人工鼻对预防人工气道患者肺部感染的Meta分析

The HME to prevent pulmonary infection in patients with artificial airway: a Meta-analysis

摘要: 目的:系统评价人工鼻(HME)使用对人工气道患者肺部感染的预防效果。方法:计算机检索Cochrane 图书馆、JBI 循证护理中心图书馆、MEDLINE、Embase、Pubmed、中国知网数据库(CNKI)、维普数据库(VIP)、万方数据库,收集所有关于人工鼻预防人工气道患者肺部感染的随机对照试验和临床对照试验。2 名研究者按照纳入和排除标准独立检索文献、筛选文献、评价纳入文献的质量,并提取资料,使用RevMan 5.3 软件进行Meta 分析。结果:共纳入14 篇文献,均为RCT。Meta 分析结果显示:与未使用人工鼻相比,人工鼻可以预防人工气道患者肺部感染发生[ 随机效应模型OR =0.76,95% CI(0.47,1.20) ],差异具有统计学意义;对患者病死率的影响差异无统计学意义[ 随机效应模型OR =0.97,95% CI(0.65,1.46) ]。结论:人工鼻可以降低人工气道患者肺部感染的发生率,建议临床推广使用。

Abstract: Objective: To systematically review the effect of HME on preventing pulmonary infection in patients with artificial airway. Methods: The Cochrane Library, JBI Library, Medline, Embase, Pubmed, CNKI, VIP and WanFang databases were searched to retrieve randomized controlled trials and controlled clinical trails about the effect of HME on preventing pulmonary infection in patients with artificial airway. According to the inclusion and exclusion criteria,the literature searching, screening, information extraction and quality evaluation were completed by two reviewers independently. Meta analysis was conducted using RevMan 5.3 software. Results: Totally, 14 RCTs were included. The meta-analysis showed that compared with non HME, using HME can prevent pulmonary infection in patients with artificial airway [Random effect model, OR=0.76, 95% CI (0.47, 1.20) ], and the significant differences existed. The differences were no significant in the mortality rate [OR=0.97, 95% CI (0.65, 1.46)] . Conclusion: HME can reduce the incidence of pulmonary infection in patients with artificial airway and it is worth being popularized.
