认知功能训练护理门诊的构建与实施

Construction and practice of nurse-led cognitive function training clinic

摘要: 目的:分析综合医院建立认知功能训练护理门诊的实施及其对维持认知障碍患者认知功能,延缓疾病发展的效果。方法:选取北京某三级甲等医院2017年3-9月在认知功能训练护理门诊,进行认知功能训练的患者分为干预组22例,对照组23例。对照组采用规范化药物治疗,发放自行编制的认知障碍患者康复训练手册,干预组在此基础上由认知功能训练护理门诊的护士为患者完成认知功能训练。结果:训练6个月后,对照组患者的蒙特利尔认知评估量表和简易精神状态检查量表分值低于干预组,差异具有统计学意义(P<0.05)。结论:认知功能训练护理门诊的实施对维持患者的认知功能,促进认知障碍患者管理的系统化、规范化,实现对患者门诊-住院-居家的全程管理模式,提高护士职业认同感,推动专科护理发展具有重要意义。

Abstract: Objective: To analyze the practice of cognitive function training in general hospital and its effect on maintaining cognitive function of patients with cognitive impairment and delaying the disease progression. Methods: A total of 45 patients under cognitive function training in a tertiary grade A hospital of Beijing from March 2017 to September 2017 were selected and divided into the control group (n=23) and the intervention group (n=22). In the control group, patients received standardized drug treatment and the rehabilitation training manual for patients with cognitive impairment. Besides, patients in the intervention group were trained by the nurses in the cognitive function training. Results: After 6 months training, the scores of MMSE and MoCA in the control group lower than those in the intervention group, and the difference between the two groups was statistically significant (P<0.05). Conclusion: The implementation of nurse-led cognitive function training clinic is significant to maintain the cognitive function of the patients, promote the systematization and standardization of patients’ cognitive impairment management. It could realize the whole outpatient-inpatient-home process management model to improve the professional identity of the nurses and special nursing development.
