Abstract: Objective: To analyze risk factors of the community elderly people's fall within one year, to validate the accuracy of the modified fall risk assessment tool for elderly people in communities, and to provide prevention measures. Methods: In March and April 2016, geriatric specialist nurses and community workers used a general information questionnaire and the modified fall risk assessment tool to investigate the older adults in two communities, which were randomly selected. Results: The fall rate of the community elderly people in the recent year was 22.01%. The risk factors of falls were age65, foot problem, incorrect activity ability assessment and unsafe living environment. The accuracy of the modified fall risk assessment tool was validated. Conclusion: Fall prevention of community elderly people is always the focus of transitional care and community nursing. Geriatric specialist nurses and community workers should develop prevention measures according to the risk factors to avoid falls.
