社区脑卒中后失能患者支持性干预的效果分析

Effects of supportive interventions on post-stroke patients with disability in community

摘要: 目的:调查支持性干预对社区卒中后失能患者的干预效果。方法:将210例社区卒中后失能患者按生活自理程度匹配分为干预组和对照组各105例,对照组给予健康教育,干预组在此基础上实施以躯体康复为核心,辅以心理干预及家庭社会干预的支持性干预。于干预前、干预后及干预后6个月,采用改良Barthel指数量表(MBI)、抑郁自评量表(SDS)、一般自我效能感量表(GSES)、社会功能活动问卷(FAQ)评价两组患者的恢复效果。结果:两组患者SDS、GSES、FAQ得分的干预主效应具有统计学意义(P<0.05),且MBI、SDS、GSES、FAQ得分的时间主效应和交互效应均存在统计学差异(P<0.01)。结论:支持性干预可有效提高社区脑卒中后失能患者的日常生活活动能力(ADL)、自我效能及社会功能,缓解其抑郁情绪。

Abstract: Objective: To explore effects of supportive interventions on post-stroke patients in community with disability. Methods: Totally 210 post-stroke patients in community with disability were divided into the intervention group and the control group with 105 cases in each group according to their level of self-dependence. Patients in the control group were given health education, while patients in the intervention group were given supportive interventions which centered on physical rehabilitation and supplemented by psychological interventions and family social interventions. Modified Barthel Index (MBI), Self-rating Depression Scale (SDS), General Self-efficacy Scale (GSES) and Functional Activities Questionnaire (FAQ) were adopted before and after interventions as well as 6 months after interventions to assess recovery conditions of patients in both groups. Results: A statistical significance (P<0.05) could be seen when comparing the main effect of intervention factor through SDS, GSES and FAQ between the two groups. As to the main effect of time factor and the interaction factor of MBI, SDS, GSES and FAQ, a statistical significance (P<0.01) also existed. Conclusion: Supportive interventions can effectively improve activities of daily living (ADL), self-efficacy and social functions of post-stroke patients in community with disability, and relieve their depression.
