加强康复对脑外伤患者康复效果的影响

Effects of intensive rehabilitation program on patients with traumatic brain injury

摘要: 目的:评价加强康复对脑外伤患者康复效果的影响。方法:选取2016年1-10月的53例脑外伤患者作为对照组,给予常规康复护理,由家属或陪护督促协助患者自主练习;选取2017年1-10月的51例脑外伤患者作为观察组,在对照组的基础上成立加强康复治疗小组,由康复护士指导患者利用晚间空余时间开展加强康复治疗,对比两组患者康复治疗前后的简易智能状态检查量表(Mini-Mental State Examination,MMSE)、Fugl-Meyer运动功能评定量表和Barthel指数量表的得分。结果:两组患者康复治疗前后MMSE评分,Fugl-Meyer评分和Barthel指数评分均较治疗前有显著改善,差异有统计学意义(P<0.05),且观察组各项评分的改善幅度均明显优于对照组,差异有统计学意义(P<0.05)。结论:加强康复可以从护理角度进一步提升脑外伤患者的康复疗效,可为脑外伤康复的进一步研究提供参考。

Abstract: Objective: To investigate the effects of intensive rehabilitation program on patients with traumatic brain injury (TBI). Methods: Fifty-three TBI patients from January to October in 2016 were included in control group. During the daytime, they received a comprehensive rehabilitation program, including physical therapy, occupational therapy, speech therapy and psychotherapy; in the night, they received usual rehabilitation nursing, supervised and assisted by families or caregivers for self-exercise. Meanwhile, 51 TBI patients were included in the intervention group, and they received intensive rehabilitation program in combination with the programs received in control group. After the interventions, standard assessments on cognition, motor function and activity of daily living were conducted for all patients using Mini-Mental State Examination (MMSE), Fugl-Meyer Assessment Scale and Barthel Index scale, respectively. Results: After the interventions, the scores of MMSE, Fugl-Meyer and Barthel Index were significantly improved (P<0.05) in both groups. Moreover, patients in the intervention group had higher scores based on the three assessments as compared with the control group (P<0.05). Conclusion: Intensive rehabilitation program could further improve the effectiveness of rehabilitation among TBI patients from the perspective of nursing. It could be considered as a new working model for nurses, and is worthy of being generalized for TBI's rehabilitation in major hospitals.
