Promotion safety of catheters in pediatric patients: the status quo of evidence-based nursing practice and challenges

The safety of catheters is important for pediatric patients in the process of diagnosis and treatment, and plays a crucial role in a variety of medical scenarios. Catheter safety in pediatric patients has become an important issue of nursing quality management. This article aimed to elaborate the current status of evidence-based nursing practice of safety management of catheters in pediatric patients, the clinical benefits obtained, the challenges faced, and the implementation strategies to promote evidence-based nursing practice. It is expected to provide information of safety management for pediatric nurses.

Objective: To examine pediatric nurses' behavior in managing Peripherally Inserted Central Catheters (PICC) extubation and identify influencing factors. Methods: A survey was conducted among 230 pediatric nurses from 16 tertiary grade A hospitals in Beijing using convenience sampling. The survey included questions on nurses' knowledge, attitude, and behaviors related to removing PICC. Results: Pediatric nurses achieved mean scores of 14.83±2.76, 126.33±16.97, and 129.14±10.76 for knowledge, attitude, and behavior related to PICC removal, respectively, with the corresponding score rates being 67.4%, 93.6%, and 86.1%, separately. The results of multiple linear regression analysis showed that the nature of hospital, whether receiving PICC extubation training within 3 years, and the attitude dimension scores were the influencing factors of PICC extubation behavior of those pediatric nurses. Conclusion: Although the pediatric nurses lack knowledge and skills in PICC extubation, they have relatively good behaviors with active attitudes. Therefore, nursing administrators should prioritize training and create a professional platform for comprehensive training for nurses to enhance safe care for children.

Objective: To explore the effects of application of improvement science in reducing the incidence of PICC Catheter Related Bloodstream Infection (CRBSI) based on evidence-based medicine. Methods: In this study, a convenience sampling method was adopted to select children with PICC in the wards of a tertiary grade A children's hospital and nurses in the ward from January 2020 to December 2022. The control group adopted conventional PICC infection prevention and control measures, while the experimental group adopted the six sigma in the improved science strategy. The incidence of CRBSI and the nurses' knowledge in the two groups was analysed. Results: The incidence of PICC CRBSI of the experimental group was significantly lower than that of the control group (0.9% VS. 2.4%). The incidence of infection per thousand PICC of the experimental group was significantly lower than that of the control group (0.41‰ VS. 1.0‰). The nurses' knowledge of infection prevention and infection control of PICC was higher than that of the control group , and the difference was statistically significant (P<0.05). Conclusion: Evidence-based nursing practice based on improvement science can effectively reduce the incidence of PICC CRBSI, improve nurses' knowledge of catheter infection prevention and control, and improve the quality of clinical nursing work.

Evidence-based practice of assessing the location of nasogastric tubes for children

Objective: To apply the evidence of assessing the location of nasogastric tube end for children to clinical practice and evaluate its application effects, so as to provide a reference for standardizing the assessment of nasogastric tube end position of pediatric nurses and improving the quality of specialized catheter nursing. Methods: According to the evidence transformation model issued by the evidence-based nursing center of Fudan University, we summarized the evidence regarding nasogastric tube end position assessment for children to formulate the corresponding clinical indexes for assessment. After analysis of obstacle factors in the process of implementation, we figured out proper strategies to implement the evidences. Then we compared the accuracy of nasogastric tube end position assessment, the incidence of nasogastric tube detachment, the implementation rate of each index and nurses' knowledge of nasogastric tube related operation and position assessment before and after the application of evidence. Results: Five indicators were established in this study. Nurses' compliance to apply the evidence was improved as well as their knowledge of nasogastric tube insertion with the differences being statistically significant (P<0.001). The accuracy rate of nasogastric tube end position assessment for children increased from 81.81% to 97.1% and the incidence of nasogastric tube detachment reduced from 24.2% to 2.9% with statistically significant differences (P<0.05). Conclusion: Applying the evidence of assessing the location of nasogastric tube end for children not only provides reference for clinical decision making, but also improves the evidence-based nursing knowledge and adherence to evidence for nurses to effectively increase the accuracy rate to locate nasogastric tubes for children, reduce the incidence of accidental nasogastric tube detachment, which can promote the quality of pediatric nursing.

Construction and application of the CVAD safety management information platform for children

Objective: To construct an evidence-based Central Venous Access Devices (CVAD) safety management information platform (hereinafter referred to as the platform) and evaluate its application effects to enhance the compliance of nurses in evidence-based practice, promote the clinical transformation of evidence, reduce catheter-related complications and improve catheter safety. Methods: Literatures on the placement, maintenance and removal of CVADs for children were searched to extract specific measures which were then assessed and adopted according to feasibility, appropriateness, meaningfulness and effectiveness (FAME). Later the way of embedding in the information system was formulated to form an evidence transformation information platform, which was officially and thoroughly applied in January 2020. Indicators such as CVAD-related complications and unplanned extubation in hospitalized children were compared before (2019) and after (2020-2023) the application of the platform. Results: After the implementation of the platform, the rate of CVAD-related complications and unplanned extubation in hospitalized children decreased significantly (P<0.001). Conclusion: The construction and application of the platform can ensure the information management of the whole process from catheter placement, maintenance to extubation, improve nurses’ compliance to evidences in practice to boost the homogenization of patient health education and reduce catheter-related complications. Meanwhile, the platform can provide timely and objective data and early warning, so that information communication among all departments in a hospital can be realized to improve catheter safety management for patients.

Objective: To develop a fertility guidance program for breast cancer patients of childbearing age who received chemotherapy for breast cancer. Methods: A research group was established, and the draft of the fertility guidance program for breast cancer patients of childbearing age was initially formed on the basis of evidence summary and findings of the qualitative interviews. Two rounds of expert consultation were conducted by Delphi method, and the final version of the program was determined. Results: The questionnaire recovery rate of the two rounds of expert consultation was 100%, and the expert authority coefficient was 0.86. In the first round of expert consultation, the Kendall harmony coefficient of the item's importance was 0.16 (P<0.001), and the Kendall harmony coefficient of feasibility was 0.15 (P<0.001). In the second round of expert consultation, the Kendall Harmony Coefficient of the item's importance is 0.25 (P<0.001), and the Kendall Harmony Coefficient of the item's feasibility is 0.23 (P<0.001). The final program included 3 intervention opportunities, 11 intervention themes and 29 intervention contents. Conclusion: The fertility guidance program for breast cancer patients of childbearing age undergoing chemotherapy is scientific, comprehensive and innovative, which can provide reference for clinical nurses to carry out fertility education.

Relationship between illness acceptance and self-management in Maintenance Hemodialysis patients: a latent profile analysis

Objective: To explore the latent categories of illness acceptance in Maintenance Hemodialysis patients, and to analyze the relationship between each category and the level of self-management, in order to provide patients with more effective personalized disease management and self-management support. Methods: A total of 290 patients undergoing maintenance hemodialysis who were treated in a tertiary grade A hospital in Shanxi Province from April to July 2023 were conveniently selected as the study subjects. Eight items of the Disease Acceptance Scale were used as explicit indicators for potential profile analysis, and the influencing factors of different categories of patients were identified by univariate analysis and multivariate logistic regression, and the regression mixed model was used to explore the differences in the self-management level of different categories of patients. Results: The illness acceptance of Maintenance Hemodialysis patients was divided into three potential categories, namely "low acceptance-social dependence" (41.0%), "medium acceptance-lack of self-confidence" (26.6%), and "high acceptance-action constraint" (32.4%). Per capita monthly household income, number of comorbidities and age on dialysis as factors influencing illness acceptance in different categories of maintenance hemodialysis patients (P<0.05). The total score of the patient self-management scale in the "high acceptance-action constraint group" was the highest. The scores of patients in the "medium acceptance-lack of self-confidence" group were lower than those in the "low acceptance-social dependence group" in the dimension of emotional processing. Conclusion: There is significant heterogeneity in illness acceptance among Maintenance Hemodialysis patients, and there are differences in the level of self-management among different types of patients. Caregivers should provide individualized interventions to improve self-management according to the category characteristics of patients' illness acceptance.

Social isolation and its influencing factors of elderly with hearing impairment in community

Objective: To explore the social isolation of elderly with hearing impairment in community and analyze its influencing factors, and to provide reference for community nurses to develop targeted interventions. Methods: Convenience sampling was used to select 300 elderly with hearing impairment who lived in a community in Beijing from May to August 2023. The survey was conducted using the general information questionnaire, Lubben Social Network Scale-6, Medical Outcomes Study Social Support Survey, Social Impact Scale, Impact on Participation and Autonomy Questionnaire, and Geriatric Depression Scale-15. Logistic regression was used to identify the influencing factors of social isolation. Results: The total score of social isolation among elderly with hearing impairment was 14.45±5.67, with a incidence of 35.67%. Logistic regression analysis showed that the age, education level, living conditions, hearing impairment, self-rated health, chronic disease status, and social supports were the influencing factors of social isolation (P<0.05). Conclusion: The social isolation situation of elderly with hearing impairment in community is not optimistic and needs more attention. Emphasis should be placed on increasing social support for the elderly, helping them improve their physical health, and developing targeted nursing plans to improve social networks and avoid social isolation.

Objective: To sinicize The Nursing Presence Scale for Hospitalized Patients and test its reliability and validity, so as to provide a reliable and effective evaluation tool for clinical investigation of patients' nursing experience. Methods: After obtaining authorization from the original author of the scale, we translated the English version of The Nursing Presence Scale for Hospitalized Patients and conducted back-translated, and carried out cultural adaptation. Formal questionnaire survey were conducted among the 530 inpatients of a tertiary grade A hospital in Wuhan by convenience sampling. Results: The Chinese version scale contains 33 items and was divided into 4 dimensions. The Cronbach's α for the total scale was 0.931, and for the dimensions ranged from 0.738 to 0.852. The content validity index of the scale was 0.98. Confirmatory Factor Analysis (CFA) showed that χ2/df was 1.897, GFI was 0.812, CFI was 0.869, IFI was 0.871, TLI was 0.859, RMR was 0.024, RMSEA was 0.062, which indicated that the four-factor model provided the best fit. Conclusion: The Chinese version of The Nursing Presence Scale for Hospitalized Patients has good reliability and validity, and is suitable for evaluating Chinese inpatients' feelings and experience of nursing presence.

Objective: To construct an early rehabilitation training model for patients with cognitive impairment after craniocerebral injury and explore its application effects. Methods: The early rehabilitation training model was constructed based on literature analysis and expert correspondence. We selected 194 patients with cognitive impairment after craniocerebral injury admitted to the Department of Neurosurgery of a tertiary grade A hospital in Qinghai from March 2021 to September 2023, and divided them into the control group (n=97) and the observation group (n=97) using the randomized numerical table. The control group was given routine neurosurgical intervention training while the observation group received early intervention based on the rehabilitation training model in addition to the training of the control group. The Loewenstein Occupational Therapy Cognitive Assessment (LOTCA), Mini-Mental State Examination (MMSE), National Institute of Health stroke scale (NIHSS), and the activities of daily living (ADL) scale were used to compare the cognitive function, neurological function, and daily living ability of the two groups. Results: After 2 months of intervention, the LOTCA score, MMSE score and ADL score of patients in the observation group were significantly higher than those of the control group, and the NIHSS score was significantly lower than that of the control group (P<0.001). Conclusion: The application of early rehabilitation model for patients with cognitive impairment after craniocerebral injury can effectively improve the cognitive function and neurological function of the patients, promote the rehabilitation and improve their quality of life.

Construction of risk assessment based graded bereavement support strategies checklist for primary family caregivers

Objective: To construct a risk assessment based checklist of graded bereavement support intervention strategies for primary family caregivers, guiding nurses to provide bereavement care for primary family caregivers. Methods: Based on a thorough literature review, a list of graded bereavement support intervention strategies for primary family caregivers was constructed. The Delphi method was used to consult 17 experts, and the content of the list was modified, added, and deleted according to their scores on the importance and feasibility of items and their opinions. Results: Two rounds of expert consultations were conducted, and the final "Risk Assessment Based Graded Bereavement Support Strategies Checklist" was established, including 2 first-level indicators, 7 secondary-level indicators, and 22 tertiary-level indicators. 18 indicators were applied to all bereaved caregivers, and the other 4 indicators were only applied to caregivers on high bereavement risk. The response rates of 2 rounds were both 100%; the familiarity coefficient, judgment basis coefficient, and authority coefficient of expert consultation were 0.88, 0.93, and 0.91 respectively. Conclusion: The risk assessment based graded bereavement support strategies checklist has good scientific, feasible, applicable, practical, systematic, and targeted characteristics, and can provide standardized and convenient practical guidance for clinical nurses.

Analysis of the current status and clinical outcomes of enteral nutrition support in Pediatric Intensive Care Unit patients

Objective: To investigate the current status and clinical outcomes of enteral nutrition support in Pediatric Intensive Care Unit (PICU) patients, in order to provide a basis for improving the quality of critical care. Methods: A total of 330 children admitted to the intensive care unit of a tertiary grade A children's hospital in Shanghai from July 2022 to January 2023 were recruited by continuous sampling method. The study investigated the intake of early enteral nutrition, energy, protein and the clinical outcome of the children. Results: Among the 330 cases, 275 cases (83.3%) had early enteral nutrition and 55 cases (16.7%) had delayed nutrition. There were 125 cases (37.9%) with unreasonable energy intake. Protein intake was insufficient in 155 cases (47.0%). There were 6 cases (1.8%) of retropyloric feeding, 210 cases (63.6%) of enteral nutrition interruption, and 77 cases (23.3%) of enteral nutrition complications. There were significant differences in weight gain, mechanical ventilation time, multiple organ dysfunction, PICU hospitalization time and hospitalization cost between early enteral nutrition group and delayed enteral nutrition group (P<0.05). There were significant differences in weight gain, multiple organ dysfunction and hospitalization time in PICU between rational energy intake group and irrational energy intake group (P<0.05). There were significant differences in mechanical ventilation time and hospitalization time of PICU between reasonable protein intake group and insufficient protein intake group (P<0.05). Conclusion: Patients in PICU have low retropyloric feeding rate, high incidence of nutritional disruption and nutritional complications, and high unreasonable rate of energy and protein intake. The early enteral nutrition, reasonable energy and protein intake of patients have better clinical outcomes such as hospitalization time in PICU. It is necessary to standardize the management of enteral nutrition support to improve the quality of care for PICU patients.

Objective: To describe the experience of social alienation in young and middle-aged patients with Maintenance Hemodialysis (MH), and to provide reference for further nursing support. Methods: A descriptive qualitative study was conducted, and 14 patients with uremia undergoing MH treatment in a grade A hospital of traditional Chinese medicine in Hangzhou were selected from June to October 2023 for semi-structured in-depth interviews by purposive sampling. The themes were summarized and refined by content analysis. Results: The experience of social alienation in MH patients was summarized into four themes: Social avoidance, Self-isolation, Restriction of freedom, Discovering the power of the support system. Conclusion: Young and middle-aged patients with MH treatments experience social alienation due to impaired physical health, poor psychological status, limited social support and other reasons. It is necessary to strengthen the symptom management and psychological counseling, and strengthen multidimensional support to help this population return to and adapt to society.

Development of a perioperative nursing quality evaluation standard system base on Enhanced Recovery After Surgery concept

Objective: To develop a scientific, reliable and operable perioperative nursing quality evaluation standard system under the Enhanced Recovery After Surgery concept. Methods: The theoretical framework of this study is the Structure-Process-Result three-dimension quality evaluation model. The system was developed based on literature review and group discussion. Two rounds of expert consultation were conducted by Delphi correspondence method. The reliability of the correspondence was verified by calculating the expert positive coefficient, coefficient of variation, expert authority coefficient, and Kendall harmony coefficient. Results: The recovery rates of the effective questionnaires were 100.0%. The authority coefficients of the experts were 0.900 and 0.892, respectively. Kendall concordant coefficients were 0.119 and 0.173, respectively (P<0.001). After two rounds of correspondence, the perioperative nursing quality evaluation standard system was finally determined. The system included 3 primary items, 17 secondary items and 48 tertiary items. Conclusion: The perioperative nursing quality evaluation standard system is scientific, reliable and widely applicable, which can be used as an objective tool for perioperative nursing quality assessment.

Design and application of the integrated information-based triage and registration system in the emergency department based on Business Process Reengineering Theory

Objective: To design an integrated information-based triage and registration system in the emergency department based on Business Process Reengineering (BPR) theory and explore its effects. Methods: Constructing an integrated information-based triage and registration system in the emergency department through literature review and group meeting discussion. At the same time, opened the electronic diagnosis and treatment card application authority, redesigned the triage registration list, and improved the function of the triage information system. Convenience sampling method was used to select patients who visited the emergency department of a tertiary grade A hospital in Changsha from January to December 2022 as the research subjects. Six hundred patients visiting the emergency department from January to June 2022 were included in the control group, and another 600 patients from July to December 2022 were included in the experimental group. The experimental group adopted the integrated information-based triage and registration process, while the control group adopted the conventional one. The time spent in each link of triage and registration (triage time, registration time, information entry time, total time) and patient and staff satisfaction were compared between the two groups, and semi-structured interviews were used to collect staff opinions on the integrated information-based triage and registration process. Results: The time spent on triage, registration, information entry, and the total time for triage and registration in the experimental group were shorter than those in the control group (P<0.001). Both the patient satisfaction and staff satisfaction in the experimental group was significantly higher than those in the control group (P<0.05). Conclusion: The application of the integrated information-based triage and registration system reduces the time consumed in each link of patient triage and registration, improves the overall patient satisfaction and their treatment experience as well as staff efficiency and satisfaction.

Methods: Using a historical controlled study method, 22689 patients for emergency triage in April 2021 from a tertiary grade A hospital in Hangzhou were conveniently selected as routine group and received routine emergency triage management, while 22236 patients for emergency triage in April 2022 were as the observation group and received the whole-process management in triage based on the feedback of big data, including the dynamic assessment and the continuous improvement of pre-check, middle, and back-end quality. The quality and efficiency of emergency triage before and after the implementation of the whole-process management were compared. Results: After the implementation of the whole-process management based on big data feedback, the completion rate of triage information, the compliance rate of triage and the target response time were significantly better than those with routine management (P<0.05), while the waiting time for patient and the incidence of adverse events were significantly lower (P<0.05). Conclusion: The whole-process management based on the feedback of big data visualization technique can effectively improve the quality and efficiency of emergency pre-check and triage, and guarantee the safety of emergency patients.

Objective: To establish a scientific and operable information system of the cardiovascular disease nursing quality control. Methods: Based on RAND-UCLA expert consensus method, the information system of the cardiovascular disease nursing quality control was established. According to the development process of NDNQI sensitivity index, some medical institutions were selected for pilot data collection. Indicators with better sensitivity and data availability were screened and determined from the index system and an information system was built based on these indicators. Medical institutions were recruited to use this information system through the Nursing Committee of the National Committee of Cardiovascular Experts. Results: The information system of the cardiovascular disease nursing quality control initially developed contained 18 indicators. While 7 indicators were finally left after the pilot data collection. A total of 145 cardiovascular disease-related wards from 28 medical institutions across the country participated in the data reporting by this information system. Conclusion: The information system of the cardiovascular disease nursing quality control established in this study is scientific and operable. This information system provides the possibility to carry out the national cardiovascular disease nursing quality monitoring. Through the integration, analysis and feedback, and resource sharing of data, the information system can provides important support for the quality control and improvement of cardiovascular disease nursing.

Objective: To systematically integrate ICU nurses' experiences and needs in implementing early mobilization, providing reference for the development of early mobilization plans for ICU patients. Methods: Qualitative studies on ICU nurses' implementation of early mobilization in domestic and international databases were searched, with a search time frame from the construction of the database to October 2023. The integration was performed using the Meta integration method. Results: A total of 13 articles were included in this study, which were analyzed and categorized into nine groups, ultimately resulting in three integration: problems and challenges faced by nurses implementing early mobilization; positive factors for nurses implementing early mobilization; needs of nurses implementing early mobilization. Conclusion: Nursing administrators should pay attention to the barriers encountered by ICU nurses in implementing early mobilization, while focusing on reducing nurses' physical and mental stress. It is essential to introduce scientific early mobilization processes, conduct relevant training, and establish a multidisciplinary team, as well as provide and develop personalized rehabilitation equipment to lay a solid foundation for the widespread implementation of early mobilization practices.

Exercise intervention on the quality of life in patients with Systemic Lupus Erythematosus: a Meta-analysis

Objective: To evaluate the effect of different exercise programs on the quality of life of patients with Systemic Lupus Erythematosus (SLE). Methods: According to the PICOS principle, an experimental study on the impact of exercise intervention on the quality of life of SLE patients published before November 2023 was retrieved from Chinese and English databases. The study was evaluated by the literature quality evaluation tool published by the Australian JBI Evidence Based Health Center. RevMan 5.4 was applied for data analysis. Results: Nine studies with 529 subjects were included exercise program was classified into aerobic exercise and resistance exercise. Subgroup analysis showed that patients exercised ≥3 times per week had significant improvements in quality of life including physical health [SMD=0.69, 95% CI (0.48, 0.90), P<0.001] and mental health [SMD=0.90, 95% CI (0.45, 1.35), P<0.001]. Patients exercised 30-50 minutes each time showed significant improvements in Quality of Life including physical health [SMD=0.72, 95% CI (0.41, 1.04), P<0.001] and mental health [SMD=0.81, 95% CI (0.36, 1.26), P<0.001]. Conclusion: SLE Patients who exercise ≥3 times a week for about 30-50 minutes each time have a significant improvement in their quality of life. Further large-scale high-quality RCT studies are needed to elucidate the precise exercise plan for patients with SLE.

Objective: To systematically search, evaluate, and integrate the relevant evidence on Hospital-Acquired Functional Decline (HAFD) in the elderly to provide evidence-based support for healthcare professionals in preventing and managing HAFD in older adults. Methods: Following the "6S" model, we searched computer decision systems, guidelines, professional society websites, and databases for relevant evidence on age-related decline in geriatric healthcare, including guidelines, clinical decisions, expert consensus, evidence summaries, best practices, and systematic reviews from the database inception to March 10, 2024. Two researchers independently performed literature screening and quality assessment, extracted evidence based on themes and professional judgment. Results: A total of 15 articles were included, comprising 1 clinical decisions, 10 systematic reviews and 4 randomized controlled trial studies. From the aspects of assessment, risk identification, exercise, nutrition and specialized wards for the elderly, 18 pieces of best evidence were summarized. Conclusion: This study provides an evidence-based summary of the best evidence for prevention and management of HAFD in the elderly, offering evidence-based guidance for clinical practice. Healthcare professionals can selectively choose the best evidence based on clinical scenarios, facilitators/barriers identified in the evidence, and patient preferences to minimize hospital acquired functional decline in the elderly.

Objective: To systematically review and integrate qualitative studies on the influencing factors of Diabetic Retinopathy (DR) screening in diabetic patients. Methods: PubMed, Web of Science, Embase, Cochrane Library, China National Knowledge Infrastructure, WanFang Database, VIP Database, and China Biology Medicine Disc were searched to collect qualitative studies related to DR screening in diabetic patients. The search time limit was from the establishment of the database to March 31, 2024. The quality evaluation was carried out using the quality evaluation standard for qualitative research of the Australian JBI Evidence-based Health Care Center, and the Meta-integration was carried out using the aggregative synthesis. Results: A total of 18 articles were included, and 24 main findings were extracted, which were summarized into 8 categories and summarized into 3 integrated results: individual factors; family and social factors and medical context. Conclusion: There are many barriers and facilitators for DR screening in diabetic patients. It is necessary to strengthen the education of DR Screening for diabetic patients. At the same time, we should pay attention to the role of social and family in the management of DR in diabetic patients, improve the awareness of DR prevention in diabetic population, and promote the health of vision in diabetic patients.

Risk prediction models for central venous Catheter-Related Bloodstream Infections: a systematic review

Objective: To systematically search and evaluate the risk prediction models for central venous Catheter-Related Bloodstream Infections, to provide reference for constructing a high-quality risk prediction model for central venous Catheter-Related Bloodstream Infections. Methods: We searched for relevant literature on central venous Catheter-Related Bloodstream Infections from Chinese and English databases from their inception to December 11, 2023. Two researchers independently conducted literature screening and data extraction, using PROBAST tool to evaluate the risk of bias and applicability of included studies. Results: A total of 16 articles were included, involving 15 risk prediction models for central venous Catheter-Related Bloodstream Infections. The AUC or C index of 15 models ranges from 0.67 to 0.930; 5 models received internal validation and 8 models external validation. Quality evaluation of the 16 articles showed high bias risk and low applicability risk. The predictive factors with higher frequency of occurrence included: parenteral nutrition (TPN), diabetes, duration of central venous catheterization, ICU patient or hospital history, number of lumens, use of antibiotics, femoral vein insertion, and malignant tumors. Conclusion: The existing risk prediction models for central venous Catheter-Related Bloodstream Infections have good predictive performance to determine the predictive factors which possess certain guiding significance for clinical practice, but the risk of bias is high. Further validation of future response models is needed to provide a basis for developing effective prevention strategies.

Objective: To apply the best evidence on the prevention and management of fall in hospitalized children to clinical practice and evaluate its effect, so as to provide reference for improving the quality of care for hospitalized children. Methods: From June 2021 and November 2021, a total of 189 children and 23 nurses in the respiratory department of a tertiary-level hospital in Guangzhou were selected with convenience sampling method. The knowledge scores of nurses and caregivers, the implementation rate of the criteria, and fall rate were compared among the baseline review, the first review, and the second review. Results: For the data of nurses, the knowledge scores increased (P<0.05), and the implementation rates of each indicator in the two rounds of review were higher than those in the baseline, and the differences in some indicators were statistically significant (P<0.05). For the data of children, the incidence of fall was 6.5%, 1.7%, and 0 respectively in the three rounds of review, but the difference was not statistically significant (P>0.05). For the data of caregivers, the knowledge scores improved compared with that in the baseline (P<0.05). Conclusion: By developing the practice program for the prevention and management of falls in hospitalized children and applying it to clinical practice, the incidence of falls in hospitalized children can be reduced, the awareness level of nurses and caregivers can be improved, and the compliance of nurses to the implementation of the best evidence can be strengthened, and promoting the improvement of nursing quality.

Objective: To construct the Pressure Injury (PI) nursing decision support system and explore its effectiveness in PI management. Methods: Establish a research and development team, construct a PI assessment content and nursing measures knowledge base based on the latest guideline evidence and practice recommendations, and to design Human-computer interaction rules. Using computer intelligence achieve comprehensive evaluation and recording of PI. Compare the accuracy of PI staging and tissue type identification, the effectiveness of PI treatment, and the implementation rate of PI nursing record before and after the application of the decision support system. Results: After applying the decision support system, the correct rate of PI staging judgment increased from 84.5% to 96.0%, and the correct rate of tissue type judgment increased from 79.5% to 93.5%, the effective rate of PI treatment increased from 68.2% to 82.4%, the implementation rate of PI nursing records measured at 4 time points has increased from 75.4%, 81.1%, 88.3%, and 77.7% to 92.7%, 96.0%, 94.8%, and 94.3%, respectively, the differences were statistically significant (P<0.05). Conclusion: The use of the PI nursing decision support system improved the effectiveness of PI treatment and the standardization of nursing records, promoting an increase in the quality of PI nursing care.

Objective: To develop a WeChat mini program for home rehabilitation management of patients with gynecological malignancies and evaluate its usability, and to provide a feasible tool for home management in this population.Methods: A semi-structured interview was conducted with patients and their caregivers to analyze the needs of home management. Multidisciplinary team meetings were held to determine the structure, functionality, and main contents of the mini program. An iterative approach was used for the development of the mini program. From April to May 2023, patients with gynecological malignancies were selected from the gynecological ward of a tertiary grade A hospital in Beijing using convenient sampling method. A questionnaire adapted from the Technology Acceptance Model was utilized to evaluate its acceptance. Results: The "Zhi Kang Fu" mini program was developed, comprising six modules: expert introduction, health education, postoperative behavior suggestions, complication warning, chemotherapy management, and psychological health. The total acceptance score of the mini program among 54 patients was 156.15±30.93, indicating a high level of acceptance, and most participants reported its convenient and user-friendly of the mini program, and it,s beneficial for disease management. Conclusion: The content and functions of the "Zhi Kang Fu" mini program are comprehensive and targeted, and it can meet the needs of patients for home rehabilitation management.

Application of mobile health care in pain management of children with malignant tumors: a scoping review

Objective: To review the scope of research on the application of mobile health in pain management in children with malignant tumors, in order to identify its intervention elements and application status. Methods: The JBI Scope Review Guidelines was used as the methodological framework to retrieve relevant studies in PubMed, Web of Science, Cochrane Library, Embase, CINAHL, CNKI, Wanfang database, VIP database and CBM, etc. The search period was from the inception of the database to May 16, 2023, and the included literature was summarized and analyzed. Results: A total of 13 literatures were included. The application types of mobile medicine in pain management of children with malignant tumors mainly referred to application program, network platform and Wechat mini programs. The content elements of mobile health intervention included pain assessment, monitoring and online pain intervention, and the outcome indicators included availability, feasibility, acceptability and effectiveness. Conclusion: Mobile health care that fits children's psychological and behavioral characteristics is favored by them, and its usability, feasibility and acceptability in pain management have been proven to exert positive effects. In the future, mobile medical applications suitable for children with tumor pain in China should be developed, and high-quality studies be conducted to explore the effectiveness of mobile medical pain management, so as to promote the establishment of a pain follow-up database for children and improve their pain experience.

It also summarizes the advantages of photo-elicitation in community care and analyzes the challenges it faces in terms of resources, ethics, time, and standardization of reporting.
