Abstract: Patients during the ICU transition period are both physiologically and psychologically specific. ICU transitional care may have both short-term and long-term effects on their overall recovery overcome. This review summarized the concept of Transitional Care Mode, nursing needs of ICU patients, current strategies of ICU transitional care, evaluating indicators both in China and abroad to provide evidence and reference for nursing research and practicing of ICU transitional care.
