Status and influencing factors of dyadic appraisals congruence between patients with Chronic Heart Failure and theirs caregivers

Abstract: Objective: To investigate the status and influencing factors of dyadic illness appraisals congruence between patients with Chronic Heart Failure and their caregivers. Methods: A total of 198 patients with Chronic Heart Failure and their caregivers were recruited from the cardiology department of a tertiary grade A hospital in Wuhan from August 2021 to September 2022. Patients and their caregivers were investigated with a general information questionnaire, the Memorial Symptom Assessment Scale, Dyadic Coping Inventory, the Revised Piper's Fatigue Scale-Chinese Version, and Self-Concealment Scale. Results: There was a significant difference in psychological symptoms between patients with Chronic Heart Failure and caregivers in dyadic illness appraisals (P<0.05) and the caregiver score was lower than the patient score. Multiple linear regression results showed that patients' binary coping, self-concealment, number of comorbidities, gender, and fatigue level of caregivers were the main influencing factors for dyadic illness appraisals congruence. Conclusion: Caregivers underestimated the severity of patients' psychological symptoms. Medical professionals should encouraged patients to take the initiative to disclose, and conduct communication and empathy training for caregivers to improve the consistency of dual disease evaluation for those patients with dual coping passivity, high level of self-concealment, and a large number of comorbidities, as well as male and high level of fatigue caregivers.
