曲线型仰卧手术体位用于预防术中压疮的研究

The study of curvilinear supine position used for preventing intra-operative pressure ulcer

摘要: 目的:分析曲线型仰卧手术体位预防手术患者术中压疮的临床效果。方法:将患者随机分为实验组50例和对照组51例,分别给予曲线型仰卧手术体位和传统仰卧位。结果:①体位安置后,实验组患者枕部、骶尾部、足跟压强均低于对照组(P<0.05),而肩胛部压力降低不明显。但实验组患者小腿压力略高于对照组,且差异具有统计学意义(P<0.05);②实验组患者体位舒适率高于对照组(P<0.01);③随着手术时间的延长,实验组患者骶尾部、足跟部压力持续低于对照组(P<0.01);④手术结束即刻,实验组患者未发生压疮,对照组发生9例I期压疮,实验组压疮发生率明显低于对照组(P<0.01),术后24 h两组均无压疮发生。结论:曲线型仰卧手术体位能够使仰卧位的手术患者身体压力再分布,有效降低主要受压部位的压力,并对高压部位进行持续减压,从而降低术中压疮发生率,提高患者身心舒适度,提高手术室护理质量。

Abstract: Objective: To analyze the clinical effect of curvilinear supine position on preventing intra-operative pressure ulcers in surgical patients. Methods: The patients were randomly divided into experimental group (50 cases) and control group (51 cases). The patients in experimental group were given the curvilinear supine position and the patients in control group accepted traditional supine position. The incidence of pressure ulcer, the peek pressure of the high-pressure areas, patients′ comfort and doctors′ satisfaction were compared between the two groups. Results:①After positioning, the peek pressures of occipital, caudal, and heel of experimental group were much lower than those of the control group (P<0.05). The pressure decrease of the shoulder blade was not obvious. But, the pressure of the lower leg in the experimental group was higher than control group (P<0.05).②The percentage of patients feeling comfortable with the position in the experimental group was higher than that in the control group (P<0.01). ③The pressure decreases on the sacral and heel persistently of the experimental group with prolonged operation time (P<0.01).④The incidence of intra-operative pressure ulcer immediately after surgery in experimental group was zero, much lower that of the control group (9 cases of stage I pressure ulcer) (P<0.01), and there were no pressure ulcers occurred in both two groups 24 hours after operation. Conclusion: Curvilinear supine position has a positive effect on lowering the interface pressure through pressure redistribution and continuous decompression to the high-pressure areas of the surgical patients, so as to effectively reduce the incidence of intra-operative pressure ulcers, improve patients′ comfort and the quality of nursing in operation room.
