肱骨肿瘤反肩关节置换术后患者早期功能锻炼方案的构建

Construction of early functional exercise program for humerus tumor survivors with Reverse Total Shoulder Arthroplasty

摘要: 目的:构建肱骨肿瘤反肩关节置换术后患者早期功能锻炼方案,为患者术后早期进行功能锻炼提供参考。方法:研究小组以循证为基础,初步制定反肩关节置换术后患者早期功能锻炼方案,于2020年3月10日—30日通过两轮德尔菲专家函询,并按照专家建议加以调整。结果:第1轮对17名专家进行了函询,共收回有效问卷15份,有效回收率为88.2%,各条目变异系数为0.061~0.286,肯德尔和谐系数为0.104(P<0.05);第2轮对15名专家进行了函询,有效回收率为100.0%,各条目变异系数为0~0.128,肯德尔和谐系数为0.112(P<0.05),重要性赋值均数为4.67~5.00。最终形成的方案包括3个一级条目,9个二级条目,22个三级条目。结论:本研究构建的反肩关节置换术后患者早期功能锻炼方案科学可靠,可操作性强,具有临床应用价值。

Abstract: Objective: To develop an early postoperative functional exercise program for humeral tumor survivors with Reverse Total Shoulder Arthroplasty (RTSA), and to provide a reference for early postoperative functional exercise. Methods: Based on the evidence-based practice principles, the research team initially formulated an early functional exercise program for RTSA. Two rounds of Delphi expert consultation were conducted from March 10, 2020 to March 30, 2020, and adjustments were made according to expert suggestions. Results: In the first round, 17 experts were consulted, and 15 valid questionnaires were collected, with an effective recovery rate of 88.2%. The variation coefficient of each item was between 0.061 and 0.286, and the Kendall harmony coefficient was 0.104 (P<0.05). Fifteen experts were consulted in round two with an effective recovery rate of 100.0%. The variation coefficient of each item was between 0 and 0.128, and the Kendall harmony coefficient was 0.112 (P<0.05). The mean value of importance assignment was 4.67 to 5.00. Finally, 3 first-level items, 9 second-level items, and 22 third-level items were formed. Conclusion: The early postoperative functional exercise program developed in this study is scientifically reliable, highly operable, and has clinical application values.
