Abstract: Objective: To establish and application post-discharged continuity of care information platform for cancer patients. Methods: By coupling the special software of multiple modules such as the hospital HIS, outpatient clinic system, and extracting the basic information and oncology-related diagnosis and treatment information to establish a hospital-based postdischarged continuity of care information platform for cancer patients. Results: The information platform with practicality was easy to operate, and provided higher quality information material of the cancer cases to ensure information integrity and correctness. Conclusion: The development and application of information technology provide a guarantee for the carrying out of post-discharged continuity of care information platform for cancer patients, as well as providing a basis for cancer research.
