Latent profile analysis and influencing factors of Subjective Well-Being among outpatient nurses in general hospitals

Objective: To explore the latent category characteristics of Subjective Well-Being (SWB) among outpatient nurses in general hospitals, analyze the influencing factors of different categories, and provide reference for the formulation of targeted intervention measures. Methods: Using a convenient sampling method, 2163 outpatient nurses from 38 tertiary grade A general hospitals in 18 provinces (municipalities and autonomous regions) across China were selected as survey participants in November 2024. Surveys were conducted using a general information questionnaire and the General Well-Being Schedule. Latent profile analysis was employed to establish the latent category model of SWB among outpatient nurses. Univariate analysis and multivariate logistic regression analysis were conducted to analyze the influencing factors of different SWB categories. Results: The SWB of outpatient nurses could be divided into three potential categories: low SWB-high stress (23.86%), moderate SWB-interest deficiency (44.29%), and high SWB-relaxation (31.85%). Professional title and position were the influencing factors of the SWB of different types of outpatient nurses (P<0.05). Conclusion: The SWB of outpatient nurses in tertiary general hospitals exhibits significant heterogeneity. Individualized and group-based intervention programs should be developed for outpatient nurses of different SWB classes to enhance their SWB.

Objective: To evaluate the effects of exercise rehabilitation in patients after Percutaneous Coronary Intervention (PCI) in specialist nursing clinics, so as to provide reference for the construction of specialist nursing clinic. Methods: In June 2023, our hospital opened a cardiac rehabilitation specialist nursing clinic, providing services such as exercise risk assessment, exercise prescription establishment, simulated exercise practice guidance, outpatient MDT, family participation in multiple health education, follow-up and home exercise supervision. The changes in rehabilitation exercise knowledge, belief and behavior, exercise ability and satisfaction before and after the opening of the outpatient clinic were compared. Results: After the opening of specialist nursing clinic, the scores of exercise knowledge, belief and action, Peak VO2, Peak METs and satisfaction were higher than those before the opening (P<0.05). Conclusion: The specialist nursing clinic can improve the rehabilitation exercise knowledge and practice level of patients after PCI, improve the exercise ability of patients, and enhance the medical experience of patients.

Application effects of KANO model in outpatient nursing service

Objective: To analyze the application effects of KANO model in outpatient nursing services, so as to provide reference for improving the quality of outpatient nursing service. Methods: From October 2022 to April 2023, totally 1200 patients who came to our hospital for outpatient treatment were selected by convenience sampling method. The first 600 patients were the control group, who received conventional outpatient nursing services. The last 600 patients were the experimental group, who received optimized outpatient nursing services guided by the KANO model. Then the application effects of the two groups were compared. Results: The average waiting time of the experimental group was significantly shorter than that of the control group (P<0.05). The correct rate of preliminary examination and triage, the rate of disease knowledge and the satisfaction rate of nursing services in the experimental group were higher than those of the control group patients (P<0.05). Conclusion: Providing outpatient nursing services guided by the KANO model can significantly shorten patients' waiting time, improve the correct rate of preliminary examination and triage, enhance patient satisfaction, and greatly improve the level of nursing services.

Objective: To evaluate the effect of digital intelligent management model in day surgery in reproductive outpatient department, and to provide reference for improving the management quality of minimally invasive surgery. Methods: A digital intelligent management model of outpatient day surgery was constructed from the aspects of team management, information system optimization, and pre-hospital service process optimization. The average waiting time for admission, cancellation rate of same-day operation, and patient satisfaction were compared before and after the implementation of the model. Results: After the implementation of the digital intelligent management model, the average waiting time for admission and the cancellation rate of same-day operation were significantly reduced (P<0.05), the satisfaction of patients was significantly improved (P<0.05). Conclusion: The digital intelligent management model can improve the satisfaction of patients and the efficiency of reproductive medicine outpatient day surgery, which is worthy of further promotion.

Objective: To construct an evaluation index system of emergency preparedness ability for outpatient emergencies, so as to provide a basis for the evaluation of emergency management capacity of large general hospitals in China. Methods: Based on the "situation-task-capability" analysis framework, an initial assessment index system for emergency preparedness ability of outpatient emergencies was constructed through a retrospective survey and literature review. During April to September 2024, two rounds of Delphi consultation were conducted to determine the importance and relevance of the assessment indicators. Results: After two rounds of consultation, totally 8 first-level indicators, 21 second-level indicators and 77 third-level indicators were formed. The enthusiasm of experts was 100%, the authority coefficient of experts was 0.960, the coefficient of variation was not more than 0.158, the Kendall coordination coefficient was statistically significant, the I-CVI and S-CVI were greater than 0.85. Conclusion: The evaluation index system of emergency preparedness ability for outpatient emergencies can help hospital management and medical staff to evaluate and self-examine their emergency response ability, improve the quality of outpatient management, ensure patient safety, and improve the medical experience of outpatients.

Reliability and validity of the Chinese version of the Epilepsy Self-stigma Scale

Objective: To analysis the reliability and validity of the Chinese version of the Epilepsy Self-Stigma Scale Methods: The Epilepsy Self-Stigma Scale was translated, culturally adapted and pre-surveyed to form the Chinese version of the Epilepsy Self-Stigma Scale. Two hundred and seventy-five epilepsy patients who were treated in the neurology departments of three tertiary grade A hospitals in Hunan province from September to October 2023, and met the inclusion criteria were selected as the subjects. The reliability and validity of the scale was tested. Results: The Chinese version of the ESSS included 3 dimensions and 8 items. The content validity of the items from 0.875 to 1.000, and the content validity of the scale was 0.969. Exploratory factor analysis extracted three common factors, with a cumulative variance explained rate of 90.7%. The results of the validation factor analysis showed the overall model fit was good. The Cronbach's alpha coefficient of the scale was 0.907. The retest reliability was 0.924. Conclusion: The Chinese version of the Epilepsy Self-Stigma Scale has good reliability and validity. It can be used to assess self-stigma of epilepsy patients in China.

Application of a multidisciplinary collaborative interventional program in parturients undergoing premature maternal-infant separation

Objective: To explore the effects of a multidisciplinary collaborative interventional program on lactation level and postpartum Post-Traumatic Stress Disorder (PTSD). Methods: A multidisciplinary collaborative team was organized by the professionals from obstetrics department, the Neonatal Intensive Care Unit, the traditional Chinese medicine department, the nutrition department, and the psychological department. The parturients undergoing maternal and infant separation in the obstetrics department of a tertiary grade A hospital in Qingdao from January to October 2023 was recruited, including parturients from January to May in 2023 in the control group and parturients from June to October in 2023 in the experimental group. The parturients in the control group received routine care, while the parturients in the experimental group received the multidisciplinary collaborative interventional program. The lactation volume, the incidence of delayed initiation of lactation stageⅡand the level of postpartum PTSD were compared between the two groups. Results: There were no significant differences in lactation level on the first day and second day after delivery between the two groups. However, the lactation level on the third day and 14th day after delivery in the experimental group were significantly higher than those in the control group. The incidence of delayed initiation of lactationⅡin the experimental group was significantly lower than that in the control group (P<0.05). The level of postpartum PTSD in the experimental group was lower than that in the control group (P<0.05). Conclusion: The multidisciplinary collaborative interventional program can effectively promote the normal initiation of lactationⅡphase, help to maintain sufficient lactation, and reduce the level of postpartum PTSD in parturients undergoing maternal-infant separation.

Network analysis of symptom clusters and bridge symptoms in elderly with chronic heart failure

Objective: To identify symptom clusters in elderly patients with chronic heart failure, explore the relationships among symptoms within these clusters, and identify the bridge symptoms. Methods: Convenience sampling was employed to select 252 elderly patients with chronic heart failure from two hospitals in Wenzhou. A questionnaire survey was conducted using the Memorial Symptom Assessment Scale-Heart Failure. Symptom clusters were identified through exploratory factor analysis, and network analysis was used to identify the bridge symptoms among these clusters. Results: We identified 4 symptom clusters: respiratory sleep disorder, peripheral edema, dizziness and fatigue, and oropharyngeal discomfort. The top four bridge symptoms were swelling in the legs or arms (rS=0.559, rE=3.756), difficulty breathing when lying flat (rS=0.408, rE=4.556), loss of appetite (rS=0.396, rE=3.999), and dry mouth (rS=0.363, rE=2.199). The symptom expected to have the greatest impact was waking up with shortness of breath at night (rE=4.750, rS=0.202). Conclusion: Swelling in the legs or arms serves as the core bridge symptom in elderly patients with chronic heart failure. Waking up with shortness of breath at night, difficulty breathing when lying flat, loss of appetite, and dry mouth act as bridge symptoms connecting symptom clusters. Targeted interventions for these bridge symptoms, based on their bridging roles among different symptom clusters, have the potential to enhance the effectiveness of symptom management to a certain degree.

Development and validity of the Patient Self-regulation Ability Scale

Objective: To develop a Patient Self-regulation Ability Scale and to test its reliability and validity in patients with rectal cancer after sphincter-preserving surgery, so as to provide an evaluation tool for medical staff to measure the self-regulation ability of patients. Methods: Based on the definition of self-regulation described in the Integrated Theory of Health Behavior Change, combining literature analysis, expert consultations, pre-survey results, and group discussions, the Patient Self-regulation Ability Scale was developed. From August 2022 to March 2023, a total of 354 patients with rectal cancer undergoing sphincter-preserving surgery in the department of rectal surgery of two tertiary grade A hospitals in Beijing were selected by convenient sampling method to test the reliability and validity of the scale. Results: The Patient Self-regulation Ability Scale were with 17 items and 3 dimensions. The content validity index of the scale and item were both 1.00. Three common factors were extracted by exploratory factor analysis, and the cumulative variance contribution rate was 75.06%. Confirmatory factor analysis results showed that the fitting index of the scale structure model was good. Criterion validity was 0.580 (P<0.05). The overall Cronbach's α coefficient of the scale was 0.951 and the test-retest reliability was 0.958. Conclusion: The Patient Self-regulation Ability Scale has good reliability and validity, which is suitable to evaluate the self-regulation ability of patients.

Construction of risk stratification screening indexes for transitional care in patients with Chronic Heart Failure

Objective: To establish risk stratification screening indicators for transitional care in patients with Chronic Heart Failure (CHF), and to provide the basis for risk stratification assessment of transitional care for patients with CHF after discharge. Methods: With the biological-psycho-social medical model as the theoretical framework, literature analysis, research group discussion and Delphi method were used to construct the risk stratification screening indicators for transitional care in patients with CHF. The weight and risk rating of each indicator were determined by the analytic hierarchy process and ABC classification, and a pre-survey was carried out. Results: A total of 2 rounds of expert letter consultation were completed, the positive degree of experts was 88.89% and 100.00%, and the expert authority coefficient was 0.92 and 0.93, respectively. The coordination coefficient of indicators at all levels in the second round of expert letter consultation was 0.377 and 0.130, respectively (all P<0.05). A total of 1 indicator was modified in the pre-survey. Finally, 3 primary indicators (physiological factors of disease, psychosocial factors and treatment factors) and 18 secondary indicators were determined. Conclusion: The risk stratification screening index for transitional care of patients with CHF established in this study is scientific and reliable, which can effectively identify patients with different risk in transitional care, to provide the basis for the implementation of hierarchical transitional care.

Preoperative management of antithrombotic drugs adopted by elderly patients undergoing ambulatory surgery

Objective: To explore the effects of the preoperative management of antithrombotic drugs based on feedforward control model and to provide a reference for the scientific management of antithrombotic drugs in elderly patients undergoing ambulatory surgery. Methods: By means of convenience sampling, 393 elderly patients who received antithrombotic therapy and had their first ambulatory surgery were enrolled. The patients were divided into the control group and the experimental group according to the time of admission. Later the control group was offered conventional management of antithrombotic drugs, while the experimental group received management on the basis of feedforward control model after cause analysis. Results: After the intervention, the cancellation rate of operation caused by antithrombotic drugs in the experimental group was significantly lower than that in the control group (χ2=6.886, P=0.009). Statistically significant differences were observed in the drug treatment adherence between the two groups (χ2=6.658, P=0.037). Moreover, nurses' awareness of the types and names of antithrombotic drugs, the time to discontinue drugs, the names and usage of alternative drugs before and after the training were also statistically different (P<0.05). Conclusion: Preoperative management of antithrombotic drugs based on feedforward control model can reduce the cancellation rate of ambulatory surgery, enhance the medication compliance in elderly patients, optimize nurses' management of antithrombotic drugs to comprehensively improve patients' medical experience.

Construction and practice of whole-process rehabilitation management model based on Health Action Process Approach theory for patients with frozen shoulder

Objective: To construct a whole-process rehabilitation management model based on the theory of Health Action Process Approach for patients with frozen shoulder, and to evaluate its application effects. Methods: Through the literature retrieval and the expert consultations, we constructed a whole-process rehabilitation management model for patients with frozen shoulder based on the theory of Health Action Process Approach. Patients with frozen shoulder admitted to a tertiary grade A hospital in Zhejiang were included by convenient sampling method. Patients admitted from May to July 2022 were set as the control group (n=40), provided with the routine nursing care. Patients admitted from September to November 2022 were set as the observation group (n=40), provided with the whole-process rehabilitation management. The differences of Constant scale score, QuickDASH scale score, NRS score at rest and activity were compared between the two groups at 1st day and 1st month after surgery. Results: After the intervention, the Constant scale score of the intervention group was higher than that of the control group, QuickDASH scale score and the maximum NRS score during activity of the observation group was lower than that of the control group at 1st day and 1st month after surgery (P<0.001). Conclusion: The whole-process rehabilitation management model based on the theory of Health Action Process Approach can relieve the pain of patients with frozen shoulder, promote comfort, and speed up shoulder joint rehabilitation. It can provide new ideas for nurses during the nursing management of frozen shoulder patients.

Current situation and influencing factors of Pressure Injury knowledge among ICU nurses in 14 tertiary grade A hospitals

Objective: To investigate the current situation of mastering knowledge of Pressure Injury (PI) prevention and nursing among ICU nurses in tertiary grade A hospitals in China, analyze influencing factors, provide a basis for in-service training. Methods: From April 30th to July 30th, 2023, eligible ICU nurses from 14 tertiary grade A hospitals were selected by using purposive sampling method to independently answer the knowledge test on PI in adult intensive care patients. Using Logistic regression to analyze the influencing factors of questionnaire scores. Results: A total of 1020 valid questionnaires were obtained, with a correct answer rate of 34.3% to 89.7% for 24 questions and a score of 4 to 24 (16.10±5.58) points. There were 9 questions (37.5%) with a correct answer rate of less than 60%, included 1 question on care of stage 2 PI, 4 questions on identifying PI stages based on pictures, 1 question on the selection and use of pressure reducing mattresses for high-risk patients, 1 question on the repositioning frequency and 2 questions on the prevalence characteristics of PI in ICU patients. The pass rate of the questionnaire was 50.5%, Logistic regression showed that gender, nurse category, and ICU of different departments were the influencing factors. Conclusion: The pass rate of PI knowledge among ICU nurses in 14 tertiary grade A hospitals is only half. Due to gender, different nurse categories, and ICU positions, they mainly lack of PI staging, local treatment, pressure reducing mattresses, repositioning frequency, and epidemiological knowledge. It is recommended to increase the relevant knowledge training that ICU nurses lack when developing in-service training plans, and consider the impact of gender, nurse category, and ICU position to improve training effectiveness.

Evidence summary for the evaluation and intervention of Post-Extubation Dysphagia

Objective: To systematically retrieve, integrate and analyze the relevant evidence for the evaluation and intervention of Post-Extubation Dysphagia (PED), and to provide the best evidence for clinical practice. Methods: A systematic search was conducted in accordance with the "6S" pyramid evidence model across relevant guideline websites and databases both at home and abroad from their inception to June 6, 2024. The search focused on clinical decision-making, best practices, guidelines, systematic reviews, evidence summaries, expert consensus, expert opinion and randomized controlled trials (RCTs) which were related to the assessment and intervention of PED. Evidence was extrated and integrated after quality evaluation of the included studies. Results: A total of 29 studies were included in the final analysis: 2 clinical decision-making studies, 3 guidelines, 8 systematic reviews, 2 evidence summaries, 7 RCTs, 6 expert consensus documents, and 1 expert opinion. The evidence synthesis led to the identification of 25 best practices, which were organized into five key themes: multidisciplinary collaboration, screening and assessment, rehabilitation training, feeding management, and health education. Conclusion: The study summarizes the evidence for improving the swallowing function of patients with PED. When apply the evidence to clinical nursing, health professionals should formulate a personalized plan according to the patients' situation and characteristics.

This can significantly improve the quality of care provided to critically ill neonates and enhance their health outcomes. This article introduces the concept of DSS and explores its current applications in neonatal critical care, including areas such as nutritional support, medication safety, condition monitoring, nursing documentation, and health education. Additionally, it analyzes the barriers encountered by domestic nursing personnel in the development and implementation of DSS and suggests potential solutions, so as to provide insights for the creation of more specialized and systematic decision support systems for neonatal care.

Objective: To explore the effectiveness of a full cycle breastfeeding management program for high-risk infants based on chain theory. Methods: A historical controlled study was conducted and high-risk infants who were treated in a tertiary grade A hospital were recruited. The high-risk infants from October 2021 to September 2022 were treated as the control group while the high-risk infants from October 2022 to September 2023 were treated as the interventional group. Infants in the control group received routine breastfeeding education and management, while infants in the interventional group received a full cycle breastfeeding management program based on the chain theory. The breastfeeding knowledge, self-efficacy, and quality of breastfeeding between the two groups of infants' mothers were compared between the two groups. The start time of breastfeeding, maintenance rate of breastfeeding, pure breastfeeding rate, and growth and development status of infants in the two groups were also compared. Results: The knowledge, self-efficacy, and quality of breastfeeding of mothers in the interventional group were significantly higher than those in the control group (P<0.05). Infants in the interventional group showed significantly improved start time of breastfeeding, maintenance rate of breastfeeding (at discharge, 42 days after birth, 3 months old, and 6 months old), and pure breastfeeding rate than the control group (P<0.05). The height and weight of infants in the interventional group were significantly higher than those in the control group at 42 days of follow-up, 3 months, and 6 months (P<0.05). Conclusion: The full cycle breastfeeding management program for high-risk infants based on chain theory can effectively shorten the initial breastfeeding time of high-risk infants, improve the maintenance rate of breastfeeding and pure breastfeeding, and promote growth and development.

Objective: To develop the comfort experience scale for adult ICU patients with mechanical ventilation and test its reliability and validity to provide a basis for measuring the comfort status of mechanical ventilation patients in ICU in China. Methods: Based on the Kolcaba comfort theory model, the dimensions and item pool of the scale were constructed through literature review, qualitative integration, focus group discussion, and reference to previous scales. The initial scale was formed using the Delphi method and pre-investigation. From May 2022 to June 2024, 285 patients with mechanical ventilation in the ICU were finally selected by convenience sampling method for investigation to test the reliability and validity of the scale. Results: The final intensive care comfort experience scale for adult ICU patients with mechanical ventilation included 24 items, and four common factors were extracted by exploratory factor analysis, with a cumulative variance contribution rate of 67.185%. Cronbach's α coefficient of the total volume table was 0.943, the split-half reliability was 0.841, and the Content Validity Index of the entries and the Content Validity Index of the scale were both 1. Confirmatory factor analysis showed that the scale had good structural validity. Conclusion: The comfort experience scale of adult ICU patients with mechanical ventilation is reliable and valid. It can evaluate the comfort status of mechanical ventilation patients in the ICU.

Objective: To systematically integrate nurses' attitudes and expectations towards nursing robots, so as to provide references for clinical practice. Methods: Qualitative studies on nurses' attitudes and expectations of nursing robots in Chinese and English databases were searched from inception to 31 March 2024. The quality of the literature was evaluated, and the results were consolidated by the aggregative integration method. Results: A total of 10 studies were included, and 34 topics were extracted, which were summarized into 11 new categories and 3 integrated results, including: affirming and accepting of nursing robots; concerns and challenges in applying nursing robots; desiring scenarios for care robot applications. Conclusion: Nurses have positive attitudes toward nursing robots and expect a wide range of scenarios for application, although there are still some concerns. In the future, nursing robots should be developed, and the clinical application scenarios should be expanded, to provide references for intervention programs.

Objective: To develop a nursing handover information system and observe its application effects to improve the quality of clinical nursing handover. Methods: Based on the formation of a multidisciplinary team and the development of a structured nursing handover form, a nursing handover information system was developed based on the electronic medical record system. The system included four main functional modules: handover report, historical record query, quality control management and statistical analysis. The system was implemented hospital-wide. Comparisons were made before and after system application regarding nurse handover preparation time, completeness of handover records, and error rate of handover report information. Additionally, nurses' feedback on the system was surveyed. Results: After the application of the nursing handover information system, the time required for nurses to prepare for handover was reduced by 14.05 minutes (P<0.05). The completeness rate of handover records has improved from 93.13% to 98.14% (P<0.05), and the error rate in handover reports has decreased from 8.40% to 0.06% (P<0.05). Nurses rated their satisfaction with the system highly (mean score for each item >4). Conclusion: The application of nursing handover information system can improve the quality of handover, improve the correctness, completeness and standardization of handover content, further strengthen the closed-loop management of clinical handover, and ensure the effective implementation of handover work system on the premise of improving clinical nursing work and management efficiency.

Objective: To construct, analyze and assess the information platform for nursing risk management based on checklist management, in order to provide reference for nursing risk prevention and control. Methods: Clinical nursing risk points were found under the guidance of 4R crisis management theory. Then evidence was systematically retrieved and integrated combining with clinical situations to form a risk management checklist to finally build the information management platform, which was applied to practice from January 2021. The differences between the number of adverse nursing events, service efficiency, nurse satisfaction were compared before and after the application of the platform. Results: A total of 342 checklists were established and implanted into the information platform referring to 8 aspects, including high-risk drugs, instruments and equipment, high-risk nursing practices, disease complications, education and nurse-patient communication, risks of specialized wards, nursing-sensitive indicators, etc. After the application, the incidence of nursing adverse events decreased from 246.74/100,000 to 42.48/100,000, and the annual percentage change was -61.50% (P<0.001); the service efficiency and nurse satisfaction increased steadily with remarkable effects. Conclusion: The information platform for nursing risk management based on checklist can effectively reduce the occurrence of nursing adverse events and improve their service efficiency to provide certain reference for clinical practice.

Objective: To develop an intelligent management platform for hand hygiene. Methods: An intelligent management platform was developed based on the integrate artificial intelligence vision analysis technology and accurate positioning technology. To develop the platform, a data set for training the model of objective detection was built, a logic for touch detection was set up, and the internet of things was also integrated. The intelligent management platform can monitor and remind the hand hygiene of health professionals. The effects of the intelligent platform were observed through compliance with the hand hygiene through big data and direct observation. Besides, the relevant indicators of nosocomial infection were also used for the effects evaluation. Results: The compliance rate of hand hygiene monitored by the platform was consistent with that of direct observation (P<0.05). After the launch of the platform, the compliance rate of hand hygiene was increased, the detection rate of multi-drug resistant bacteria was decreased, and the consumption of hand disinfectant was increased (P<0.05). Conclusion: The intelligent management platform for hand hygiene based on artificial intelligence and internet of things can comprehensively and accurately record the performance of hand hygiene of health professionals, improve the compliance rate of hand hygiene, and decrease the nosocomial infection. The intelligent platform can make hand hygiene management more refined, intelligent and provide data support to clinical decisions for hospital administrators.

Analysis of the mediating effect of professional identity on the relationship between hospital ethical climate and the moral sensitivity of hemodialysis nurses

Objective: To analyze the mediating effect of nurses' professional identity in hemodialysis room on the relationship between hospital ethical atmosphere and moral sensitivity, so as to provide a reference for improving the level of moral sensitivity of clinical nurses. Methods: From December 2023 to April 2024, totally 218 nurses in hemodialysis room from 6 general hospitals in Shandong province were recruited by convenient sampling method, and the general information questionnaire, nurse professional identity rating scale, hospital ethical atmosphere scale and nurses' moral sensitivity questionnaire were used to investigate. Results: The scores of moral sensitivity, professional identity, and hospital ethical atmosphere of 218 nurses in hemodialysis room were 40.35±7.45, 118.52±19.58, and 104.64±16.47 respectively. Hospital ethical atmosphere was positively correlated with professional identity (r=0.709, P<0.001) and moral sensitivity (r=0.536, P<0.001), and professional identity was positively correlated with moral sensitivity (r=0.560, P<0.001). Professional identity played a mediating role in the relationship between hospital ethical atmosphere and the moral sensitivity of nurses in hemodialysis room, and the mediating effect accounted for 47.93% of the total effects. Conclusion: The professional identity of nurses in hemodialysis room is a mediating variable of hospital ethical atmosphere and moral sensitivity. Hospital managers can enhance the professional identity of nurses by creating a good ethical atmosphere, thereby improving the moral sensitivity of nurses and improving the quality of nursing services.

Objective: To analyze the mediating effect of clinical nurses' work engagement on perceived management care and person-organization fit, so as to provide reference and basis for managers to formulate management measures and improve the person-organization fit level of clinical nurses. Methods: Clinical nurses from 6 tertiary grade A general hospitals in Beijing and Hubei province were selected as subjects in November 2023 by convenience sampling method. General data questionnaire, person-organization fit scale, Caring Assessment Tool-administration and Utrecht Work Engagement Scale were used as investigation tools, and Amos 26.0 software was used to construct the mediation effect model. Results: A total of 1480 clinical nurses were investigated. The person-organization fit score was 183.74±25.89, the total score of perceived management care was 131.68±23.58, and the total score of work engagement was 51.49±10.96, all of which were positively correlated (P<0.05). Work engagement had a partial mediating effect between perceived management care and person-organization fit, and the mediating effect size was 0.112 accounting for 16.20% of the total effect. Conclusion: Work engagement is the mediating variable between perceived management care and person-organization fit. Nursing managers can stimulate the level of work engagement of nurses by improving their management care level and literacy, so as to improve the person-organization fit level of nurses.

Objective: To explore the current situation and latent characteristics of nurses' career calling in China, and to analyze the influencing factors, so as to provide reference for improving nurses' career calling. Methods: A total of 4797 nurses from six tertiary general hospitals were enrolled in January 2023 by convenience sampling. A general information questionnaire and Chinese Calling Scale were used for data collecting, the latent profile of nurses' career calling was analyzed, and the influencing factors of different types of career calling were explored by Logistic regression. Results: The overall score of nurses' career calling was 37.74±7.93, which was categorized into four categories, including: occupational alienation type (5.8%), low-orientation type (22.7%), high-altruistic-contribution type (43.1%), and high-motivation type (28.4%). The regression analysis results showed that the average monthly income, job satisfaction, marital status, and work department were the influencing factors (P<0.05). Conclusion: Nurses' career calling is at a moderate to high level with heterogeneity. Nursing managers can provide corresponding supports for different categories.

Objective: To investigate the fear of disease progression among caregivers of children with malignant solid tumors and analyze its influencing factors, so as to provide a reference for developing targeted interventions. Methods: Convenience sampling was used to select 211 caregivers of children with malignant solid tumors in a tertiary grade A hospital in Shanghai from November 2022 to November 2023. The survey was conducted using the general data questionnaire, Simplified Fear of Disease Progression Scale (Caregiver version), Social Support Rating Scale, Simple Coping Style Questionnaire, and Hospital Anxiety and Depression Scale. Multiple linear regression analysis was used to analyze the influencing factors. Results: The score of fear of disease progression in the caregivers of children with malignant solid tumors was 38.95±9.66. Multiple linear regression analysis showed that caregivers' gender, economic level, negative coping, social support, anxiety were the influencing factors of caregivers' fear of disease progression, which could explain 50.0% of the total variation. Conclusion: The fear of disease progression in the caregivers of children with malignant solid tumors is at a high level. Healthcare professionals should develop targeted intervention strategies according to relevant influencing factors to improve the quality of life of these caregivers.

Objective: Exploring the intervention effect of modified Mindfulness-Based Cognitive Therapy on psychological rehabilitation of patients with head and neck cancer after surgery. Methods: By convenience sampling, forty-eight patients after head and neck cancer surgery in a tertiary grade A hospital in Beijing from April 2021 to September 2022 were selected. They were divided into control and observation groups according to the random number table method. The control group received conventional psychological care, while the observation group received modified Mindfulness-Based Cognitive Therapy for psychological intervention. The anxiety, depression, sleep quality, and satisfaction levels were compared between the two groups before and after the intervention. Results: After 6 weeks of intervention, the anxiety, depression, and sleep quality scores of the observation group patients were lower than those of the control group, and the satisfaction scores were higher than those of the control group (P<0.05). Conclusion: The application of modified Mindfulness-Based Cognitive Therapy for rehabilitation training can alleviate anxiety and depression in patients with head and neck cancer undergoing tracheotomy and improve sleep quality and satisfaction.

Correlation between the changes of the "five-point temperature" in the flap area and the blood supply and prognosis after flap transplantation

Objective: To analyze the relationship between the temperature changes of the flap after flap transplantation, blood supply, and prognosis, in order to timely detect skin flap blood flow disorders, reduce the incidence of complications, improve nursing quality, and improve patient prognosis. Methods: Sixty-five hospitalized flap transplantation patients were included from December 2021 to December 2023. Starting from the first postoperative day, the "five-point temperature" of the flap, capillary response, and flap color were observed every 2 hours for 5 consecutive days. The flap's blood supply, hospitalization duration, and prognosis were recorded. Results: The "five-point temperature" on postoperative days 1 to 5 were from 35.87±0.48 °C to 36.20±0.18 °C. Vascular crises occurred in 11 patients (16.9%), with 10.8% (n=7) experiencing venous crises and 6.2% (n=4) arterial crises. Correlation analysis showed that the "five-point temperature" on postoperative days 1 to 3 was negatively correlated with vascular crises (r=-0.621, -0.523, -0.462; all P<0.001) and flap necrosis (r=-0.492, -0.649, -0.739; all P<0.001). In the normal blood supply group, the flaps achieved primary healing with an average hospital stay of 24.24±10.97 days. In the venous crisis group, the flaps survived after treatment, with an average hospital stay of 39.00±11.51 days. In the arterial crisis group, the flaps experienced necrosis requiring additional surgery, with an average hospital stay of 52.33±14.30 days. The differences in hospital stay among the three groups were significant (P<0.001). Conclusion: The "five-point temperature" of the flap gradually increased from postoperative days 1 to 5. If the temperature is below 36.05 °C on days 1 to 3, vascular crisis should be suspected, and if it is below 35.70 °C, flap necrosis should be suspected. Therefore, accurately monitoring flap temperature is crucial for the early detection and management of vascular crises and necrosis, as well as for improving flap prognosis.

Objective: To construct a self-management program for patients with rectal cancer after sphincter-preserving surgery based on the individual and family self-magement theory and evaluate its application effects, so as to provide reference for improving patients' symptoms. Methods: Using literature review, group discussion, semi-structured interviews and expert correspondence, a self-management program for patients with rectal cancer after sphincter-preserving surgery was constructed. Patients with rectal cancer after sphincter-preserving surgery in a tertiary hospital in Tianjin from January 2021 to August 2023 were selected by convenience sampling, and the patients were divided into a control group (48 cases) and an intervention group (48 cases) according to the order of admission. Those in the control group received routine nursing care, while those in the intervention group were with self-management program for 3 months. The incidence of symptoms of Low Anterior Resection Syndrome (LARS), self-management ability and quality of life were compared between the two groups before and after intervention. Results: The incidence of LARS in the intervention group was lower than those in the control group (P<0.05), the scores of self-management was higher than that in the control group (P<0.05), and the score of quality of life was lower that in the control group (P<0.05). Conclusion: The self-management program for patients with rectal cancer after sphincter-preserving surgery can alleviate the symptoms of LARS, improve the self-management ability and quality of life of patients.

The increasing population aging has posed a huge challenge to the healthcare worldwide. The World Health Organization (WHO) has pointed out the concept of "healthy aging" and the Integrated Care for Older People (ICOPE) model. Currently, ICOPE has been practiced in several countries, including China (Beijing), yet with fewer reports on it. This paper systematically analyses the current status of the implementation of the WHO ICOPE model at home and abroad and puts forward relevant insights into the practice of ICOPE in China, with a view to providing a reference for the introduction and promotion of ICOPE in China in the future.

This article reviews and summarizes recent national and international health economics research on the prevention and control of CLABSI, with the aim of providing reference and guidance for future studies in this area.
