Abstract: The lung transplantation center in our hospital established a nurse-based case management model with a multi-discipline cooperation and comprehensive management. In the different stages of treatment in lung transplantation, case management nurses delivered "multi to one" service model for patients with lung transplantation and their families according to their unique responsibility, and provided professional, individualized full-range service during the whole treatment. The model achieved good effects.
