Abstract: Chronic disease has become a huge burden for Chinese health care system. As a community based nursing care program, the Omaha System has being used in the field of chronic disease management more and more widely. In this paper, the authors reviewed the concept, characteristics, implementation and effects of Omaha System in the management of chronic diseases.
