护士对化疗所致周围神经病变评估及管理知信行的调查

Knowledge, attitude and practice regarding Chemotherapy-Induced Peripheral Neuropathy assessment and management among oncology nurses

摘要: 目的:调查肿瘤相关科室护士对化疗所致周围神经病变(Chemotherapy-induced Peripheral Neuropathy,CIPN)评估及管理的知信行现状及影响因素,明确护士评估CIPN的障碍因素及提高相关知识的途径。方法:采用自行设计的调查问卷,对309名护士进行调查。经单因素和多因素分析明确护士对CIPN评估知、信、行的影响因素。结果:护士对CIPN评估及管理知识(9.16±1.93)分,态度(27.96±6.65)分,行为(25.84±12.48)分,分别占满分的57.3%、77.7%和53.8%;护士对CIPN评估及管理实践的主要影响因素为是否经过相关培训和职称。评估CIPN的障碍主要体现在:不知道怎样进行神经系统的体格检查(68.9%)、不知道如何获取合适的评估问卷(57.9%)、不知道如何评估(55.7%)等。护士较倾向的可以提高CIPN相关知识的途径为科室组织进行理论学习(76.4%),其次为专业学习班(71.2%)。结论:护士对CIPN评估及管理知信行较差,管理者需要提高护士对CIPN的重视,积极开展以CIPN基本知识、评估、护理预防、指南解读为主要内容的肿瘤专科护士培训工作,探索适合我国患者且便于临床应用的CIPN评估工具。

Abstract: Objective: To examine the knowledge, attitude and practice regarding Chemotherapy-induced Peripheral Neuropathy (CIPN) assessment and management among oncology nurses, and identify the barriers of CIPN assessment and management as well as nurses' needs of continuing education. Methods: The self-designed questionnaire was used to collect data. Univariate and multivariate analysis were used to identify the influencing factors of oncology nurses' knowledge, attitude and practice regarding CIPN assessment and management. Results: The average scores of knowledge, attitude and practice regarding CIPN assessment and management were 9.16±1.93, 27.96±6.65 and 25.84±12.48 respectively, accounting for 57.3%, 77.7% and 53.8% of the full score. Nurses' practice were influenced by their training experience and professional title. Barriers of performing CIPN assessment were do not know how to perform neurologic assessment (68.9%), no proper assessments (57.9%), and do not know how to evaluate (55.7%). Nurses preferred to take part in group study (76.4%) and academic workshop (71.2%) to improve knowledge. Conclusion: Nurses' knowledge, attitude and practice regarding CIPN assessment and management needed to be improved. Managers should organize specific training programs to improve nurses' ability, and also develop proper assessment tool for CIPN.
