Preventive effects of Low Anterior Resection Syndrome in rectal cancer patients based on the theory of Timing It Right

Objective: To explore the effects of prophylactic intervention based on the theory of "Timing It Right" in patients with rectal cancer on reducing the incidence and severity of postoperative Low Anterior Resection Syndrome (LARS). Methods: Totally 101 patients with low rectal cancer who were treated in the gastrointestinal surgery department of a tertiary grade A hospital from February 2020 to July 2021 were selected by convenience sampling method, and they were divided into 50 cases in the control group and 51 cases in the intervention group according to the order of admission time. The control group received routine care, and the intervention group received prophylactic intervention based on the "Timing It Right" on the basis of the intervention in the control group. The incidence, severity of LARS and quality of life were compared between the two groups at 1 month and 3 months after stoma reduction. Results: The LARS score and severity at 3 months after stoma reduction in the intervention group were significantly lower than that in the control group (P<0.05). The incidence (64.70%) was also significantly lower than that in the control group (88.00%) (P<0.05); and the quality of life in the intervention group was significantly higher than that in the control group at 1 month and 3 months after surgery (P<0.05). Conclusion: Prophylactic intervention based on the theory of "Timing It Right" can effectively reduce the incidence and severity of LARS in patients with low rectal cancer at 3 months after surgery, and improve patients' short-term quality of life after surgery.

Objective: To evaluate the application effects of maxillofacial oral massage based on whole-process management model in nasopharyngeal carcinoma radiotherapy patients, in order to improve the quality of care for patients with radiotherapy related complications. Methods: Totally 221 patients with radiotherapy for nasopharyngeal carcinoma (111 cases in the experimental group , 110 cases in the control group) were selected for clinical application in the oncology department of a tertiary grade A military hospital in Chongqing from November 2020 to November 2021. The control group received routine radiotherapy care. Based on the combination of multiple disciplines and departments, the whole periradiotherapy management model of nasopharyngeal carcinoma based on maxillofacial oral massage technology was implemented in the experimental group with the help of a sand table model and led by specialist nurses. The self-care ability, incidence of severe radiation oral mucositis, incidence of severe radiation dermatitis and incidence of unplanned admission were compared between the two groups. Results: After 4 weeks radiotherapy, the self-care ability, the incidence of severe radiation oral mucositis, the incidence of severe radiation dermatitis, and the incidence of unplanned admission 0-14 days after radiotherapy statistically significant differences between the two groups (P<0.05). Conclusion: The whole-process management model based on maxillofacial oral massage technology led by specialist nurses can improve the self-care ability of nasopharyngeal carcinoma patients during peri-radiotherapy, reduce the complications related to radiotherapy, and improve the quality of patient care.

Effects of care bundles on reducing the enteral feed intolerance in patients with hemorrhagic stroke

Objective: To analyze the implementation effects of evidence-based care bundles in reducing the incidence of feeding intolerance in patients with hemorrhagic stroke, which will provide ideas for the meticulous management of enteral nutrition patients with hemorrhagic stroke. Methods: Four evidence-based care bundle strategies were formed, including establishing a nurse-led multidisciplinary enteral nutrition management team, controlling feeding pathways and speeds based on gastrointestinal dynamics, providing individual enteral nutrition preparation, and using abdominal massage and hot compress to promote gastrointestinal motility. Patients with hemorrhagic stroke admitted to a tertiary hospital from December 2020 to June 2021 were selected as experimental group (n=69) and compared them with patients (n=70) admitted from April to October 2020, to analyze the changes in feeding intolerance and nutritional status. Results: The incidence of feeding intolerance in the intervention group was significantly lower than that in the control group, and the nutritional status were significantly better than that in the control group (all P<0.05). Conclusion: Care bundles should be actively promoted to reduce enteral feed intolerance for patients with hemorrhagic stroke, improving their nutrition status.

Continuous follow-up and effect evaluation on prevention of adult oral tracheal intubation unplanned extubation

Objective: To evaluate continuous effects of cluster nursing strategy for preventing tracheal intubation unplanned extubation in adults outlined in the 2018 edition provides reference for clinical nursing decision-making for ICU nurses. Methods: From June to December 2018, the strategy was implemented in 11 selected hospitals in Tianjin and proved to be effective. Following its success, the strategy was then widely promoted throughout the city. Continuous tracking and evaluation of its effects were conducted for four consecutive years. Results: After the implementation of the project, from 2019 to 2022, the median rate of unplanned extubation remained at 0. However, the compliance rate of overall strategy execution and individual item execution showed fluctuations. In 2020 and 2021, two items were below the target value, and in 2022, two items were also below the target value. Other items achieved the target value. Conclusion: The content of the 2018 edition of cluster nursing strategy for preventing tracheal intubation unplanned extubation in adults is scientific, feasible and effective, and its popularization and application can improve the quality of nursing care, reduce the rate of unplanned extubation of tracheal intubation, and ensure the safety of patients.

Objective: To evaluate the effects of integrated medical and nursing management for postoperative radiotherapy patients with head and neck tumors in the head and neck tumor rehabilitation nursing clinic. Methods: From March 2022 to March 2023, a total of 92 patients with head and neck tumors who underwent postoperative radiotherapy were selected at a tertiary grade A hospital in Beijing, and randomly divided into an experimental group (n=47) and a control group (n=45). The rehabilitation management plan for the experimental group patients were jointly developed by the supervising doctor and specialized nurses in the head and neck tumor care outpatient department. The control group patients received regular health education and follow-up through regular brochure and mouth opening training videos. The disease knowledge level, mouth opening training compliance, VAS mouth opening pain score, and mouth opening restriction score of the two groups were compared before radiotherapy, and 2 weeks, 1 month, and 3 months after radiotherapy respectively. Results: After intervention, the disease knowledge level, compliance with mouth opening training, VAS mouth opening pain score, and incidence of mouth opening restriction between the two groups of patients showed statistically significant changes (P<0.05). Conclusion: Implementing integrated medical and nursing management based on nursing clinics can improve the disease knowledge level and compliance with mouth opening training for postoperative radiotherapy patients with head and neck tumors, so as to improve the symptoms of mouth opening pain, and reduce the incidence of mouth opening difficulties.

Application of nursing intervention based on Health Promotion Model in the management of patients with hyperphosphatemia receiving Maintenance Hemodialysis

Objective: To evaluate the effect of nursing intervention based on Health Promotion Model (HPM) on the management of patients with hyperphosphatemia receiving Maintenance Hemodialysis (MHD). Methods: This study included 80 patients with hyperphosphatemia who underwent MHD treatment in the hemodialysis center of a tertiary grade A hospital from July to November 2019. Patients receiving dialysis every Monday, Wednesday, and Friday were included in the control group (n=40), and patients receiving dialysis every Tuesday, Thursday, and Saturday were in the intervention group (n=40). The control group received routine nursing, while the intervention group received the nursing care based on Health Promotion Model (HPM). The knowledge, behavior, and self-efficacy scores of hyperphosphatemia management, serum phosphorus levels, serum phosphorus pass rate, and serum albumin levels of the two groups were compared before intervention, 1 month and 4 months after intervention, respectively. Results: After 1 months of intervention, the knowledge and behavior scores in the intervention group were obviously higher than those in the control group, while the serum phosphate level was lower than that in the control group (P<0.05). After 4 months of intervention, the knowledge and behavior scores, self-efficacy scores, serum phosphorus pass rate and albumin levels were higher than those of the control group, yet the serum phosphorus level in the intervention group was lower than that in the control group, and the differences were statistically significant (P<0.05). Conclusion: The nursing intervention based on HPM can effectively improve patients' knowledge, behavior and self-efficacy level of hyperphosphatemia management, reduce the serum phosphorus level and improve their nutritional status.

Objective: To develop Family Caregiver-Nurse Collaboration Scale for elderly patients and to test its reliability and validity, in order to provide a effective evaluation tool for evaluating the level of collaboration between nurses and family caregivers. Methods: Based on the perspective of family caregivers of elderly patients and the tentative collaboration model, the primary scale was formed by literature review, semi-structured interview, Delphi method and preliminary survey. From April to September 2022, 594 family caregivers of elderly patients were selected for questionnaire survey to test the reliability and validity of the scale. Results: The final scale included 4 dimensions: collaboration conditions, decision-making participation, role activities, and collaboration results, with a total of 39 items. The item-level content validity index was 0.830-1.000, and the scale-level content validity index was 0.967. A total of 4 factors were extracted by exploratory factor analysis, and the cumulative variance contribution rate was 79.424%. The results of confirmatory factor analysis met the adaptation criteria. Criterion validity was 0.727 (P<0.001). The Cronbach's α coefficient of the scale was 0.858, the retest reliability coefficient of the scale was 0.982, and the split-half reliability of the scale was 0.861. Conclusion: The Family Caregiver-Nurse Collaboration Scale for elderly patients has good reliability and validity, and can be used to evaluate the level of collaboration between nurses and family caregivers of elderly patients.

Objective: To construct a Primary Percutaneous Coronary Intervention process for patients with acute myocardial infarction based on Value Stream Map and to evaluate its application effect, in order to provide reference for improving the treatment efficiency of patients with acute myocardial infarction. Methods: According to the length of hospital stay, patients with acute ST-segment Elevation Myocardial Infarction who underwent Primary Percutaneous Coronary Intervention were divided into control group (n=32) and observation group (n=30). The control group was treated with routine treatment, and the observation group was treated with Primary Percutaneous Coronary Intervention process based on Value Stream Map. The process time, important treatment node time and treatment effect of the two groups were compared. Results: After implementation, the non-value-added time and total process time of the observation group were significantly shortened (P<0.05). The time from admission to balloon dilatation, the acquisition time to obtain the first ECG, the time of troponin reporting and the start-up time of the catheterization room of the observation group were significantly shortened (P<0.05). The cure and discharge rate was higher than that of the control group (P<0.05). Conclusion: The implementation of Primary Percutaneous Coronary Intervention process for patients with acute ST-segment Elevation Myocardial Infarction based on Value Stream Map can promote the efficiency of treatment.

Reliability and validity of the Chinese version of Perception of Protective Isolation Scale among patients with Hematopoietic Stem Cell Transplantation

Objective: To translate Perception of Protective Isolation (ISOLA) scale into Chinese and evaluate its reliability and validity among hematopoietic stem cell transplantation patients, and to provide a tool for assessing protective isolation in this population. Methods: The translation and cross-cultural adaptation of ISOLA scales were completed according to Brislin criteria. A convenient sample of 151 patients with hematopoietic stem cell transplantation in protective isolation was recruited from 6 haematology centers in Beijing and Hebei provinces between May to July, 2021. The psychometric properties of the scale were evaluated by item analysis, validity test and reliability analysis. Results: The Chinese version of ISOLA included 3 dimensions and 13 items. The S-CVI was 0.949 and the range of I-CVI was between 0.860 to 1.000. Cronbach's α of the total scale was 0.825, and Cronbach's α of the 3 dimensions were between 0.728 to 0.838, the split-half coefficient for the total scale was 0.827. Conclusion: The Chinese version of ISOLA scale has good reliability and validity, which can be used as an effective tool for assessing protective isolation of loneliness among hematopoietic stem cell transplantation patients.

Construction and validity evaluation of a risk assessment scale for aspiration in hospitalized adults

Objective: To develop and evaluate the reliability and validity of a risk assessment scale for aspiration in adult inpatients, with the aim of providing an effective tool for identifying at-risk patients. Methods: A draft of the aspiration risk assessment scale was established based on literature analysis, qualitative interviews and expert correspondence results. The scale was then validated through retrospective analysis of 320 inpatients with aspiration risk in a tertiary hospital in Jinhua. Results: The scale consists of 16 items referring to 3 dimensions: basic information, disease evaluation, and treatment measures. 6 common factors were extracted through exploratory factor analysis, with a cumulative variance contribution rate of 69.909%. The area under the receiver operating characteristic curve was 0.899 [95% CI (0.863-0.935), P<0.001]. The item-level content validity index ranged from 0.886 to 1.000, and the scale-level content validity index was 0.942. The Cronbach's α coefficient was 0.711, and the Kendall's W coefficient measuring inter-rater reliability was 0.882. Conclusion: The inpatient aspiration risk assessment scale has shown good reliability and validity to screen and identify patients at risk of aspiration, providing a scientific and feasible assessment tool for clinical nursing management of aspiration.

Systematic evaluation of patient safety-oriented transitional care assessment tools based on COSMIN

Objective: The psychometric properties and methodological quality of patient safety-oriented tools measuring transitional care were systematically evaluated to provide reference for medical health care professionals to select the best assessment tools. Methods: Databases were searched to collect studies on the instruments of measuring transitional care from their inception to October 27, 2022. COSMIN was used by 2 researchers independently to screen the literature and extract the data. In this paper, the content validity, structure validity, internal consistency, stability and hypothesis testing were analyzed in detail. Results: A total of 27 studies were included involving 9 measurement tools for transitional care. In terms of content validity, 1 tool was evaluated as “sufficient”, 8 tools were evaluated as “uncertain” or “not mentioned”. As to structure validity, 1 tool was evaluated as “sufficient”, 8 tools were considered as “uncertain”. In terms of internal consistency, 1 tool was evaluated as “insufficient”, 1 tool was evaluated as “uncertain”, 7 tools were evaluated as “sufficient”. When it comes to stability, 1 tool was classified as “insufficient”, 1 tool was classified as “uncertain”, 7 tools were classified as “not mentioned”. On the aspect of hypothesis testing, 4 tools were evaluated as “sufficient”, 5 tools were evaluated as “uncertain” or “not mentioned”. Conclusion: In view of evaluation results and grading of evidence, the recommended rating of the Care Transition Measure was grade A, the other 8 tools was grade B. No tool was grade C. Therefore, it can be temporarily recommended for use. It has good reliability and validity, but whether it can be widely used in China still needs further research.

Evidence summary for the maintenance of the dressing of Central Venous Catheter in children

Objective: To evaluate and summarize the relevant evidence related to the maintenance of the dressing of Central Venous Catheter in children and provide reference for clinical practice. Methods: We systematically searched domestic and foreign guide websites, professional associations, and databases for evidence related to the maintenance of the dressing of Central Venous Catheter in children. The quality evaluation and data extraction of the included literature were carried out independently by four researchers, and the evidence was summarized according to the judgment of professionals. Results: A total of 7 literatures were included, including 2 guidelines, 2 evidence summaries, 1 recommended practice, 1 systematic review, and 1 expert consensus. Totally 18 pieces of evidence were collected from three aspects: selection, fixation and replacement of the dressing of Central Venous Catheter in children. Conclusion: The evidence summary for the maintenance of the dressing of Central Venous Catheter in children can provide evidence-based basis for improving the standardization of the dressing of Central Venous Catheter application, so as to reduce the occurrence of complications of Central Venous Catheter in children.

Life perception and experience of patients after Ventricular Assist Device implantation: a Meta-synthesis

Objective: To systematically review and synthesize qualitative studies on the life perception experience of patients after Ventricular Assist Device (VAD) implantation, and to provide a basis for the development of targeted measures which improve patients' quality of life. Methods: We searched for qualitative research on life perception and experience of patients after VAD implantation from Chinese and English databases. The retrieval period is from the establishment of the database to February 2023. The Joanna Briggs Institute Critical Appraisal Tool for qualitative studies in Australia (2016 edition) was used to evaluate the quality of included studies. And the results were consolidated using aggregative integration approach. Results: A total of 15 articles are included, with 56 complete research findings grouped into 10 categories by their similarities. And 4 integrated findings were synthesized, namely multidimensional self change, adaptation to VAD, the needs and resources of VAD patients, device management and security of VAD. Conclusion: The patients after VAD implantation experience multidimensional self change. Medical staff should guide patients correctly and assist them in making positive adaptation, provide support by integrating resources to meet the needs of patients, and help them achieve self-management, so as to promote their early reintegration into society.

Objective: To systematically evaluate and integrate the causes of low adherence to low sodium diet in patients with Chronic Heart Failure, with the aim of providing evidence-based guidance for clinical healthcare professionals to develop low-sodium diet-related interventions. Methods: The relevant databases were retrieved, the qualitative studies meeting the inclusion criteria were included, and the findings were integrated. Results: Twelve articles were included, 44 research findings were extracted, which were further grouped into 7 new categories and 2 integrated themes. Theme 1 was personal factors and theme 2 was environmental factors. Conclusion: There were two reasons for low sodium dietary compliance in patients with chronic heart failure. Nursing professionals should pay attention to the causes of low adherence to low sodium diet in patients and provide individualized care.

Status quo and influencing factors of returning to work in colorectal cancer survivors

Objective: To investigate the status quo of returning to work and its related factors in colorectal cancer survivors, and to provide theoretical reference for promoting their return to work. Methods: A convenient sampling with 281 colorectal cancer survivors were included from the outpatients at a tertiary grade A tumor specialized hospital in Hangzhou from December 2021 to July 2022. The patient general information questionnaire, the Chinese Version of the MD Anderson Symptom Inventory-Gastrointestinal Cancer Module, Social Impact Scale and The Work-ability Support Scale were used to collect data. Results: Totally 180 survivors (64.06%) of colorectal cancer had returned to work, the results of binary logistic regression analysis showed that the score of Social Impact Scale (OR=0.89, 95% CI:0.81-0.99), work-ability (OR=1.14, 95% CI:1.02-1.28), willingness to work (OR=1.14, 95% CI:1.02-1.28), supports from families and friends (OR=31.13, 95%CI: 5.29-183.06), employer contacting (OR=11.94, 95% CI:2.28-62.38) were associated with the outcome of whether return-to-work. Conclusion: Medical staff should correctly assess the physical and mental condition of colorectal cancer survivors and their ability to return to work, and more supports from the families, workplaces and the society are expected to, help survivors build confidence and actively return to work.

Objective: To explore the application of stepwise comprehensive rehabilitation program in the patients with breast cancer-related lymphedema. Methods: The quasi-experimental research method was adopted. A total of 33 patients with breast cancer-associated lymphoedema who were treated in the lymphedema nursing clinic of a tertiary grade A hospital in Zhejiang province from January 2022 to June 2022 were set as the control group, and routine nursing management mode was adopted. A total of 36 patients from July 2022 to December 2022 were set as the experimental group, and the stepwise comprehensive rehabilitation program was adopted on the basis of the control group. The bioelectrical impedance, grip strength, upper limb dysfunction, family intimacy, family adaptability and so on of the two groups were measured before and after intervention. Results: After intervention, the experimental group was superior to the control group in Body Mass Index control, moisture content of affected limb, bioelectrical impedance, grip strength, upper limb dysfunction, family intimacy, family adaptability and so on (P<0.05). Conclusion: The stepwise comprehensive rehabilitation program is effective in promoting the rehabilitation of patients with breast cancer-related lymphedema. It is of great significance in relieving the symptoms of upper limb lymphedema, improving the limb dysfunction of upper limb lymphedema and promoting family intimacy, so it is worth popularizing it in clinical nursing work.

Evidence summary for the management of dysphagia in postoperative patients with oral cancer

Objective: To summarize the evidence for the management of dysphagia in postoperative patients with oral cancer, in order to provide reference for clinical practice. Methods: We searched and analyzed guidelines, expert consensus, systematic reviews and experimental studies from relevant guideline and professional association websites and databases. After literature quality evaluation, we extracted and integrated the evidence, and finally obtained the evidence applicable to the clinical situation in China after expert discussion. Results: A total of 26 articles were included, and 19 recommendations were determined in 4 aspects, including management team, dysphagia assessment, swallowing rehabilitation training and complication prevention. Conclusion: The evidence for the management of dysphagia in patients with oral cancer after surgery can provide a reference for evidence transformation and clinical application. The evidence-based nursing practice should be carried out considering clinical practice and patient characteristics in the future.

Objective: To construct the risk management indicators of outpatient blood collection in cancer hospitals based on Kaiser Model, in order to provide reference for improving the quality of outpatient blood collection in specialized hospitals for cancer patients. Methods: Based on the Kaiser Model, the initial survey tool was formed by consulting relevant literature papers and policies, combined with the semi-structured interview results, and then the Delphi expert correspondence method was used to build the risk management indicators for outpatient blood collection in cancer hospitals. Results: During the two rounds of correspondence, the effective recovery rates of the questionnaires were 83.33% (15/18) and 100% (15/15), respectively. The authority coefficients of the two rounds of expert consultation were 0.85 and 0.92, respectively, with the Kendall harmony coefficients of expert consultation ranging from 0.290 to 0.381. The final Kaiser Model-based risk management indicators of outpatient blood collection in cancer hospitals included 3 first-level indicators, 8 second-level indicators and 24 third-level indicators. Conclusion: The risk management indicators of outpatient blood collection in cancer hospital is scientific and reliable, which can provide reference for improving the quality of outpatient care.

Objective: To explore the effects of implementation of vertical management of nursing staff in hospital. Methods: The vertical management of nursing staff in hospital was implemented in the hospital, including job setting and staffing, human resources deployment, nursing staff training and allocation, performance evaluation management and other aspects. The practical results were summarized. Results: After implementing vertical management, the nurses in front line accounted for 98.23% of the total number of nurses in the hospital, the turnover rate of nurses decreased by nearly 50.00% compared with that before the reform, and the nurses' job satisfaction increased from 95.00% to 98.00%. The incidence of adverse nursing events in inpatients decreased from 0.11% to 0.08%, and the proportion of accidents decreased from 33.95% to 23.61%. Conclusion: Conducting nursing vertical management is beneficial to maximize the value of nursing human resources and establish a fair and effective nursing performance evaluation mechanism.

Objective: To analyse the current situation and the advancement of nursing handoff evaluation tools in China and abroad, and to provide references for the development and implementation of nursing handoff evaluation tools in future. Methods: According to the research method of scoping review, related databases were searched from inception to December 16, 2022, the literatures and their references on nursing handoff evaluation tools were retrieved. Results: A total of 13 literatures were collected, including 8 kinds of nursing handoff evaluation tools. The evaluation methods were nurse self-evaluation, third-party evaluation and patient evaluation. The evaluation contents included nurse handover, nurse satisfaction, nurse cognitive workload, nurse perception and adaptation readiness of electronic handover, patient satisfaction, and patient's feeling of participating in bedside handover. Conclusion: Nursing handoff evaluation tools combined with clinical needs have been developed in recent years. In order to improve the development of nursing handoff, nursing handoff evaluation tools should be developed or introduced in China.

Objective: To establish the regional medical information platform and explore its application in the follow-up of patients with coronary atherosclerotic heart disease. Methods: A total of 120 patients with coronary atherosclerotic heart disease admitted to one hospital from April 2020 to February 2022 were divided into the control group (from April 2020 to March 2021, n=60) and the observation group (from April 2021 to February 2022, n=60). The control group received routine follow-up, while the observation group was followed up using the regional medical information platform. Both groups were followed up for 3 months. The results of coronary self-management scale (CSMS) and the Quality of Life scale of the two groups were compared before discharge and after follow-up. Meanwhile, the incidence of adverse drug events and adverse cardiovascular diseases, the number of doctors' consultations, the number of nurses' visits and the number of patients' consultations within 3 months of follow-up were observed as well as patients' satisfaction. Results: After 3 months of follow-up, the self-management ability and quality of life of the observation group were improved compared with the control group, and the difference was statistically significant (P<0.05). Compared with the control group, the incidence of adverse drug events and adverse cardiovascular diseases in the observation group during follow-up was lower, the number of doctor consultations, nurse visits and patients' disease consultations was higher, and patients' satisfaction with follow-up work was higher, with statistical significance (P<0.05). Conclusion: The establishment of regional medical information platform and its application in the follow-up of patients with coronary atherosclerotic heart disease can improve patients' self-management ability and their quality of life, and reduce the incidence of adverse events and adverse cardiovascular disease during the follow-up period. Patients also show high satisfaction towards the follow-up work.

Objective: To establish the prediction model of the acceptance of smart care for the elderly in the community based on the Unified Theory of Acceptance and Use of Technology, and provide reference for predicting the acceptance of smart care for elderly people. Methods: From May to December 2022, a total of 1031 elderly from 4 communities in Beijing, the Community Health Service Center of Shandong provincial Third Hospital and the Community Health Service Center of Liaocheng were recruited. Data were collected using a general information questionnaire, a questionnaire on influencing factors of the acceptance of smart care for the elderly, and Technophobia Scale. The variables were screened by LASSO regression, and the sample was randomly divided into training group (n=723) and test group (n=308) according to the ratio of 7:3. A decision tree model was established in the training group, and the accuracy, sensitivity, specificity and Receiver Operating Characteristic (ROC) curve were used to test the model in the test group. Results: Performance expectancy, effort expectancy, social factors, occupation, health management needs, and experience in using smart elderly care products were the influencing factors of the acceptance of smart care for the elderly in the community. The depth of the decision tree containing 7 layers and a total of 10 leaf nodes. The accuracy, sensitivity, specificity and AUC of the decision tree were 80.2%, 80.2%, 77.7% and 0.812. Conclusion: The decision tree of the acceptance of smart care for the elderly in the community has good prediction efficiency, which can provide scientific guidance for evaluating and promoting the acceptance of smart care for the elderly.

Objective: To improve the collection, storage and delivery quality of venous blood specimens, shorten the time for delivering specimens, and reduce the incidence of unqualified specimens. Methods: The nursing department collaborated with medical affairs department, laboratory department, logistics management centre, clinical departments, and information department. The core focused on specimen collection and storage quality, barcode quality, collection and storage method quality, specimen delivery quality, data analysis, and continuous improvement. Based on the hospital's holographic view system of patients, this study established a workflow for venous blood specimen management before the test, consisting of specimen trajectory display, problem information warning, feedback for clinical processing, management information push, and continuous quality improvement. A quality management pathway for venous blood specimens before the test was constructed, which consisted of three modules. The effectiveness of the pathway was evaluated after a 3-month application period. Results: In comparison to the same period last year, after the implementation of the quality management pathway for venous blood specimens before the test, the rate of unqualified venous blood specimens decreased from 0.31‰ to 0.19‰. The rate of unqualified specimens due to improper collection and storage decreased from 0.17‰ to 0.07‰. The turnaround time was shortened, and the delay rate of specimen transportation decreased, and the differences were statistically significant (P<0.05). Conclusion: The quality management pathway for venous blood specimens before the test based on the holographic view system of patients achieved tracing and management of the entire specimen process, providing a convenient query channel for clinical medical staff, reducing communication time costs, effectively improving the quality of venous blood specimens before the test, and reducing the rate of unqualified specimens.

Objective: To explore the effects of the full course care with intelligent interactive system on Very Low Birth Weight Infant (VLBWI) for providing a reference for establishing the whole-process management system of these infants. Methods: Totally 91 VLBWI and their parents were enrolled at a tertiary hospital in Xiamen from Jan 1, 2021 to Dec 31, 2022. The preterm who admitted from Jan, 2022 to Dec, 2022 were selected as experimental group (n=49), and those admitted from Jan, 2021 to Dec, 2021 were selected as control group (n=42). The experimental group received the full course care with intelligent interactive system. The control group received inpatient care and follow up by primary nurse and clinic nurse, respectively. Parental satisfaction at discharge, infants' anthropometric assessment and follow-up compliance and infants' neurodevelopment were compared. Results: Parental satisfaction of experimental group was higher than those of control group (P<0.05). The height and weight of the preterm infants in the experimental group were better than those of control group at the corrected age of 1, 3 and 6 months (P<0.05). The follow-up compliance of experimental group were higher than those of control group at the corrected age of 3 and 6 months (P<0.05). In addition, the scores of fine motor, adaptability and social contact of infants in experimental group were higher than those of infants in control group at the corrected age of 6 months (P<0.05). Conclusion: With the help of intelligent interactive system, the full course care of VLBWI can realize integrated management of inpatient care and follow-up, which has significant clinical value for promoting the physical and intelligent development of infants, improving their parents' satisfaction and compliance.

Objective: To develop a risk score scale for Symptomatic Knee Osteoarthritis (SKOA) incident and its manual, and to provide a tool for SKOA risk assessment and related risk factors intervention in clinical and community. Methods: Based on the risk prediction model of SKOA incident of 10 variables constructed by the research group, and referring to the method used in the literature, the risk score scale was developed. And for the key points of using the scoring table, a manual was developed to standardize the use of tools. Results: The total score of the SKOA risk score scale ranged from 0 to 51, 20.5 was determined as the best cut-off point for identifying individuals at high risk developing SKOA. According to the formula: risk probability=individuals' risk value/51×100%, the probability of the individual suffering from SKOA in four years could be calculated. The manual explained the key points for standardizing usage of the scoring table from three aspects: guidelines, attachments for variable evaluation, and usage precautions. Conclusion: The SKOA risk score scale is an easy-to-use and easy-to-understand tool, can be used as a tool in practice to assess the risk of SKOA incident in four years, and to guide the development of SKOA risk intervention, evaluation and management.

Objective: To investigate the effects of alternating prone ventilation based on AIDET communication mode on 72 sober patients in ICU. Methods: A total of 72 sober patients with severe pneumonia who received prone ventilation in the Step Down wards of three hospitals in Zhejiang Province from December 29, 2022 to February 1, 2023 were selected as subjects by convenience sampling method. Alternate prone ventilation scheme based on AIDET communication mode was implemented and data were collected. Results: The alternation between semi-prone position (120 degrees) and semi-prone position (90 degrees) lasted the longest (2.20±0.74) h, and 88.9% of patients' blood oxygen saturation improved after 1 hour of ventilation. Patients felt more comfortable during the process. Conclusion: The alternating prone ventilation scheme based on AIDET communication mode for conscious patients standardizes the process of prone ventilation, improves the comfort and respects the wishes of patients, prolongs the duration of prone position and reduces the occurrence of complications to improve the clinical outcome of patients.
