Abstract: Objective: To evaluate the effect of hospital-community-family-care model on pulmonary rehabilitation in patients with chronic obstructive pulmonary disease (COPD). Methods: Totally 40 patients were guided with acknowledge of pulmonary rehabilitation according to the hospital-community-family-care model, and were investigated with airways questionnaire 20 (AQ20-R) and pulmonary function index before and after intervention. Results: After applying the hospitalcommunity-family-care model, the pulmonary function and the quality of life were significantly improved than before (P<0.01).Conclusion: The pulmonary rehabilitation program based on hospital-community-family-care model can improve the quality of life and lung function index in patient with COPD.
