Abstract: Objective: To develop the outcome evaluation indicator system for hospital transitional care of chronic illness. Methods: Literature review and Delphi technique were used to develop the outcome assessment indicator system for transitional care of chronic illness. The Analytic Hierarchy Process (AHP) was used to determine the weights of all indexes. Results: 4 first-level, 10 second-level and 38 third-level indicators were included in the final outcome assessment indicator system for transitional care of chronic illness. Authority coefficients of two-round expert consultations were 0.84 and 0.88, coordination coefficients of indicators of all levels ranged from 0.312 to 0.434 in the two-round expert consultations. Conclusion: Centralized experts' opinions, high level of experts' enthusiasm and authority were found for the outcome assessment indicator system, which can provide powerful basis for evaluating the outcome of hospital transitional care of chronic illness.
