Abstract: The transitional period of adolescents to adults is an important stage for patients with Congenital Heart Disease (CHD) to achieve self-management of the disease. Providing comprehensive and perfect transitional care for adolescents with CHD can ensure the continuity of care, improve their self-management ability and their cardiac function. This article reviews the research on the current development, nursing needs and nursing programs of transitional nursing for adolescents with CHD, and puts forward relevant suggestions, in order to provide reference for the practice of transitional nursing for this population in China.
