The influence of magnetic nursing on strengthening professional development

The 21st century is full of competition, change and innovation characters, the professional development has to rely on group power to achieve continuous improvement. To achieve it, nurses need to cultivate personal and professional strength, attract talent; by showing the nature of magnetic nursing, talents are willing to work to the best of their ability to promote professional development; by building magnetic workplace environment to reassure patients and family on healthcare delivery completely. Therefore, the aim of the study was to promote magnetic nursing by drawing from experience of the nursing theorists,and then strengthen professional development.

Competencies are established with reference to job portfolio which is in turn determined by the services in local context. To keep aligned with future direction, nursing profession has to formulate a forward looking, long term development strategic plan to nurture talents. The Hong Kong Hospital Authority has delineated a dual career path of clinical and management streams for nurses. With a 5-phase approach, it starts off with grooming fresh graduate on knowledge consolidation and skill application to eventually becoming an expert and leader with eye-opening and network broadening exposure. This enables the nursing profession to take on a leadership role in the healthcare system. This is a step-by-step incremental pathway that builds on a firm foundation from the previous phase for life-long learning.

In this paper, the author introduced the definition of care quality, how to set up the care quality indicators, how to use scientific methods in collecting and analyzing data to find the reasons why the quality indicators did not achieve the ideal level, and how to implement interventions to continuously improve the care quality.

With the rapid advancement of medical knowledge and technology, division of work between the medical and nursing care becomes finer and finer. Adding the harmonious coordination of these two fields to that, the health care service becomes more and more perfect, satisfying the needs of the patients and the requirements for a thorough nursing care. Orthopedic is reflected in holistic care. The patient is taken care from the onset of an emergency case to rehabilitation, from being at the hospital up to his return to the community. In addition to the medical specialist, orthopedic specialist nursing support for the patient's care must not be missing. To maintain the professional standard of orthopedic care, the most important is the need to receive high-quality orthopedic nursing care training to keep up with the pace of scientific advancement. Orthopedic specialty nurses receiving over the years a systematic training from the foundation to specialist level should have their knowledge and skill more solid and thorough.They should have their professional mission and confidence to make them sensitive to the service needs innovation. Hong Kong orthopedics specialty nurses carry out cross-boundary advisory services and patient care in outpatient clinics, demonstrating the mature development of the orthopedics care. To offer better service to the internal partner (doctors) and the external customers (patients), orthopaedic nurses must equip themselves with the up-to-date knowledge and skill that patients will recover from disable eventually.

Construction and practice of online hierarchical information evaluation platform for nurses

Objective: To build the online hierarchical information evaluation platform for nurses, and to improve the nursing management efficiency. Methods: Several measures were taken to set up continuous improvement in the hierarchical online examination of nurses. By using the method of expert argumentation, we determined the structure of theory examination of nurses in different levels, and established a question bank packed with different specialized subjects and hierarchies. We also pushed on the online examination platform by different stages and strengthened the feedback system after examination.Results: The structure of theory evaluation for nurses in different levels was confirmed and an online examination question bank was established with a total of more than ten thousand test questions which contains the questions from different specialties subjects. All nurses in the whole hospital could share the layered online assessment. And according to the feedback, overall satisfaction of nurses was 98.5%.Conclusion: With continually updating and perfecting, the question bank can ensure the quality of the hierarchical examination and the nurse training could be more effective. The cost of the management could be reduced to a certain extentand the work efficiency is also improved. This method is very convenient for clinical nurses to arrange their time. With continuous improvement in the effective feedback mechanism and test management, the nurses’ satisfaction could improve greatly.

Bibliometrics analysis of PICC-related venous thrombosis in China from 2004 to 2013

Objective: To systematically analyze the research trend of PICC-related venous thrombosis in China, so as to provide evidence for future studies. Methods: Articles about PICC-related venous thrombosis published from 2004 to 2013 were retrieved from CBM, CKNI and Wanfang databases, and were analyzed systematically.Result:Altogether 210 related articles published in 91 journals were retrieved. Authors were located in 29 provinces and most were from medical institutions. Most articles were about clinical research, but only 2.86% were randomized controlled trials. Most research focused on case report and analysis,and prevention and management of PICC-related venous thrombosis. Their references were mainly Chinese literature (70.37%), and 9 papers were cited more than 10 times. Nevertheless, only 5.71% of them were supported by research grants. Conclusion: PICCrelated venous thrombosis has attracted wide concerns by clinical staff. High-quality research papers and PICC-related thrombosis management standard are needed, so as to provide more powerful evidences for PICC-related venous thrombosis prevention.

Investigation and analysis of medication adherence and influencing factors among Acute Myocardial Infarction patients with coronary stent implantation

Objective: To observe the change trend and status of medication adherence among patients with Acute Myocardial Infarction (AMI) after coronary stent implantation, and analyze the influencing factors in different periods. Methods:Totally 126 AMI patients with coronary stent implantation were conveniently sampled in this study. There were 4 times' telephone visit at 3, 6, 9 and 12 months after coronary stent implantation. The investigation tools included the medication adherence questionnaire, the Social Supports Rating Scale (SSRS), and the Charlson comorbidity index. Results: The medication adherence level of the AMI patients with coronary stent implantation was relatively low, and it was obviously decreased over time. The positive factors of medication adherence were social support, age, and the education level; while the negative factors included the Charlson comorbidity index and non-local residence. Conclusion: The medication adherence status was not optimistic among AMI patients with coronary stent implantation. Interventions should be implemented according to the influencing factors in different periods to improve medical adherence of AMI patients with coronary stent implantation.

The effect of work-family conflict on turnover intention among nurses in pediatric outpatient department

Objective: To explore the turnover intention and work-family conflict among nurses in pediatric outpatient department and the effect of work-family conflict on turnover intention, and to provide references to prevent nurses' turnover.Methods: A total of 243 staff nurses in pediatric outpatient department from 13 tertiary Grade A general hospitals in Shandong province were investigated with Work-Family Conflict Scale and Turnover Intention Questionnaire. Results: The mean score of work-family conflict was (2.31±0.58) and 47.7% of nurses had turnover intention. Logistic regression indicated that age, overtime work, base on pressure conflict and base on behavior conflict were the influencing factors of turnover invention. Conclusion:Turnover intention among nurses in pediatric outpatient department is high and it is influenced by work family conflict. Hospital managers should take effective measures to reduce job pressure and work family conflict to decrease turnover rate among nurses in pediatric outpatient department.

Objective: To establish clinical care quality standard index system of Vacuum Sealing Drainage treatment of soft tissue defects infected wound and to explore its rationality and operability. Methods: Delphi expert consultation method was used to establish quality standards for nursing index system. Results: Nursing quality standard included 5 Grade I indexes and 40 Grade II indexes. The experts had high initiatives and the authority coefficients were 0.92-0.94. The consistency of experts'opinion was good. Conclusion: The experts’ opinions of nursing quality index system have high consistency and credibility. The results could provide an objective and quantifiable basis for Vacuum Sealing Drainage treatment of soft tissue defects infected wound.

The effect of transitional care on the anticoagulant therapy compliance among discharged patients undergoing cardiac valve replacement surgery

Objective: To explore the effect of transitional care on the anticoagulant therapy compliance among discharged patients undergoing valve replacement surgery. Methods: We recruited 121 patients undergoing cardiac valve replacement surgery in the cardiothoracic surgery department of our hospital from May 2010 to May 2013, and divided them into control group (n=61) and experimental group (n=60), respectively, according to their discharge order. All of the patients received routine discharge guidance. The control group adopted routine outpatient follow-up and recheck, and the experimental group received a telephone follow-up of specific disease and related health education, and received short messages about disease knowledge and medication reminders through short message platform provided by full-time nurse. We compared the anticoagulation knowledge,the anticoagulant therapy compliance, and the complications caused by improper anticoagulation between the two groups 3 months, 6 months, and 12 months after discharge by questionnaire. Results: There were significant differences in each of the evaluation index between the two groups 3 months, 6 months, and 12 months after discharge (P <0.05). Conclusions: Transitional care interventions among discharged patients with cardiac valve replacement surgery could meet their health needs, improve the anticoagulant therapy compliance, and reduce the incidence of complications caused by improper anticoagulation.

The knowledge and behaviors of oral care among elder people with removable partial denture

Objective: To investigate the knowledge and behaviors of oral care among elder people with removable partial denture (RPD). Methods: Totally 102 elder people with RPD were investigated with a self-designed oral care knowledge and behavior questionnaire. Results: Regarding the cleaning of natural teeth, 27.45% and 20.58% of the elder people knew and cleaned natural teeth after each meal, respectively.About 13.73% cleaned the natural teeth by dental floss or oral irrigator and only 6.86% mastered the correct teeth brushing method. Regarding the cleaning of artificial teeth, 33.33% elder peoplecleaned the artificial teeth after each meal correctly and 46.08% didn't master the correct cleaning method. About 18.63% didn't remove the artificial teeth at nightand only 7.84% knew there-examination time. The elderpeople obtained oral care knowledge from dentists or nurses (55.88%) and television or broadcast (43.14%). Conclusion: The elder people with RPD lack of correct oral care knowledge and their oral care behaviors are incorrect. We suggested to enhance health education in the clinic to improve elder people's oral care knowledge and behaviors.

The effects of applying pre-hospital management model among patients before eye surgery

Objective: To explore the effects of applying pre-hospital management model among patients before eye surgery. Methods: The patients were recruited by the convenience sampling method. The control group consisted of 300 patients from a tertiary hospital in Beijing who received the traditional care from October 2011 to March 2012. From April to September 2012,another 300 patients from the same hospital were recruited as the experiment group and cared by pre-hospital management model. The control group received the conventional hospital operation management, while the experimental group received the pre-hospital management model besides the traditional care. Results: After the application of pre-hospital management model, the surgical knowledge and self-appraisal on the hospitalized operation in the experimental group were significantly better than those in the control group (P﹤0.01). The rate of the cancelled operation was reduced from 3.77% to 2.78%. The average pre-operative hospitalized length were shortened by 0.1 day. Conclusions: The pre-hospital management model could provide continuous care to the pre-surgical patients before their hospitalization. This model is able to improve the patients' compliance of self-medication and self-care as well as the adaptation and cooperation to the surgery and treatment during the operational waiting period.

Design of nursing students′ second class based on taking the development of humanistic caring capacity as core

Objective: To explore the practice and effects of second class on improvement of nursing students′ humanistic caring capacity. Methods: Guided by theoretical study, a series of second class was designed and implemented in grade 2011 nursing class. Results: Nursing students′humanistic caring capacity was greatly improved after the activities (P<0.001). The top three popular activities were clinical practice, skill competition and TV play review. The students reported that those activities integrated theory with practice, and contributed to improve students' learning interest and capacity. Conclusion: Practice of second class could effectively improve nursing students′humanistic caring capacity, and help them master theoretical knowledge and skills.

Implementing combination of Problem-originated Clinical Medical Curriculum with Case Based Teaching in training of junior nurses

Objective: To evaluate the effect of junior nurses training by combination of Problem-originated Clinical Medical Curriculum (PCMC) with Case Based Teaching (CBT). Methods: Totally 96 junior nurses rotating in the Infections Department were selected and randomly divided into two groups (control group and experimental group, 48 cases in each group). The nurses in experimental group were trained by combination of PCMC with CBT, while the nurses in control group were trained with traditional methods. After 2-month training, nurses' examination score, their evaluation of training methods, and patients'assessment of nursing were compared between groups. Results: These three parameters in experimental group were significantly higher than those in control group (P<0.01). Conclusion: Combination of PCMC with CBT could improve comprehensive capability of junior nurses effectively; improve training quality and patients' satisfaction to nurses.

The relationship between social support and quality of life of nurses in Guangdong Third-grade Class-A Hospital

Objective: To investigate the current situation of social support and quality of life among nurses in Guangdong Third-grade Class-A Hospitals, and explore the relationship between social support and quality of life. Methods: Social support rating scale and WHO quality of life summary questionnaire were used to measure the level of social support and quality of life of nurses and 1851 nurses in five hospitals of Guangdong Province were investigated. Results: The average score of social support was 38.37±7.91, which was significantly higher than the score of the norm (P<0.05). Among the three dimensions of social support, subjective support score is the highest one in clinical nurse, followed by the objective support, the utilization of support.The average score of quality of life was 76.96±11.96, which was significantly higher than the score of the norm (P<0.05). The highest score of four dimensions was physiological dimension, followed by environmental dimension, psychological dimension,and social relationship. Nurses' social support was positively correlated with their quality of life (P<0.05). Conclusion: The levels of social support and quality of life of nurses in Guangdong Province were moderate. By strengthening the nurse's social support system, creating a good working environment, it will help to improve the overall quality of life of nurses.

Investigation of the status of emotion management in emergency nurses and its possible influencing factors

Objective: To investigate the status of emotion management ability in emergency nurses and to analyze its influencing factors. Method: Convenience sampling was used and 182 emergency nurses from grade III and first-class hospitals in Qingdao City were investigated with self-designed questionnaire of emotion management of nurses in emergency department from October to November 2013. Result: The overall score of emotion management ability of nurses in emergency department was (49.59±4.83), and the dimension scores from high to low was emotional adjustment (3.57±0.48) , emotional awareness (2.84±0.32), emotional using (2.79±0.37), and emotional expression (1.89±0.30). Multi- regression analysis indicated that job burnout and emotional transfer were the influencing factors of emotion management for nurses in emergency department.Conclusion: The overall emotion management ability in emergency department nurses is in the middle level. It is suggested that emotion management should be introduced into nursing field. Emergency department nurses should be trained according to their different characteristics to improve the level of emotion management ability.

The current status and influencing factors of workplace violence among outpatient nurses

Objective: To describe the current status and influencing factors of workplace violence among outpatient nurses in tertiary general hospitals. Methods: Totally 204 nurses in outpatient department from 8 tertiary general hospitals in Zhejiang province were recruited and investigated by the Modified Version of Hospital Workplace Violence Questionnaire. Results: The overall incidence rate of workplace violence among 204 participants was 74.4% in the past year. Multivariable logistic regression analysis showed that work position, from one-child family, positive coping style and the effective prevention measures for violence that hospital take entered the regression model. Conclusions: The incidence rate of workplace violence among outpatient nurses was high. The influencing factors of workplace violence among outpatient nurses were work position, from one-child family, positive coping style and the effective prevention measures for violence that hospital take. In the future we should conduct the related intervention research to decrease the incidence rate of workplace violence among outpatient nurses.

Analysis and countermeasures among 83 cases of nursing adverse events

Objective: To explore the categories, reasons and characteristics of nursing adverse events, and to provide references for drawing up nursing countermeasure. Methods: We analyzed 83 adverse events reported to nursing department in the past three years using nursing quality management tool. Results: We could reduce the occurrence of nursing adverse events by paying close attention to the main risk factors and events, strengthening the process supervision, and improving working flow.Conclusion: We should pay more attention to the high-incidence period and crowd, put forward the pointed prevention measures,and to ensure nursing safety.

The current status and influencing factors of frailty among elders in community

Objective: To investigate the current status of frailty among elderly in community and to identify the influencing factors of frailty. Methods: A total of 683 elders in eight communities from two districts were recruited by convinience sampling method and investigated by the questionnaires and physical assessment. Results: The prevalence of robustness, pre-frailty, and frailty were 43.2%, 45.7%, and 11.1%, respectively with grip weakness and low physical activity as the main frailty problems.Male, increasing age, urinary incontinence, poorer self-report health, depression, cognitive impairment, and poor sleep quality entered the multiple logistic regression equation. Conclusions: The prevalence of frailty is worth drawing attention. The frailty of elders in community is influenced by many factors and we should take multifaceted interdisciplinary intervention.

The nurses' willingness of mobile nursing workstation and its influencing factors analysis in Xinjiang area

Objective: To explore nurses' willingness of mobile nursing workstation in Xinjiang area, in order to put forward relevant countermeasures. Method: The self-designed questionnaire about status of mobile nurse workstation was adopted, and 1068 nurses in hospitals with grade III in Xinjiang field were investigated. Result: The scores of using willness of mobile nurse workstation were ranged 16-80 (65.86±9.21), which showed as an average level; Single factor analysis showed that different age,gender, department, position, title, the time of participate in the work patients in charge, training frequency were statistically different in scores (P<0.05); Multiple stepwise regression analysis showed gender, department were slected into regression equation. Conclusion: The nursing staff will be more willing to use mobile nurse workstation predominance function section;Nursing managers should take corresponding measures to improve the use of mobile nurse workstation care according to the national minority area characteristic.

Results: The information platform with practicality was easy to operate, and provided higher quality information material of the cancer cases to ensure information integrity and correctness. Conclusion: The development and application of information technology provide a guarantee for the carrying out of post-discharged continuity of care information platform for cancer patients, as well as providing a basis for cancer research.

Effect of fall risk management of hospitalized psychiatric patients

Objective: To explore the effect of fall risk management in preventing falling of hospitalized psychiatric patients and reducing the damage impact of falling. Methods: Totally 4647 psychiatric patients hospitalized from July in 2011 to July in 2013 were participated as observation group, while psychiatric patients hospitalized from June in 2009 to June in 2011 as control group. Risk management methods were used in observation group. Results: The damage degree in grade I, II, III between two groups had significant difference (P<0.001). The incidence of falls between two groups had no significant difference (P>0.05), however, the reporting rate about no hurt fall event between two groups had significant difference (X2=12.392, P<0.001). Patients’compliance of fall risk management between two groups had significant difference (P<0.01). There was no complaint and nursing dispute in observation group, while there was one case of fall-related nursing dispute in control group, resolved through economic compensation. Conclusion: Fall risk management of hospitalized psychiatric patients can promote the quality awareness and risk awareness of nursing staff, and improve their ability to prevent falls. It also can promote the patients to participate in fall prevention management, reduce and mitigate the serious injury caused by falls, improve the nursing quality and ensure safety of patients.

Objective: To evaluate the effects of mind map combined with motivational interview on young diabetes patients'self-management. Methods: The 120 young diabetes patients were randomly divided into two groups: experimental group (n=60)and control group (n=60). Experimental group received education management of motivational interview combined with mind map,while control group received routine diabetes education. We evaluated the difference from effects of mastering diabetes knowledge,self-management behavior score and biochemical indicators between the two groups respectively. Results: The effects of mastering diabetes knowledge, self-management behavior score and biochemical indicators in the experimental group were better than in control group, and the difference was statistical significant (P<0.05). Conclusion: Motivational interview combined with mind map,which can improve clinical indexes of young diabetes patients, is a good method and tool to improve patients' quality of life.

Objective: To improve nurses' intention to stay. Methods: Construction of support system between nursing colleagues, hierarchical training, psychological support, "Mentors system for nurses within three years", "Secret Angel in action"were carried out to improve nurses' intention. Results: After two years of implementation, we compared nurses' intent to stay and nurse satisfaction to the nursing work before and after implementation, and the difference was statistically significant (P<0.01).Nurse's turnover rate reduced from 7.8% in 2011 to 3.6% in 2013. Conclusions: The construction of colleague support system may improve the nurses' job satisfaction and nurses' intention to stay, and reduce their turnover rate.

The authors reviewed the studies related to professional identity of domestic nursing students in this paper.They summarized that nurse students had a medium or even lower level professional identity in most studies, and gender, education level, grade, practice status and family background were associated with their professional identity level. So strengthening education on nurse students' professional identity is critical nowadays, and teaching hospital should take more responsibility on it.

Pamela Youde Nethersole Eastern Hospital is the leading hospital in the East Network of Hongkong, which pays more attention on implementing effective method to solve problems. They applied Situation Background Assessment Recommendation (SBAR) to manage the communication problems between physicians and nurses. Crew Resources Management(CRM) was used to improve medical safety management, staff's capacity to face emergency critical events, and team collaborative abilities. They also organized love transfer activities to pass their caring culture to staffs and patients. All of these methods were effective to improve communication and collaboration among staffs, and build a humane working atmosphere,which provides reference for nursing managers in Mainland China.
