Abstract: Objective: The aim of transitional care is to ensure a safe transition of patients from hospital to community or home,and to solve problems of the discharged patient for improving the quality of life. This paper reviewed the objects, the implementation methods, the responsible personnel, the problem and the countermeasures of transitional care to the discharged patients.
