Abstract: Objective: To explore the effect of hospital-community-family nursing intervention model on the rehabilitation of stroke patients. Methods: Totally 56 stroke patients were recruited by convenience sampling method and delivered 1 years of hospitalcommunity-family nursing intervention model. We evaluaged the patiens daily life ability and quality of life before the intervention and 12 months after the intervention. Results: The scores of Barthel and stroke quality of life in 12 months after the intervention were significantly improved than that before intervention and the difference was statistically significant (P<0.05). Conclusion: The hospital-community-family care intervention model could improve the daily life ability and the quality of life of stroke patients.
