Abstract: Along with the rapidly aging population, the prevalence of chronic diseases also increased greatly in the elderly. Chronic diseases are complex and lifelong conditions, and transitional care plays an important role in the chronic disease management. In this review, the authors summarized the concept of transitional care, care model, problems and suggestions for implementing transitional care in patients with chronic diseases.
