Abstract: Objective: To construct an advanced practice nursing model for liver transplantation nursing care and summarize the practical experience and challenges. Methods: Based on the analysis of nursing practice needs and nursing models in liver transplantation in a tertiary grade A hospital in Hangzhou, Advanced Practice Nurses (APN) position for liver transplantation nursing care were set up, the APN work model, responsibilities and staff division were clarified, and the implementation effects were observed and evaluated. Results: Six nurses with the right qualifications and abilities were selected to form the liver transplant APN team. They participated in multi-disciplinary treatment discussions in 596 cases, directly participated in the care of 249 liver transplant patients in difficult and critically disease, and 162 nursing consultations. They participated in the follow-up of liver transplant patients, with a 0 failure rate and a 100.00% completeness rate of follow-up files, and presided over 9 quality improvement projects. the nursingquality of liver transplantation has improved greatly. Conclusion: This advanced nursing practice model for liver transplantation nursing care is benefit to further develop high quality clinical care, improve the nursing standards, and promote the sustainable development of the discipline.
