Abstract: Objective: To establish and apply transitional care information platform in Sichuan, and to promote the transitional care work effectively in Sichuan. Methods: The hospital developed transitional care software, built information platform. The hospital, the transitional care center and the community were linked together to share the information from information network and to ensure that high quality medical services could be extended to the family. Results: The transitional care information platform could promote the effective implementation of transitional care, which brought convenience for the patient, reduced the patients' and their families' mental and economic burden, and improved patients' quality of life and their nursing service satisfaction. Conclusion: The building and effective implementation of transitional care information platform is an effective method for promoting the transitional care.
