Abstract: Treatment-care combination is one of the endowment patterns of urgent need for elderly patients, and the big hospitals and nursing homes have their respective advantages and resources. The problem being how to be combined and implemented the dual referral pathway of treatment-care combination between hospitals and nursing homes would be the need to explore and sovle. This paper is to explore the dual referral pathway of treatment-care combination for elderly patients based on the medical alliance, and put forward the thinking and outlook.
