Abstract: The problem of uneven distribution of medical resources is becoming more and more serious. It is urgent to push forward the reform of medical treatment and the construction of medical partnerships. In order to match the ability of primary nursing staff in our country with the transitional care mode of "hospital-community-family" linkage, the purpose of this article is to introduce the practice of community diabetes management nurses' training based on the mode of tutorial system, and to provide some reference for improving the management ability of chronic diseases of community nurses.
