Abstract: Objective: To evaluate the effect of the "sugar Guard" APP applied in the transitional care of diabetes. Methods: The APP was applied to clinic after docking with diabetes management platform developed by the First Affiliated Hospital of Guangxi Medical University. A total of 60 patients who met the inclusion criteria were recruited and divided into experimental group and control group with 30 patients in each group. On the basis of the existing health education model, the control group used telephone follow-up, while the experimental group used the "sugar Guard". Results: After 3 and 6 months' intervention, The improvement of blood glucose,blood lipid and other metabolic parameters, monitoring compliance, timely referral rates and patient satisfaction in the experimental group patients were better than those in the control group (P<0.05). Conclusion: Based on APP diabetes transitional care model, we could manage the patients scientifically and improve the patients' metabolic indicators, monitoring compliance, timely referral rate and satisfaction.
