The status quo and countermeasures of nursing post management in China

Objective: To analyze the current situation of nursing post management in China and provide suggestions for its further development. Methods: Fully consided the economic development level, level characteristics, and regional conditions of each region, we conducted an online questionnaire survey in 162 hospitals nationwide and field research in 6 hospitals to review the nursing position setting and human resource allocation in some hospitals in China. Results: The average total number of registered nurses in the surveyed hospitals was 946.26, of which 55.61 (5.88%) were nursing management personnel. Most hospitals had 5 levels of nursing job setting (69.14%), which were basically divided into three major categories: nursing management, clinical nursing, and other positions, with significant variation in the latter two areas. Medical institutions mainly relied on the bed-to-nurse ratio required by national policy documents and actual clinical work to allocate nursing personnel. Conclusion: The classification of nursing positions in different regions is basically the same yet with differences in the hierarchical management system. The arrangement of nursing positions in various categories is unreasonable, and the connotation of post settings needs to be deepened with a patient-centered approach.There is a relative shortage of nursing manpower in some departments, and a more scientific method of manpower allocation needs to be explored.

Evidence-based practice in the prevention and management of pregnancy-related low back pain during pregnancy

Objective: To explore evidence-based prevention and management programs for pregnancy-related low back pain, and evaluate the application effects, so as to provide reference for pregnancy management. Methods: Evidence-based methods were used to build the evidence application strategy, and implemented in the Obstetrics Clinic of a tertiary grade A hospital in Henan province from December 2020 to June 2022. The indicators of patients before and after the application of evidence and the implementation rate of the reviewed entries were compared. Results: After the implementation of the evidence, there was no statistically significant difference in the incidence of pregnancy-related low back pain compared to before implementation (P>0.05). The pain degree of pregnant women with pregnancy-related low back pain from the third assessment was lower than that of the baseline review group, and the average pain degree was lower than that of the baseline review group at the four times after implementation (P<0.05). In terms of disability, the degree of disability after implementation from the second evaluation was lower than that of the baseline review group, and the average disability level after implementation was lower than that of the baseline review group (P<0.05). Conclusion: The evidence of pregnancy-related low back pain during pregnancy can guide nursing practice, help to reduce the degree of pregnancy-related low back pain and dysfunction index, and standardize pain management behavior.

Construction of a grief counseling program for family members of patients with sudden death in the emergency department

Objective: To construct a grief counseling program for family members of patients with sudden death in the emergency department, and to alleviate their grief and reduce the risk of prolonged grief disorder. Methods: The grief counseling program was developed based on literature review and clinical experience. The program was further revised and its feasibility was demonstrated through the Delphi method. Results: The authority coefficient of the Delphi expert consultations was 0.904. After the second round of expert consultations, the coefficients of variation for the importance and feasibility of each item ranged from 0.05 to 0.24, respectively. The Kendall concordance coefficients were 0.174 and 0.215 (P<0.001). The final program comprised 4 primary items, 9 secondary items, and 28 tertiary items, including the establishment of a grief counseling team, assessment of grief counseling needs, implementation of grief counseling, and follow-up. Conclusion: The grief counseling program for family members of patients with sudden death in the emergency department is scientific, practical, and reliable, providing a reference for implementing grief counseling for family members of sudden death patients in clinical settings.

Objective: To explore the development of professional values and identity among senior nurses with over 30 years of nursing experience. Methods: Qualitative interviews were conducted with 21 nurses with over 30 years of nursing experience coming from different levels of public and private hospitals in Zhejiang province, and thematic analysis was employed later. Results: Totally 4 themes and 10 sub-themes were extracted from the interview data. The four themes were: (1) the altruistic/egoistic value of nursing profession; (2) the formation of nursing professional values; (3) self/other-identity of professional value; (4) the inspiration of professional identity for the sustainable development of the nursing team. Conclusion: Senior nurses recognize altruism as the core professional values and have achieved their own sense of professional benefits. In order to support the realization of altruistic values, it is necessary to pay attention to challenges faced by senior nurses in their career development, so as to improve the professional environment and stabilize the nursing team.

Objective: To translate the Nurses' Attitudes towards Futile Treatment Scale (NAFTS) into Chinese, and to test its reliability and validity, and to provide a tool for the assessment of nurses' attitude towards futile treatment in China. Methods: The Chinese version of the NAFTS was developed by translation, back translation and cultural debugging based on the Brislin translation model. In September 2023, a convenience sample of 420 nurses in ICU and oncology departments from six tertiary grade A hospitals in Zhejiang province were selected to test the reliability and validity of the scale. Results: The Chinese version of NAFTS were with 4 dimensions and 18 items. The Cronbach's α was 0.901 and the re-test reliability was 0.805, the correlations of the scores between each item and the total scale were ranged from 0.451 to 0.714 (P<0.05), the I-CVI ranged from 0.830 to 1.000, the S-CVI/UA was 0.833, and the S-CVI/Ave was 0.972. Four common factors were extracted from exploratory factor analysis, and the cumulative variance contribution rate was 66.195%. The results of confirmatory factor analysis showed that the model fitted well. Conclusion: The Chinese version of NAFTS has good reliability and validity, and it can be used as an effective tool to evaluate the nurses' attitudes towards futile treatment in China.

Implementation of the progressive early activity plan in ICU patients undergoing mechanical ventilation

Objective: To explore the effectiveness and safety of implementing a progressive early activity plan based on the International Classification of Functioning, Disability and Health(ICF) concept for ICU mechanically ventilated patients, so as to provide a reference plan for early activity of ICU mechanically ventilated patients. Methods: Totally sixty eight mechanically ventilated patients admitted to the intensive care department of a tertiary grade A comprehensive hospital in Hunan province from January to November 2022 were recruited and randomly divided into a control group and an intervention group. The control group carried out routine nursing care and functional exercises for ICU mechanically ventilated patients, while the intervention group received a progressive early activity plan based on the ICF concept, which included physical function training, daily activity training, and environmental sound and light control. The mechanical ventilation time, ICU stay time, muscle strength, incidence of lower limb deep vein thrombosis, delirium, ICU acquired weakness, adverse events, and potential incidence of adverse events were compared between the two groups of patients. Results: The intervention group had less mechanical ventilation time and ICU hospitalization time than the control group (P<0.05). The incidence of delirium and ICU acquired weakness in the intervention group was lower than that in the control group (P<0.05). The intervention group had higher muscle strength than the control group (P<0.05). There was no statistically significant difference in the incidence of adverse events between the two groups (P>0.05). The potential incidence of adverse events in the intervention group was higher than that in the control group (P<0.05). Conclusion: The progressive early activity program based on the ICF concept is an effective and safe intervention program that can shorten the mechanical ventilation time and ICU hospitalization time of ICU patients, prevent delirium and ICU acquired weakness, and improve patient muscle strength. At the same time, ICU medical staff need to pay attention to the prevention of potential adverse events during early activity of mechanically ventilated patients to ensure the safety of early activity implementation.

Objective: To apply the evidence related to children's peripheral venous indwelling needles in the 8th edition of the "Practice Standards for Infusion Therapy" to clinical practice and evaluate its effectiveness. Methods: From July 2022 to March 2023, totally 24 department nurses and pediatric patients who were admitted to the Rheumatology and Immunology Department of a tertiary children's specialty hospital in Beijing and required indwelling needle infusion treatment were recruited. We adopted the evidence conversion model of Fudan University's Evidence Based Nursing Center and formulate policies based on clinical conditions. A survey questionnaire was used to investigate the awareness of nurses towards the evidence, and on-site observation and self-report were used to evaluate their compliance with the implementation of the evidence. Results: Application of evidence improved the awareness and compliance of nurses in puncture and maintenance of the evidence. Evidence was applied, which effectively prolonged the indwelling time (P<0.001) without increasing the incidence of indwelling needle phlebitis (P=0.205), significantly reduced venous treatment related costs (P<0.001), and thus reduced hospitalization costs. Conclusion: The application of evidence can improve the nursing quality of indwelling needles. Without increasing complications, the retention time of the indwelling needle can be prolonged, the number of punctures and hospitalization costs for the patient can be reduced , thereby reducing the workload of nurses and saving manpower.

Objective: To search, evaluate and summarize the relevant evidence on the prevention and management of Ventilators Associated Pneumonia (VAP) in children in Neonatal Intensive Care Unit (NICU), and to provide implications for clinical practice. Methods: Evidence on neonatal VAP prevention and management was systematically searched from Chinese and English databases, guideline websites and related association websites. Searching was from January 2013 to August 2023. Literature quality evaluation, evidence extraction and summary were performed independently by researchers. Results: A total of 14 articles were included, consisting of 23 pieces of evidence. The evidence were summarized in risk factor identification, oral care, endotracheal suction, head elevation, ventilator management, medication use, holistic care. Conclusion: The evidence summarized in this study is comprehensive and practical, allowing healthcare professionals to follow the evidence to provide prevention and management strategies for VAP in neonates receiving mechanical ventilation in NICU, thereby reducing the occurrence of VAP.

Reliability and validity of the Chinese version of the Social Inclusion Scale for parents of children with autism spectrum disorder

Objective: To translate the Social Inclusion Scale (SIS), and to test its reliability and validity among parents of children with Autism Spectrum Disorder (ASD). Methods: The original English version of SIS was translated and back-translated according to the Brislin model. The Chinese version of the scale was finalized after cultural adaptation and pretest on the scale. A total of 679 parents of children with ASD aged 3 to 14 years old in 33 special children's rehabilitation institutions in Hunan Province were investigated from July 2023 to January 2024 to test the reliability and validity of the Chinese version of SIS. Results: There were 19 items in the Chinese version of SIS, the Item-level Content Validity Index (I-CVI) was 0.83 to 1.00, the consensus Scale-level Content Validity Index (S-CVI/UA) was 0.8, and the average Scale-level Content Validity Index (S-CVI/Ave) was 0.908. The exploratory factor analysis identified four factors, and the cumulative variance contribution rate was 57.973%. The confirmatory factor analysis showed that χ2/df=2.456, RMSEA=0.066, GFI=0.904, AGFI=0.873, PNFI=0.727, PGFI=0.682, IFI=0.913, CFI=0.912, which indicated that the fitting degree of each index was good. Conclusion: The Chinese version of SIS has good validity and reliability, and can be used to evaluate the social inclusion of parents of children with ASD in the context of Chinese culture.

Objective: To evaluate the effectiveness of Pediatric Parenteral Nutrition Intravenous Administration Evidence-adaptive Clinical Decision Support System (PN-CDSS) on nurses' compliance with safe Parenteral Nutrition (PN) administration behaviors. Methods: From November 2021 to August 2022, a step-wedge cluster randomized controlled trial of the implementation plan of PN-CDSS was conducted in five wards of a tertiary grade A pediatric specialty hospital in Shanghai. Nurses in these departments were intervened, with each ward being included in the step as per the randomization results. The compliance of nurse medication safety behaviors before and after implementation were compared. Results: After the implementation of PN-CDSS, the rate of pre-administration assessment for PN increased from 0 to 100.0%. And significant improvements were observed in the double-checking rate for PN and the execution rate of PN infusion via a Central Venous Catheter (P<0.05). However, there was no significant change in the rate of hourly assessment for PN (P>0.05). Conclusion: The PN-CDSS aid pediatric nurses in conducting pre-administration assessments, performing double checks and choosing right catheter, thereby enhancing the nursing medication administration safety behaviors.

Application of optimizing health education programs for home symptoms monitoring in elderly patients with Heart Failure

Objective: To explore the effects of improving the home-based symptoms self-monitoring ability in elderly patients with Heart Failure (HF) through optimizing the health education programs based on symptoms management theory, aiming to provide reference for home-care of such patients. Methods: Totally 115 elderly patients with HF admitted to the cardiology department of a tertiary grade A hospital in Jiangsu province from April to September 2023 were recruited as the research participants. The patients were randomly divided into an observation group (n=58) and a control group (n=57) using a random number table method. The control group received routine health education, while the observation group implemented an intervention plan based on symptoms management theory to optimize the health education programs on the basis of routine health education. The mastery of physical perception and self-monitoring symptoms of HF was compared between two groups of patients at three time points: relief of acute HF symptoms, discharge, and 3 months after discharge. The readmission of two groups of patients due to acute HF was followed up for 3 months after discharge. Results: The two groups of patients showed comparability in various indicators after the relief of acute HF symptoms. The mastery of self-monitoring symptoms content in the observation group before discharge was better than that in the control group (P<0.05). The physical perception and self-monitoring symptoms and readmission rate of HF in the observation group after 3 months of discharge were significantly better than those in the control group (P<0.05). Conclusion: Implementing forward discharge guidance and optimizing the health education programs based on symptoms management theory can help improve the home-based symptoms self-monitoring ability in elderly patients with HF and reduce the recurrence rate of acute HF.

Development and application of a health education program for patients with PICC catheterization

Objective: To develop a health education program for patients with PICC catheterization based on Timing It Right theory and to examine the program effects among patients with PICC. Methods: A comprehensive literature review, group discussions, and the Delphi technique were employed to inform the health education program development for patients with PICC and the program is also guided by Timing It Right theory. A total of 67 patients who underwent PICC catheterization in the hematology department of a tertiary grade A hospital from March to July 2023 were recruited and assigned to the control group. Standard nursing care was provided to these patients. Another 64 patients from August to December 2023 were assigned to the experimental group and the health education program was applied in this group. The self-management ability and complication rates of PICC catheterization were compared between the two groups at baseline, one month post-catheterization, and three months post-catheterization. Results: The significant group and time effects were observed on the total self-management ability scores between the two groups (P<0.05). One month following the intervention, no significant differences were detected in the incidence of catheter-related complications between the two groups (P>0.05). However, three months post-intervention, patients in the experimental group exhibited a lower incidence of catheter-related complications compared to those in the control group (χ2=4.518, P<0.05). Conclusion: The health education program developed for PICC patients based on Timing it Right theory is scientifically sound, reliable, and practical. It enhances the self-management ability of patients with PICC and reduces the occurrence of PICC-related complications. This program offers a scientific, practical, and comprehensive framework for health education in patients with PICC catheterization.

Construction and application of regional collaborative treatment platform for diabetic foot

Objective: To develop a regional collaborative treatment platform for diabetic foot and explore its application effect, and to provide a reference for the continuing care. Methods: The platform was used in one tertiary hospital and twelve community health service centers within the same medical alliance network, and online technical support, offline consultation, referral, education, and other services were provided by experts from the tertiary hospital. The cure rate and improvement rate of diabetic foot were compare before and after using the platform, and the usage of the platform and the satisfaction of nurses were analyzed. Results: The healing rate of grade 1 and grade 2 diabetic foot in the experimental group was higher than that in the control group at 4 weeks (P<0.05), the healing rate of grade 2 diabetic foot in the experimental group was higher than that in the control group at 6 weeks (P<0.05), and the improvement rate of grade 4 diabetic foot was higher at 12 weeks after the usage of the platform in the experimental group than those in the control group (P<0.05). The satisfaction with the platform of the liaison nurses in the tertiary hospital was 96.6%, and the satisfaction of community nurses was 100.0%. Conclusion: The development and application of the regional cooperative treatment platform for diabetic foot can promote the wound healing of diabetic foot, improve the satisfaction of nurses, and contributed to the continuous nursing care.

Application of mHealth in the continuous care of enterostomy patients: a scoping review

Objective: To conduct a scope review on the application of mHealth in the continuing care of enterostomy patients and summarize the intervention forms, content elements, outcome indicators, and effects of mobile health to provide reference for future research in this field. Methods: Based on the methodological framework of scope review guidelines issued by the Joanna Briggs Institute (JBI), we systematically searched PubMed, Embase, Web of Science, The Cochrane Library, China National Knowledge Infrastructure, WanFang Data and VIP databases from their inception to February 2024 Then the included literature were summarized and analyzed. Results: A total of 18 articles were included. The intervention forms of mHealth included mobile apps, WeChat mini-programs, remote conferences, and website platforms. The content elements mainly referred to health education, health monitoring, psychological nursing, professional staff support, and peer support. The outcome indicators contained knowledge, attitude and behavior related to stoma, complication outcomes, quality of life, severity of symptoms, patient experience and resource utilization. Conclusion: The application of mHealth in continuing care for patients with colostomy is feasible with positive influences. Future research should pay more attention to peer education, the friendliness of human-machine interaction, and patient privacy protection. Based on theoretical frameworks and intelligent information technology, remote education and health management to further improve the quality of life for colostomy patients remain to be explored.

Objective: To apply the evidence of nursing care of gastrointestinal symptom clusters in colorectal cancer patients with chemoradiotherapy and evaluate the effects, so as to provide reference for symptom management. Methods: Based on Knowledge to Action Framework (KAT), the evidence-based nursing practice of gastrointestinal symptom clusters in colorectal cancer patients with chemoradiotherapy was carried out from December 2021 to November 2022, which included three steps: knowledge output, knowledge introduction and knowledge application. The knowledge mastery level of medical staff and implementation rate on the review indicators, and the incidence of gastrointestinal symptoms were compared before and after the application of evidence. Results: After the evidence-based nursing practice, the knowledge scores of medical staff increased from 52.27±14.73 to 77.00±7.44, and the implementation rate of 14 review indicators increased from 0-91.11% to 71.43%-100.00%, and the incidence of vomiting and diarrhea in the intervention group was lower than that in the control group (P<0.05). Conclusion: KTA provides guidance for evidence-based nursing practice, which is benefit to improve the relative knowledge and behavior of medical staff and decrease the incidence of gastrointestinal symptoms in colorectal cancer patients.

Qualitative research on the experience of Virtual Reality rehabilitation for patients with stroke: a Meta-synthesis

Objective: To systematically review and synthesize the experience of Virtual Reality rehabilitation for patients with stroke, in order to provide references to improve the accessibility and experience of Virtual Reality rehabilitation for patients with stroke. Methods: We searched domestic and foreign databases from inception to December 1st, 2023, to collect qualitative studies related to the experience and needs of Virtual Reality rehabilitation for patients with stroke. The quality of included studies was evaluated according to JBI Critical Appraisal Tool for qualitative studies. Meta-synthesis was used to integrate results. Results: A total of 16 studies were included, and 56 findings were extracted and grouped into 10 new categories. These categories were integrated into three synthesis results, including perceived benefits for participating in Virtual Reality rehabilitation in patients with stroke, perceived barriers for participating in Virtual Reality rehabilitation in patients with stroke, and Virtual Reality rehabilitation needs and expectations of patients with stroke. Conclusion: Virtual Reality technology provides new ideas for the rehabilitation training of patients with stroke, and its effectiveness is acknowledged by most patients with stroke, so it should be given positive guidance. Future research should actively develop a Virtual Reality rehabilitation system adapted to patients with stroke, strengthen the construction of multidisciplinary teams, improve the rehabilitation model, and promote the development and improvement of Virtual Reality rehabilitation for patients with stroke in China.

Objective: To conduct a comprehensive review of the literature on oral care management for patients with stroke, and to provide evidence-based support for the screening and development of oral care management for patients with stroke in China. Methods: The framework of scoping review was used to search the literature on stroke oral care management in 9 databases from the inception to October 13, 2023. Results: A total of 22 studies were included, covering 10 oral symptoms or related complications and a total of 27 outcome indicators. The oral care management included the development of oral intervention tools, the oral organs and surrounding tissues, the intervention measures and the oral assessment. While there was a lack of comprehensive and universal oral care management programs and standardized models for patients with stroke. Conclusion: The oral care management programs for stroke patients varies and are still in the developmental stage, which need to be further validated and improved in clinic practice. Targeted, feasible, and effective oral care management programs should be developed and implemented according to the specific patient and hospital circumstances.

Effectiveness of an early comprehensive rehabilitation program on preventing ICU Acquired Weakness in patients with mechanical ventilation

Objective: To explore the effect of early comprehensive rehabilitation intervention on preventing ICU-Acquired Weakness (ICU-AW) in patients with mechanical ventilation, and to provide reference for effective prevention of ICU-AW. Methods: A total of 86 patients with mechanical ventilation admitted to the ICU of a tertiary grade A hospital in Nanning from January to December 2023 were conveniently included, and the patients were randomly divided into the control group and the experimental group with 43 cases respectively. Those in the control group were with routine care and exercise, and those in the experimental group were with early comprehensive rehabilitation training on the basis of routine care. The incidence of ICU-AW, muscle strength score, time of mechanical ventilation, and other indicators were compared between the two groups. Results: A total of 77 patients, including 38 in the control group and 39 in the experimental group completed the study. The incidence of ICU-AW in the experimental group was lower than that in the control group (P<0.05); the muscle strength score, ICU mobility scale score, and Barthel index score of those in the experimental group were higher than those in the control group (P<0.05); and the duration of mechanical ventilation, analgesia and sedation, and the length of ICU stay of those in the experimental group were shorter than those in the control group (P<0.05). Conclusion: The early comprehensive rehabilitation program is safe and effective, which can improve the muscle strength of patients, effectively prevent the occurrence of ICU-AW, and promote the recovery of patients.

Objective: To develop a volume management nursing protocol in patients with Cardio-Renal Syndrome (CRS), to provide a scientific basis for preventing and managing volume overload and related complications. Methods: Based on the "6S" evidence resource model, a systematic literature search of domestic and foreign databases related to CRS volume management was conducted with the searching period from January 2015 to January 2024. Evidence extraction and content analysis were performed to draft the initial nursing protocol. The initial draft was then further validated and revised through expert meetings to form the final protocol. Results: A total of 13 experts participated in the demonstration meeting, and its authority coefficient was 0.84. Six themes were formulated in the nursing protocol on volume management for CRS, including admission assessment, condition monitoring and management, dynamic volume assessment and management, nutritional support care, health education, and discharge follow-up, with a total of 21 specific implementation contents. Conclusion: The constructed nursing protocol comprehensively summarizes the key points of CRS volume management, which is scientific and feasible. It can provide guidance for medical staff to carry out CRS volume management.

Effects of complete lateral decubitus position on oral feeding ability in elderly patients with oral frailty and physical frailty

Objective: To evaluate the effect of complete lateral decubitus position on oral feeding ability in elderly patients with oral frailty and physical frailty, and to provide reference for medical personnel to improve the nutritional status of the patients. Methods: Totally 70 elderly patients with oral frailty and physical frailty were enrolled from January to December 2023 in the department of geriatrics of a hospital in Suzhou, and they were randomly divided into the control group and the experimental group with 35 cases in each group. Those in the experimental group were with the full lateral position through oral feeding, and those in the control group took the 60° semi-seated position or 90° upright position (patients retained their original eating habits). The oral feeding ability and other indicators were compared. Results: After 8 weeks of intervention, the oral feeding ability, the average feeding duration, the average eating times, the average eating volume per meal, the nutritional status of those in the experimental group were significantly better than those in the control group, the frailty score and the incidence of aspiration were lower than those in the control group (all P<0.05). Conclusion: The oral feeding ability, nutritional status and frailty of the elderly patients with oral frailty and physical frailty can be improved by eating in complete lateral decubitus position, and the incidence of aspiration can also be greatly reduced.

Objective: To apply the evidence on the management of exudates from malignant fungating wounds, and evaluate its effectiveness on improving the exudates management for malignant fungating wounds and quality of care in clinical setting. Methods: Based on the clinical evidence practice and application model of JBI, review indicators were formulated based on the evidence. Baseline reviews were conducted on 54 nurses and 20 patients from April to October 2022 to analyze the obstacles and advantages and carry out reform. A post-reform review was conducted on 54 nurses and 23 patients from April to October 2023 to compare the implementation rate of review indicators before and after the application of the evidence. The nurses' knowledge level of exudates management for malignant fungating wounds and the condition of patients' exudates were also compared. Results: After evidence-based practice, the implementation rate for 7 review indicators increased from 21.1%-81.2% to 78.6%-100.0%, and the scores of nurses' knowledge increased from 141.69±18.55 to 152.87±16.87 (P<0.001). Wound fluid leakage, odor, periwound skin, and comfort were all improved (P<0.05). Conclusion: The evidence-based practice can improve the level of exudates management for malignant fungating wounds by nurses and increase the effectiveness of exudates management from malignant fungating wounds.

It is an entry point for oral healthcare professionals to implement interventions. This paper reviewed the illness perception in periodontal patients from the following areas: current situation, assessment tools, influencing factors, and intervention measures. It is expected to enhance the attention of clinical healthcare professionals on illness perception of periodontal patients, and to provide reference for practice on this area.

First-degree relatives of patients with colorectal cancer, as a high-risk group of colorectal cancer, should undergo early and intensive colorectal cancer screening compared to the average-risk population. However, the participation rate of first-degree relatives screening was lower than expected, and their colorectal cancer incidence and mortality are higher than the average-risk population. This paper aims to review the status, influencing factors, and promotion measures of colorectal cancer screening behaviors in first-degree relatives of patients with colorectal cancer. The findings will provide references for promoting early screening and prevention of colorectal cancer among first-degree relatives.
