Abstract: Chronic post-surgical pain significantly impacts patients' quality of life and imposes substantial socioeconomic burdens. The Transitional Pain Service (TPS) achieves the foresight and sustainability of chronic postoperative pain management through preoperative assessment and prehabilitation, perioperative multimodal analgesia, and continuous post-discharge interventions. This article reviews the components, implementation strategies, and outcomes of the Transitional Pain Service, exploring its contributions to improving the efficiency of chronic postoperative pain management. Based on the current state of pain management in China, the paper offers insights and recommendations to inform and optimize domestic chronic postoperative pain management practices.
