乳腺癌患者告知父母癌症诊断前后的心理体验

Psychological experience in breast cancer patients before and after informing their parents of cancer diagnosis

摘要: 目的:调查乳腺癌患者告知父母癌症诊断前后的心理体验。方法:采用质性研究方法,以半结构式的开放性访谈问卷对19例乳腺癌患者进行个体化深入访谈,运用内容分析法分析资料。结果:多数访谈对象经历了由不告知到告知的过程。告知前的心理体验包括担心增加父母心理负担、担心影响父母身体健康、认为父母帮不上忙、心里委屈、心累;告知后出现正性和负性的心理体验,正性心理体验包括父母比想象中坚强、亲密感增加、心理负担降低;负性心理体验包括愧对父母、束缚感。结论:乳腺癌患者告知父母癌症诊断前的负性心理体验较多,告知后的心理体验较复杂,但以正性心理体验为主。护理人员应了解患者不同阶段的心理感受,帮助其建立良好的家庭支持系统。

Abstract: Objective: To explore the experience in breast cancer patients before and after informing their parents of cancer diagnosis. Methods: The qualitative research was adopted. In-depth interviews were conducted with 19 breast cancer survivors. Data were analyzed by content analysis method. Results: Most interviewees had gone through the process of not informing their parents to inform them. The psychological experience before breast cancer patients informing their parents of cancer diagnosis includes worried about increasing the psychological burden of parents, worried about affecting parental health, thinking parents can't help them, feeling aggrieved and heart tired. There are positive and negative psychological experience after breast cancer patients informing parents. Positive psychological experience including parents are stronger than we expected, intimacy increased and psychological burden reduced. Negative psychological experience includes feeling guilty about parents and a sense of restraint. Conclusion: The psychological experience before breast cancer patients informing their parents of cancer diagnosisis more negative. The psychological experience after breast cancer patients informing their parents is complex, mainly positive psychological experience. Nurses should understand the psychological experience in breast cancer patients and help them to establish a good family support system.
