Abstract: The "expert-community nurse-COPD family" disease management network was established by a specialized team. It includes four modules: organization structure and association approach, responsibilities of personnel in all levels, support of community nurses, and support of COPD families. The application of this management network effectively improved the knowledge and behavior of COPD families, the quality life of patients, the service condition of health resource.
