慢性阻塞性肺疾病患者吸入用药管理的证据总结

Evidence summary for inhaled medication management in patients with COPD

摘要: 目的:总结慢性阻塞性肺疾病(COPD)患者使用吸入装置吸入治疗的相关证据,为临床实践提供参考。方法:依据证据资源“6S”金字塔模型,自上而下依次检索数据库、相关网站中有关COPD患者吸入用药管理证据的相关文献,检索时限为建库至2023年6月13日,由经循证护理系统培训的研究人员对纳入文献独立地进行质量评价和证据提取,并结合专家意见,汇总证据。结果:纳入21篇文献,包括临床决策5篇、指南4篇、证据总结2篇、系统评价5篇、随机对照试验1篇、专家共识4篇。从多学科合作、联合用药方案、吸入装置的选择、吸入装置使用技术的评估、吸入用药依从性的评估、吸入用药教育、随访与管理、效果评价8个方面汇总了27条证据。结论:COPD患者吸入用药管理的证据可以为医护人员管理患者吸入用药提供循证依据, 提高患者吸入用药的正确性。

Abstract: Objective: To summarize the evidence of inhaled therapy with inhalation devices in patients with COPD, and to provide references for clinical practice. Methods: The literature related to the evidence of inhaled medication management in COPD patients was systematically searched from domestic and foreign databases and websites until June 13, 2023. Researchers evaluated literature quality and extracted evidence according to the literature evaluation criteria. Results: A total of 21 literatures were included, including 5 clinical decisions, 4 guidelines, 2 evidence summaries, 5 systematic reviews, 1 randomized controlled trial and 4 expert consensus. Twenty-seven pieces of evidence were summarized from eight aspects: multidisciplinary cooperation, combination medication regimen, selection of inhalation device, evaluation of inhalation device usage technique, evaluation of inhaled medication compliance, health education for inhaled medication, follow-up and management, and effects evaluation. Conclusion: This study summarized the current evidence on inhaled medication management in COPD patients and provided scientific and effective evidence-based reference for healthcare providers to manage inhaled medication, so as to improve the correctness of inhaled medication in COPD patients.
