Abstract: This paper summarized the current status of discharge planning at home and abroad, including concept, contents, executors as well as the application in heart failure patients. We proposed suggestions concerning the domestic application of discharge planning in heart failure patients to promote the development of discharge planning in heart failure patients in China.
