摘要/Abstract

摘要： 目的：评价医护一体化管理模式在ICU患者安全转出交接过程中的应用效果。方法：2015年7月至2016年12月采用医护一体化管理模式，通过制订ICU患者转出医护交接记录单、结合SBAR方式规范病情评估内容、执行医护一体化转出交接流程等措施，规范ICU患者的安全转出交接过程。结果：应用后，患者转出ICU后48 h内重返率由1.79%降为0.87%（P<0.05）；转出交接问题发生率由14.05%降为3.24%（P<0.01）；平均交接所耗时间由（18.70±2.62）min减少至（10.52±2.14）min（P<0.01），护患间纠纷发生例数由2例降低至0例。结论：医护一体化管理模式实现了转交接科室间的无缝隙对接，有效保障了ICU患者在转出交接过程中医疗护理措施的连续性，保障了患者的安全。

Abstract: Objective: To determine whether medical integration mode improves the quality and safety of handoffs for patient transitioning from the Intensive Care Unit (ICU). Methods: The medical integration mode was applied from July 2015 to December 2016, with a checklist and standardized hand-off procedure. Results: The rate of return to ICU within 48 hours decreased from 1.79% to 0.87% (P<0.05). The incidence of adverse events during transfer decreased from 14.05% to 3.24% (P<0.01). The average time of hand-off was shortened from (18.70±2.62) min to (10.52±2.14) min (P<0.01). The disputes between patients and nurses reduced to none. Conclusion: Medical integration is an accurate, safe, and efficient way to perform handoffs and provide high-quality patient care.
