Abstract: Rapid growth in the development and use of Advanced Practice Nursing (APN) roles is expected to occur over the next decade. Situating the clinical responsibilities of these roles within models of care delivery designed to achieve improved patient and health systems outcomes will be important for ensuring their successful implementation and impact. Transitional care models are one approach to leveraging the clinical experience and expertise of nurses in advanced roles. The purpose of this paper is to describe the need for and define transitional care followed by a summary of the target patient populations, characteristics and outcomes of advanced practice nurse-led models of transitional care. Strategies senior nurse managers and leaders can use to develop and evaluate new models of transitional care involving APN roles will be provided.
