北京市三级甲等医院儿科护士PICC拔管现状及影响因素分析

Status quo of pediatric nurses' behavior in managing PICC extubation and its influencing factors in tertiary grade A hospitals in Beijing

摘要: 目的:调查儿科护士经外周置入中心静脉导管(PICC)拔管的现状并分析其影响因素,为保障患儿静脉治疗安全提供借鉴。方法:采用方便抽样的方法,2024年5月对北京市16家三级甲等医院230例儿科护士进行问卷调查。调查工具为自行设计的一般资料问卷、儿科护士PICC拔管知信行问卷。结果:儿科护士对PICC拔除的知识、态度和行为各维度得分分别为(14.83±2.76)分、(126.33± 16.97)分、(129.14±10.76)分,得分率分别为67.4%、93.6%、86.1%;多元线性回归分析结果显示,医院性质、近3年内是否接受过PICC拔管操作培训、PICC拔管态度是儿科护士PICC拔管行为的影响因素(P<0.05)。结论:儿科护士对PICC拔管知识掌握情况欠佳,但态度积极,行为表现尚可。护理管理者应高度重视PICC拔管护理知识和技能的培训,积极构建专业培训平台,为儿科护士提供全面且深入的PICC拔管管理培训,从而进一步提升静脉治疗护理质量,保障患儿生命安全。

Abstract: Objective: To examine pediatric nurses' behavior in managing Peripherally Inserted Central Catheters (PICC) extubation and identify influencing factors. Methods: A survey was conducted among 230 pediatric nurses from 16 tertiary grade A hospitals in Beijing using convenience sampling. The survey included questions on nurses' knowledge, attitude, and behaviors related to removing PICC. Results: Pediatric nurses achieved mean scores of 14.83±2.76, 126.33±16.97, and 129.14±10.76 for knowledge, attitude, and behavior related to PICC removal, respectively, with the corresponding score rates being 67.4%, 93.6%, and 86.1%, separately. The results of multiple linear regression analysis showed that the nature of hospital, whether receiving PICC extubation training within 3 years, and the attitude dimension scores were the influencing factors of PICC extubation behavior of those pediatric nurses. Conclusion: Although the pediatric nurses lack knowledge and skills in PICC extubation, they have relatively good behaviors with active attitudes. Therefore, nursing administrators should prioritize training and create a professional platform for comprehensive training for nurses to enhance safe care for children.
