Establishment and verification of a prediction model for neonatal hypothermia in intra-hospital transportation

Abstract: Objective: To explore the risk factors of neonatal hypothermia in intra-hospital transfer and to establish a predictive model. Methods: A retrospective study was conducted to collect the data, and 4200 newborn infants in one hospital in Shanghai from April 2020 to April 2021 were included in this study. The Logistic regression analysis was used to identify the hypothermia-related risk factors, and a nomogram prediction model for neonatal hypothermia risks in intra-hospital transport was established and verified based on risk factors. Results: A total of 2368 newborn infants (56.4%)were observed hypothermia in intra-hospital transfer. The 12 neonatal-related factors with hypothermia in intra-hospital transportation included the gestational age, birth weight, 5-minute Apgar score, whether with skin-to-skin contact, head caps, wiping and drying, early breastfeeding, transport incubators, tracheal intubation, cardiopulmonary resuscitation and asphyxia, and the bathing time; the 4 obstetric related factors included the gestational hypertension, time of delivery, type of delivery and chorioamnionitis. The discrimination (C-statistic=0.930) and calibration (Hosmer-Lemeshow goodness-of-fit test, χ2=6.542) of the model were indicated well. Conclusion: The incidence of neonatal hypothermia in intra-hospital transportation is high. This nomogram for neonatal hypothermia in-hospital transport has good predictive ability and discrimination, which can be used for early detection of high-risk newborns with hypothermia in intra-hospital transport.
