assigned into control group (50 cases) and intervention group (50 cases). The control group were treated with routine care, and the intervention group received routine care plus transitional care. Both groups were evaluated at 3 months and 6 months after discharge with Self-care of Heart Failure Index (SCHFI) and Health Promoting Lifestyle Profile (HPLP) . Results: After 3 and 6 months after discharge, the scores of SCHFI, HPLP of the intervention group were significantly better than those of the control group (P<0.05). Conclusion:Transitional care can effectively improve self-care behaviors and health behaviors in patients with heart failure.
