Abstract: Objective: To explore the effects of extended nursing intervention inelderly patients with Chronic Heart Failure. Methods: Totally 100 patients with Chronic Heart Failure were randomly divided into experimental group (n=50) and control group (n=50). Patients in control group accepted usual discharge guidance, while patients inexperimental group accepted individualeducation for 3 months. The 6 MWT were performed at discharge and three months later. Rehospitalization of the patients within three months were recorded. Results: At the end of three months, there was significant difference in 6MWT between the control group (198.64±18.87) m and the experiment group (476.42±23.25) m (P<0.05). There was significant difference in rehospitalization rate between the two groups as well (P<0.01). The reospitalization rate of the control group was 40.0% (20 cases), while the experiment group was 12.0% (6 cases). Conclusion: Transitional care can improve the quality of life in elderly patients with Chronic Heart Failure.
